Neither one is better. They fix different problems, and asking whether Sculptra or PDO thread is better usually means a patient hasn’t yet worked out which problem they have.
Threads move tissue. A biostimulator adds substance. If your cheek has slid downward, more collagen won’t put it back. If your face has gone flat and thin, pulling on it won’t refill it.
That’s the whole decision. It takes about four minutes of looking at a face to make.
I’m skipping pricing here, and skipping body applications like buttock volume, because the reasoning changes when you’re treating a limb instead of a face.
Sculptra, a poly-L-lactic acid, is a collagen biostimulator injected under the skin. It adds volume slowly as your body builds collagen around the particles. A PDO thread lift places polydioxanone sutures beneath the skin to reposition sagging tissue. Threads lift on day one. Sculptra lifts nothing and builds substance over months.
The carrier fluid holding those particles absorbs within days. The poly-L-lactic acid stays behind and breaks down slowly, which is why nothing looks different at week one. Threads run the opposite way. The lift shows immediately, then the polydioxanone absorbs over six to nine months while collagen along the thread tracks carries part of the result forward.
Polydioxanone has served as absorbable surgical suture for decades, so the material itself was never the controversial part.
Popularity says nothing about fit. The category that includes Sculptra reached 932,861 procedures in 2024 per the ASPS annual report, up 1% year over year. That number pools several non-hyaluronic acid products and draws on member and non-member reporting. Thread lifts get no standalone line in that report at all.
Threads are for position. Sculptra is for volume. Get that backwards and you’ll spend a year chasing a result that was never on the table.
Picture the first patient. The jowl sits below the jaw line, the cheek fat pad has dropped, a fold is forming beside the mouth. Pinch the skin and it snaps back. Tissue moved, so move it back.
Now the second. Hollow temples, flat cheeks, skin thin enough that light passes through it oddly. Nothing fell. Substance left. Threads have little to grip here, and the little they grip looks pulled.
Most faces past 45 sit here, and the order stops being a preference. Volume goes first because threads need something to hold. Reverse it and you spend the lift on tissue that was too thin to keep it.
Plenty of people arrive convinced they need a lift when what they’ve lost is fill. Tightening a deflated face doesn’t make it younger. It makes it read as work, and the best result is the one nobody can identify. Add substance first, then talk about a facelift without surgery.

Both sit in the minor-complication category. Both carry one problem worth knowing before you book.
A systematic review in Dermatologic Surgery pooled nineteen studies covering 1,406 thread lift patients and counted 876 complications. Bruising came in at 19.49%, swelling at 16.79%, tenderness at 9.96%. That review combined absorbable suture materials, textures and fixation methods while excluding nonabsorbable sutures, so the rates describe absorbable thread lifting broadly rather than polydioxanone alone. Dimpling and asymmetry sit lower in those tables and higher on my list, because both track with thread count.
For Sculptra the issue is nodules. Small firm lumps can surface months later, long after a patient has stopped thinking about the injection.
And you can’t take it back. No enzyme removes poly-L-lactic acid. Hyaluronic acid is the forgiving material here, which is why whether jawline filler dissolves has a clean answer and biostimulators don’t.
Sculptra builds for months and holds around two years. Thread results appear the same day and soften well before that.
The clearest published evidence for Sculptra longevity is the trial behind its cheek approval, where 110 patients were graded and 96% improved at three months, with 94% still improved at one and two years. Read that carefully. It covers fine lines in the cheek, graded at rest, in one manufacturer-sponsored study. Cleveland Clinic puts the practical ceiling at more than two years while noting the result isn’t permanent.
Threads are harder to promise. The polydioxanone absorbs over six to nine months, and honest outcome data past the first year stays thin across the literature. Anyone quoting a firm two-year number for threads is quoting a brochure. How long a thread lift lasts depends more on the laxity you started with than on thread count.
For a jawline, usually neither wins alone. A jawline is a frame problem before it’s a skin problem.
Patients ask me for a superhero jawline. That’s a request about frame, and frame comes from chin projection and jaw angle. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, do structural work at the chin that neither option here can copy.
Jawline skin holds a thread lift best in roughly the 25 to 55 window, while recoil is still there to work with.
Bands in the neck change the answer completely. Vertical cords running down from the jaw are platysma muscle, and a dissolving thread can’t hold muscle in a new position for long. That’s the case for a nonabsorbable, removable suspension suture placed alongside platysmaplasty. For patients whose lower face still responds to repositioning, PDO thread treatment in Tampa is where I start.
Yes, and for most faces past 45 it’s the better plan. Volume first, lift second.
Threads need tissue to grab. A flat, thin face gives them less to work with, and the lift ends up looking stretched instead of restored.
Separate the two by weeks rather than stacking them in one visit. Swelling from one hides the result of the other.
Then there’s the part that has nothing to do with either product. We assess health and diet first, then adjust nutrition, supplements and peptides right before and after, so the body builds as much structure as it can. The thread and the syringe are identical at every practice in this city. The thinking around them is not.
Age comes up constantly, and the best age for threads has less to do with the number than with what your skin does when you let go of it.

Sculptra is FDA approved. Most PDO threads are FDA cleared. Those sentences don’t mean the same thing.
Sculptra’s original approval in 2004 covered facial fat loss in people with HIV. A 2009 supplement added nasolabial folds and other facial wrinkles for immune-competent patients, and cheek fine lines followed in 2023.
Threads took a different route. Most polydioxanone products carry clearance as absorbable sutures for soft tissue approximation, which is wound closure language. At least one device does carry clearance for mid-face suspension, holding the cheek fat and SMAS layer in an elevated position. So the blanket claim that all thread lifting is off-label is outdated, and the claim that threads are FDA approved for lifting is wrong.
Clearance means a device is substantially equivalent to one already sold for a stated use. It isn’t a trial proving your face will lift. Off-label use of a cleared device is legal and routine. Saying so out loud is the part most ads skip.
Three questions separate a good provider from a confident one, and none of them are about the product.
Ask which thread type or which biostimulator, and what its regulatory record says for the exact use you’re getting. If the answer is a flat “it’s FDA approved,” ask for the cleared indication. Anyone who places these regularly can tell you without checking.
Ask how many they’ve done in the specific area you care about. Not how many faces. How many jawlines, or how many midfaces. Both of these are technique-dependent in a way that a neuromodulator is not, and volume in the wrong region tells you nothing.
Then ask what happens if you get a nodule or a dimple, who fixes it, and when. A provider who says they’ve never had one either hasn’t placed many or isn’t following patients long enough to find out. I’d rather hear the number and the plan. Reading up on the downside of threads before that conversation makes it a much shorter one.
Board status takes about a minute to verify and it’s public. Do it anyway.
If your skin hangs at rest, neither Sculptra nor a thread lift will reach it.
Do this check. Lift the skin along the jaw and let go. If it drops straight back into a fold that sits there while your face is relaxed, that’s excess skin. Threads pull against it and the illusion fails within months.
Saying no to those patients is part of the job. I send them to surgeons who do skin excision well and stay in contact with them. Where the neck and jawline are the issue, I do the liposuction and skin tightening myself.
Anyone needing a visible change in two weeks for an event has no business starting Sculptra. A history of nodules or active inflammatory skin disease gets aired first.
One patient in his forties, fit for years, could not get his neck to respond to anything. Threads were never the answer, and saying so on day one saved him a wasted round.
Position or substance. Every other detail on this page hangs off that one call, and you can’t make it from a screen.

For jowls specifically, a PDO thread lift usually does more, because a jowl is tissue that has changed position rather than tissue that has thinned. Sculptra can support the area by rebuilding volume in the cheek above. If the jowl skin no longer springs back when you release it, neither treatment will hold.
No. Sculptra is a collagen biostimulator, so it adds substance where volume has been lost. Any apparent lift is the visual effect of a fuller cheek sitting higher on the face, not repositioned tissue.
Sculptra is given as a series rather than a single treatment, spaced weeks apart, because the collagen response builds gradually. The number depends on how much volume has been lost. Patients who expect a finished result from one visit are usually the ones who end up disappointed.
Possibly. Scar tissue can form along the tracks where threads were placed, and that changes the tissue planes a surgeon works through later. It is a fair thing to raise with anyone who may want surgery within a few years, and it belongs in the consent conversation.
Rapid weight loss usually takes volume rather than position, which points toward a biostimulator. Threads help only if laxity has also appeared. Many patients in this group need both, in that order.
Most polydioxanone thread products are FDA cleared as absorbable sutures for soft tissue approximation, which is wound closure. At least one device carries clearance for mid-face suspension. Clearance is not the same as approval, and any clinic advertising threads as FDA approved for lifting is overstating the record.
The polydioxanone breaks down over roughly six to nine months and leaves nothing behind but the collagen your body laid along the thread tracks. That collagen carries part of the result forward, though not the full mechanical lift you saw on day one.
