The main downside of PDO thread lift is simple. It skips surgery, but it does not skip risk. Most side effects are mild and short. A few are not.
Swelling, bruising, and tiny skin dimples are common. Thread migration, threads you can see under thin skin, and infection show up less often, but they do show up. Here’s the part most clinics gloss over. Clinical data collected by The Dermatologist puts the reported complication rate in published studies above 30 percent. That number sounds alarming until you learn most of those complications are minor and fix themselves.
So what is the downside of PDO thread lift, in one breath? It’s a set of possible side effects. Swelling, bruising, skin dimpling, thread migration, visible threads, infection, and rarely nerve irritation. Most clear up on their own within a few days to two weeks. A small share need a quick correction from your provider.
I’ve placed and removed enough of these threads at Formation in Tampa to give you the honest version, not the brochure version.
Smooth PDO threads are thin, dissolvable sutures placed just under the skin to build collagen. They don’t lift. They firm.
PDO stands for polydioxanone, the same material used in surgical stitches for decades. A provider places the threads in a grid or fan pattern in the layer just below the surface. Your body absorbs them over about four to six months. As they dissolve, they leave new collagen behind, so skin looks a little firmer and texture smooths out. Downtime is short. Results are quiet, not dramatic. At Formation we mostly use them for early lines on the face and mild crepey skin, not heavy sagging.
One more thing worth knowing about the material itself. Polydioxanone has a long safety record because surgeons have closed wounds with it for years, which is a big reason smooth threads sit on the lower-risk end of injectables. The risk isn’t the material. The risk is depth, placement, and volume, all of which come down to technique. That’s also why two people can get the same treatment and walk away with completely different experiences. Same threads, different hands.
These are two different tools for two different problems.
Smooth threads are straight, with no hooks. Their whole job is collagen. Barbed threads have tiny hooks along the length that grab tissue and physically lift sagging cheeks, jowls, or the neckline. Barbed give you a visible lift right away. Smooth give you a slow firming over weeks. If someone tries to sell you smooth threads as a facelift, walk out. They were never built for that.
The downside of PDO thread lift is the list of things that can go wrong, from common and mild to rare and serious. Here’s the real list, ranked roughly by how often I actually see each one:
Now the perspective. The scariest-sounding item on the list, nerve damage, is also the rarest. The one you’re statistically most likely to deal with is a dimple. Plan your worry accordingly.
Barbed threads carry more risk than smooth ones. They go deeper, pull harder, and leave more room for a mistake.
Smooth threads mostly stay in the mild and short lane. Barbed threads are where most of the rough before-and-after photos online actually come from. If your goal is texture and a subtle firming, smooth is the safer buy. If you want to lift a sagging neck, barbed threads may be the tool, but the risk math changes and you want an experienced hand.
Most PDO thread side effects last a few days to two weeks. A few outliers run longer. Here’s the typical arc:
If a dimple, a hard lump, or a visible thread is still there after a few weeks, that’s your signal to go back in. Don’t wait it out. Migration and extrusion don’t resolve on their own.
The biggest factor in your risk isn’t the thread. It’s the hands placing it.
Threads look easy on social media, which is exactly the problem. In some markets, these procedures are even performed by nonmedical practitioners, and that’s where the ugly complications cluster. Facial anatomy is unforgiving. Nerves and vessels sit millimeters from where the thread goes. Even with a complication rate that reads high on paper, Healthline puts the minor, correctable share at roughly 15 to 20 percent of procedures, and that share shrinks further with a skilled provider.

I turn away more thread candidates than I treat, and nobody’s upset about it a year later. Better to hear you’re not a fit than to pay for a result that was never on the table. Set expectations before your first session, not after. A good provider will tell you how many threads your face actually needs, how long the firming should hold for your skin, and what a touch-up plan looks like down the road. If you leave a consult without those three answers, you booked with the wrong person.
For the right person, yes. For a lot of the people who ask me, no.
Smooth threads earn their place if you have early, mild laxity, you want a subtle firming, and you accept that results soften as the threads dissolve. They’re not worth it if you have real jowls or a heavy neck. At that point threads underdeliver, and you’ve paid for a result that won’t hold.
Actually, let me reframe that. It’s not that threads don’t work. It’s that they keep getting sold for jobs they were never designed to do. That mismatch is what creates unhappy patients, not the technology.
For skin laxity that threads can’t handle, radiofrequency skin tightening usually does more and holds longer. I’m not going to get into pricing here, because it swings too much by area, thread count, and provider for any number I quote to be useful. What I’ll say plainly: for fine lines and texture in your 30s and 40s, smooth threads are a fair trade. For gravity, they’re the wrong tool.
The real downside of PDO thread lift isn’t any single side effect on the list. It’s getting the treatment from the wrong hands, or for the wrong problem.
Pick a medically trained provider. Walk in with clear expectations about what smooth threads actually do. Do both, and this stays a low-risk way to firm up early aging skin with barely any downtime. Miss on either one, and you join the crowd posting about dimples and threads poking through. That’s the whole story.

Yes. Barbed threads carry more risk than smooth ones because they go deeper and physically lift tissue. That means a higher chance of asymmetry, migration, lumps, more bruising, and a longer recovery. Smooth threads mostly stay in the mild, short-lived range.
Most can. Dimpling, bruising, and swelling usually resolve without treatment. Migration, extrusion, and visible threads often need a provider to reposition or remove the thread, which is a quick in-office fix in most cases.
People with heavy jowls or a sagging neck, active skin infections, very thin or lax skin, a known PDO allergy, or slow-healing conditions should skip smooth threads or address the issue first. Anyone expecting a surgical facelift result is also a poor fit.
