Wait 24 hours. Then wash with a fragrance-free, low-irritant cleanser, fingertips only, under lukewarm water. That’s the whole rule.
The shampoo to use after PRP hair treatment is any mild, fragrance-free cleanser built on a low-irritant surfactant base, introduced about 24 hours after your session. Apply it with fingertips rather than nails. Skip medicated actives, dry shampoo, and hot water for the first three days.
I’m Dr. Gopal Grandhige, a board-certified general surgeon and the owner of Formation in Tampa. Jeff Atlas, PA-C, performs our scalp injections. He spent years in orthopaedics, where platelet-rich plasma treated tendons and joints long before it reached hairlines.
I won’t cover finasteride dosing or transplant technique here. This is about the week after your injection. Candidacy is a separate question, and being a good candidate depends on how much living follicle is left.
PRP puts your own concentrated platelets into the dermis of the scalp, where growth factors signal dormant follicles and improve local blood supply. It’s injected, not applied.
That distinction decides your washing routine. Water can’t rinse out something sitting under the skin. A 2025 systematic review in Dermatology and Therapy pooled 43 randomized trials covering 1,877 patients and found activated PRP raised hair density against placebo by roughly 24 hairs per square centimeter. It didn’t move hair thickness. Density improves; strands don’t get fatter.
This works for early to moderate pattern loss. If your crown is smooth and bare, PRP won’t fill it, and I’ll point you toward transplant options. That answer belongs in the first consultation, not after three sessions.
Hallmarks addressed: stem cell exhaustion (follicular stem cell and dermal papilla signaling), altered intercellular communication (growth factor and Wnt pathway activity), and chronic inflammation (perifollicular microinflammation).
Do less than you think. Puncture sites close within hours. Leave them alone.
Scratching and rubbing cause more trouble on day one than any shampoo could. Sites can feel tender, tight, or slightly swollen. Mild redness and pinpoint marks are expected. The same restraint applies the first time you comb your hair.
Twenty-four hours, in most cases. The old 48-hour rule survives because clinics copy each other, and it rests on a wrong idea: that washing rinses the PRP away. It doesn’t. Waiting a day gives micro-punctures time to seal, which lowers irritation and infection risk. When Jeff combines PRP with microneedling, we extend the window, because there are far more channels in the skin.
Not for three days. Gels, mousse, texturizing sprays, and dry shampoo sit on the scalp and can irritate open sites. Air dry.
Read the first five ingredients, not the front label. Terms like post-treatment and scalp revival mean nothing measurable, and no randomized trial has compared shampoo formulas after PRP. Every recommendation, including mine, comes from irritation potential.
Actually, let me correct the framing I hear most. Patients ask which shampoo protects the PRP. Nothing does. It’s already in the dermis. What you’re protecting is the skin barrier around it.
My ranking:
Water temperature beats brand every time. Lukewarm water and low pressure, fingertips only.
Give it 48 to 72 hours before hard training. Salt in sweat irritates puncture sites, and pools add chlorine. Walking is fine on day one, and our guidance on exercise after PRP covers the return-to-training sequence.
Blow dryers on high, flat irons, and chemical services wait 72 hours minimum. Color and bleach wait four weeks. Peroxide on a settling scalp is a bad trade for a root touch-up.
Tampa sun on a tender scalp hurts, and that’s the honest reason to cover up. Sun doesn’t neutralize your platelets, whatever you’ve read. Loose hat or shade.
Two to four times a week suits most scalps, and oily scalps need more. Overwashing is real, but so is under-washing. Sebum and buildup smother follicles, and skipping washes to protect a treatment accomplishes nothing.
Lifestyle does the work. PRP fine-tunes it.
Protein at every meal, iron and ferritin high enough to support the growth phase, vitamin D measured rather than guessed, sleep in a dark room. There’s a short list of foods to avoid as well, mostly the inflammatory ones. Then the other side of the seesaw: fasting windows, training, and recovery that let cells clean house. I call this the Rhythm of Longevity. Growth without renewal stalls out, and renewal without building leaves you thin everywhere, hair included.
A second reason we pull several levers: the body compensates. Push one pathway hard and it routes around you through another. Follicles answer to hormones, blood supply, inflammation, and nutrient sensing, so a plan that pushes growth factors alone gets a weaker result than one working all four.
Microneedling and daily topical minoxidil pair well. Analyses from 2024 and 2025 show the combination beats either alone for density.
Unproven add-ons are where patients burn sessions. The FDA has repeatedly warned about unapproved regenerative products marketed for conditions they were never studied for. Formation holds SSRP and ISSCA certification, the bodies that certify peptides, exosomes, and stem cells, and we dispense human grade compounds only. Research grade molecules are illegal to administer, prescribe, purchase, or dispense, and we don’t touch them.
Shedding often eases inside the first month. Visible density usually arrives at three to six months, after three sessions spaced about a month apart, though how many sessions you need depends on your starting density. Maintenance follows every four to six months. The ISHRS 2025 Practice Census reported average nonsurgical patients per member up roughly 30% since 2021, to 498.
Four things tell me something’s off:
Call the office instead of switching shampoos again. Knowing whether PRP is working takes tracking, not guesswork. That 2025 review found adverse effects were mostly transient, mainly injection-site pain, redness, swelling, and bruising, and non-activated preparations carried about 3.7 times the odds of a side effect. Preparation protocol matters more than any bottle in your shower.

The shampoo to use after PRP hair treatment is the easiest call here. Get the sessions right, keep the lifestyle honest, and the bottle barely matters.
A fragrance-free, dye-free cleanser with a mild surfactant base, started about 24 hours after your session. Apply it with fingertips under lukewarm water rather than scrubbing with nails. Plain baby shampoo covers the first couple of washes as well as anything marketed for post-treatment use.
Wait until day three or four. Actives like zinc pyrithione, selenium sulfide, and salicylic acid work well long term but sting fresh injection sites. Once the punctures have sealed, you can return to your usual medicated shampoo on your normal schedule.
No. Sulfate-free is a reasonable default rather than a safety standard, and many sulfate-free formulas rely on cocamidopropyl betaine. A 2022 review in the International Journal of Environmental Research and Public Health put the pooled contact allergy risk to that ingredient at 1.7 times higher in hairdressers than in people outside the trade. Low-irritant and fragrance-free matter more than sulfate-free.
Wait four weeks. Peroxide and ammonia sit directly on the scalp, and a settling scalp reacts to them more than a healthy one. Chemical straightening and perms follow the same rule.
Not meaningfully. A 2025 systematic review of 43 randomized trials covering 1,877 patients tied hair density gains to the injections themselves, with activated preparations adding roughly 24 hairs per square centimeter against placebo. Shampoo protects the skin barrier, not the platelets.
It's better to wait a full 24 hours. Washing doesn't rinse out injected PRP, but water pressure and scrubbing on unsealed puncture sites raise your irritation and infection risk. If you need to rinse sweat off, use cool water and no product.
Yes, they get stricter. Microneedling opens many more channels in the skin, so we hold all washing for a full 24 hours minimum and keep you on a fragrance-free cleanser for the first week. Non-activated preparations also carry about 3.7 times the odds of a side effect, which is why preparation protocol comes up at your consultation.
