A PDO thread lift places fine, dissolvable threads under the skin to lift a softening jawline and lower face. You see the lift the day of treatment, and most people are back at work within 24 to 48 hours. At Formation, Dr. Gopal Grandhige checks what’s driving the softness before he recommends a single thread. Loose skin responds to threads. Fat, muscle bands and a short chin don’t.
Your virtual evaluation is with Hanna, Formation’s patient coordinator, who reviews your goals and outlines a plan before you decide to come in. The in-office consultation with Dr. Grandhige carries a fee and covers a full surgical assessment. That fee applies toward any procedure or product at Formation.
A PDO thread lift repositions loose skin along the jaw and lower face. The threads are made of polydioxanone, the absorbable material used in surgical sutures for decades. Each thread goes in through a fine cannula. Small barbs along it grip the tissue under the skin and hold it in its lifted position. There are no large incisions and nothing to remove later.
Two things follow. The mechanical lift shows immediately, which sets threads apart from energy-based tightening, where results build over months. Then, as the threads dissolve, your body’s healing response lays down new collagen along the paths they occupied.
Most thread pages blur the regulatory status, so here it is plainly. PDO threads reach the U.S. market through 510(k) clearance as absorbable sutures. One widely used thread’s FDA clearance record lists soft tissue approximation as its indication and states the suture isn’t intended for lifting and supporting tissue. Using threads for a facial lift is a physician’s off-label choice, which is legal and common in aesthetics. That means the evidence for how well the lift works, and how long it lasts, comes from published studies rather than the clearance. The sections below walk through that evidence.
Dr. Grandhige is a board-certified general surgeon specializing in aesthetic surgery. He has more than 15 years in surgery, with fellowship training in burn surgery, critical care and minimally invasive surgery. That training is why his evaluation starts below the skin.
Threads fix one problem: mild loose skin. Most necks show aging in more than one layer at once, so Formation uses the Four-Layer Neck Read to find which layer is driving the softness before choosing a tool. Each layer has its own solution.
Fat under the chin and along the jaw blurs the jawline even in lean, active people, and threads don’t remove fat. Neck liposuction is the layer-one answer.
One Formation patient, a fit professional in his 40s, had always looked heavier through the neck than the rest of his body and avoided photos. His issue was fat. After targeted neck liposuction, colleagues told him he looked rested, and he started stepping into pictures again.
Vertical cords down the front of the neck are platysmal bands, and a dissolving thread won’t hold them. Where bands are present, the layer-two solution is MyEllevate, a permanent suspension suture, combined with platysmaplasty. That suture is nonabsorbable and can be removed. PDO threads, by contrast, dissolve on a schedule.
This is where PDO threads belong. Skin that has started to slacken along the jawline, with reasonable elasticity left, can be repositioned with threads and firmed over time by the collagen response. When skin needs tightening more than repositioning, an energy-based option may fit better. That could be FaceTite, a radiofrequency-assisted tightening device, or Renuvion, a helium plasma energy device.
A short chin or a soft jaw angle makes the whole lower face read heavier, and threads can’t add structure that isn’t there. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, build chin projection and define the jaw angle. For some faces, adding frame does more than any lift.
Dr. Grandhige’s rule is to do only what’s necessary and do it well. If the read shows threads won’t move your result, he’ll say so and name the layer to treat instead. Some patients have laxity beyond what tightening or contouring can reach. For them, Formation stays in contact with the surgeons who do facelift work well and points each patient to the right one.
The best thread candidates have three things in common:
Skin age sets the window, roughly skin ages 25 to 55. Calendar age matters too, as the complication data below shows. Threads also suit people who want a visible lift with a day or two of downtime and who accept that the lift is temporary.
Threads are the wrong choice for heavy fat under the chin, visible platysmal bands, or skin that hangs in folds. They aren’t a facelift substitute either. If a facelift is in your plans for the next few years, say so at your consultation. Surgeons disagree about whether scar tissue along thread paths makes later facial surgery harder. That question belongs in your timing decision before any thread goes in.
Men often want a sharper jaw without anything that looks done, and Formation specializes in male cosmetic surgery. Our PDO thread lift for men page covers male-specific planning.
Start with Hanna, Formation’s patient coordinator, on a virtual evaluation that maps your goals and a plan. The in-office consultation with Dr. Grandhige is a full surgical assessment. Its fee applies toward any procedure or product at Formation.
A PDO thread lift is done in the office under local anesthesia. It typically takes 15 to 45 minutes, depending on how many areas are treated. You stay awake, there’s no general anesthesia to recover from, and most people feel pressure or tugging rather than pain.
Recovery usually runs on this schedule:
Most side effects are minor and short-lived: mild swelling, bruising, tenderness and tightness that settle within a few days. The best current numbers on less common problems come from a 2026 multicenter study in Aesthetic Plastic Surgery. It reviewed 2,000 PDO thread cases from 105 clinics, treated between November 2020 and December 2023.
The overall complication rate was 12.4 percent. The most common problems were:
Two factors predicted problems independently. Each added thread raised the odds of a complication by about 12 percent. Age raised the odds in steps, compared with younger patients: roughly double in the 40s, more than triple in the 50s, and about five times at 60 and older.
Read those figures with their limits in mind. The study was retrospective, and every case came from a single clinic network in Japan, so the rates may not transfer exactly to other practices or patients. The pattern still fits the restraint Dr. Grandhige applies across his work. For the rarer risk patients ask about most, our article on whether PDO threads can cause nerve damage goes deeper.
The thread is only part of the result. The collagen that firms your skin after the threads dissolve comes from your own healing response, so how well your body heals shapes what you keep.
Formation assesses your health and diet before treatment. In the window right before and after your appointment, nutrition, supplements and peptides are adjusted so your body builds the most structure it can.
This is the 60/40 Recovery Standard behind Formation’s philosophy. The procedure accounts for about 60 percent of the outcome, and the care around it accounts for the other 40. A PDO thread is the same product wherever it’s placed. The thinking around it is what changes.
Formation’s guidance is that results typically last 12 to 18 months, and maintenance treatments can extend them. The threads dissolve within six to nine months. What holds after that is the collagen they triggered.
The published evidence is thinner than thread marketing suggests. A 2025 systematic review in Dermatologic Surgery covered 12 studies and 818 patients. Every study showed improvement right after treatment, and the risk of dangerous complications was low. But the longest follow-up was two years, and the reviewers concluded that long-term efficacy data are still missing.
Treat threads as a lift with a shelf life, and plan on repeat sessions if you like the result. If you want support at the muscle level that doesn’t dissolve, that’s the MyEllevate conversation from layer two.
To see the natural, proportional results Formation aims for, browse the before-and-after gallery.
Formation is at 1315 South Howard Avenue, northeast of the Bay to Bay Boulevard and MacDill Avenue shops. From most of Palma Ceia, the drive stays on surface streets, with no interstate or bridge in between. South Tampa is one of the areas most Formation patients come from, and a 15- to 45-minute in-office appointment fits inside a Palma Ceia workday.
Two local timing notes:
You can start from home with a virtual evaluation, then come in for the in-office assessment once a plan makes sense.
Formation’s guidance is 12 to 18 months, and maintenance treatments can extend results. Published studies haven’t tracked thread results beyond about two years.
Most people report minimal discomfort. Local anesthesia goes in first, so you’ll feel pressure or tugging. Any soreness afterward is usually mild and gone within a few days.
No. A double chin is usually fat, and threads don’t remove fat. Neck liposuction treats that layer, while threads suit mild loose skin along the jaw.
Plan on one week away from training. Skip facial massage and face-down sleeping through the second week so the threads settle.
Mild swelling, bruising and tightness are expected and fade within days. In a 2026 study of 2,000 cases from one clinic network in Japan, skin dimpling or retraction was the most common complication at 6.35 percent. Risk rose with more threads and older age.
Surgeons disagree on how much scar tissue along thread paths affects later surgery. If a facelift is in your plans, raise it at your consultation so timing is part of the decision.
Yes, when the read calls for it. Botox, a botulinum toxin, relaxes muscle activity, and Radiesse or Juvederm add chin and jaw structure. That way, one plan can cover more than one layer.
Start with a complimentary virtual evaluation with Hanna, Formation’s patient coordinator, who reviews your goals and outlines a plan. The in-office consultation with Dr. Grandhige carries a fee that applies toward any procedure or product at Formation. The office on South Howard Avenue is a short drive from Palma Ceia.
A PDO thread lift does one job well: it lifts mild loose skin along the jaw and lower face, the same day, with a day or two of downtime. It doesn’t treat fat, muscle bands or a short chin, and its lift is temporary by design.
Dr. Grandhige reads all four layers first, tells you which one is driving the softness, and places threads only when they’ll move your result. For Palma Ceia patients, that read is a short drive to South Howard Avenue. You can also start with a virtual evaluation from home ahead of the in-office consultation.
Six hours. Hundreds of decisions.
High-definition liposuction is as much about precision as it is about technique.
Dr. Gopal can spend 6–7 hours on a single HD liposuction procedure, carefully evaluating the body’s anatomy, refining muscle definition, and making sure each contour transitions naturally into the next.
And that level of attention doesn’t change as the hours go by.
Because the final result is shaped by hundreds of small decisions made throughout the procedure.

Which one surprised you the most? 👀
Liposuction is one of those procedures people have heard a lot about, but not everything they’ve heard is accurate.
From what liposuction can actually achieve to how long results take to develop, there are plenty of misconceptions that can lead to unrealistic expectations.
Dr. Gopal Grandhige, board-certified plastic surgeon at Formation in Tampa, breaks down some of the most common beliefs about liposuction and explains what’s fact and what’s fiction.

This is not your typical liposuction. 👀
Most people think liposuction is simply about removing unwanted fat. But high-definition liposuction takes a different approach.
Before surgery, Dr. Gopal carefully maps out the patient’s natural anatomy, marking the chest, abdomen, and waist to guide where fat will be removed and where it should be preserved.
Why does that matter?
Because removing fat evenly isn’t necessarily going to create definition. HD liposuction uses strategic fat removal to highlight the underlying muscle structure and create a more sculpted, athletic appearance.
And those lines? They’re part of the planning that makes this level of precision possible.
The photos show an actual patient immediately after surgery. Swelling is still present, and the final results will develop as healing progresses.

Does fat move after lipo?
It’s a common concern, especially when someone notices weight gain in an area that wasn’t treated.
Here’s what actually happens.
The fat cells removed during liposuction are gone permanently. But if someone gains weight afterward, the remaining fat cells can still grow, including in areas that weren’t treated.
The fat didn’t move. The body gained weight.
That’s why Dr. Gopal emphasizes being at a weight patients can realistically maintain before surgery. Not the lowest weight they can reach, but one they can sustain long after recovery.

Want a more sculpted body?
High-definition liposuction takes body contouring a step further. Instead of simply removing excess fat, it focuses on sculpting around the body’s natural muscle structure to create more visible definition and a more athletic contour.
Dr. Gopal Grandhige specializes in HD liposuction, customizing each procedure based on the patient’s anatomy, body shape, and desired level of definition.
Because the goal isn’t to give everyone the same body. It’s to create a result that works with their natural anatomy.

Don’t plan lipo 2 months before vacation.
If the goal is to enjoy a more sculpted, defined body next summer, the planning needs to start months before that.
Fall is one of our busiest seasons at Formation, and there’s a reason for it. Many patients are already thinking about how they want to look and feel next summer.
But getting there takes time.
At Formation, Dr. Gopal requires at least four weeks of preparation before surgery. That time is spent preparing patients physically and mentally for the procedure and recovery.
After surgery, swelling needs to go down, the skin needs to contract, and the tissues need time to settle into their new contour.
It can take 6–9 months for results to look and feel natural.
That’s why Dr. Gopal recommends planning liposuction 6–12 months ahead, especially when there’s a specific season or vacation in mind.
If next summer is the goal, now is the time to start the conversation.

How defined would you go?
Not everyone wants the same look from HD liposuction. Some prefer a smooth, subtle contour, while others want their abs to stand out.
Here’s how the three levels of definition differ:
01. Low definition: A leaner, natural-looking abdomen with subtle definition and light contouring around the underlying muscles.
02. Medium definition: More visible separation between the abdominal muscles, with added definition through the abs and obliques without a sharply etched six-pack.
03. High definition: More pronounced sculpting around the abs and obliques to highlight the underlying muscle structure and create a more chiseled, athletic look.
Dr. Gopal tailors his HD liposuction technique to the level of definition you’re looking for, while considering what’s achievable for your body.
Which would you choose: 1, 2 or 3? Tell us in the comments!

This isn’t traditional lipo. It’s HD lipo.
For this patient, getting smaller isn’t the goal.
He already works out. He already has the muscle. Dr. Gopal’s job is to use HD liposuction to bring out more of that existing definition and shape the areas around it so the body looks more athletic and defined.
That’s why the markings matter. Before surgery even begins, Dr. Gopal is mapping out how the abs, chest, arms and surrounding areas should work together in the final result.

This is the part of the job that never gets old.
There’s something special about knowing how different a patient’s life might feel on the other side.
Not just because their body may look different, but because of everything that can come with it.
Feeling more comfortable in their own skin.
Saying yes to plans without thinking twice.
Looking forward to summer instead of dreading it.
Plastic surgery can change a body. But sometimes, what matters most is how differently someone gets to experience their life afterward.
And that’s a pretty incredible part of what we get to do at Formation Tampa.

Three small incisions instead of one long one.
For some patients with diastasis recti, repairing the separated abdominal muscles doesn’t necessarily require a traditional tummy tuck.
A tummy tuck makes sense when there’s loose skin that needs to be removed at the same time. But some patients are lean, don’t have much loose skin, and still have that persistent pooch from muscle separation.
For the right patient, Dr. Gopal can repair the diastasis using a minimally invasive robotic approach through three small incisions, about 8 mm each, hidden along the bikini line.
The key is choosing the right procedure for the right anatomy. If there’s significant loose skin or excess fat, repairing the muscles alone may actually make that looseness more noticeable.
That’s why the consultation matters. It’s not just about seeing a belly pooch. It’s about understanding what’s causing it and what actually needs to be addressed.
