Neck liposuction takes out the fat under your chin and along your jawline so the angle between your jaw and neck shows again. At Formation it’s done awake, in the office at 1315 South Howard Avenue in South Tampa, about 30 miles up I-75 from Ruskin. You take an oral sedative, the area is numbed, and someone drives you home to Ruskin the same day.
Neck liposuction removes the fat between your skin and the platysma, the thin sheet of muscle that runs from your collarbone up over your jaw. That’s the whole job. It doesn’t tighten skin or change muscle, and it can’t reach anything underneath that muscle.
Fat cells taken out don’t grow back, so the new line holds as long as your weight stays fairly steady. It’s a shaping tool for people near their goal weight. Formation treats it as one decision inside a larger plan for neck liposuction in Tampa, never as a product sold on its own.
Not all fullness under the chin is fat a cannula can reach. Some of it sits beneath the platysma. Some of it is the submandibular glands, which can drop lower with age. Suction above the muscle leaves both where they were, and clearing the fat on top can make them easier to see. Those necks need a different operation, and Formation refers those patients to surgeons it stays in contact with.
Rapid weight loss, including on GLP-1 medications, can create the opposite problem. There’s less fat under the chin, and the skin hasn’t tightened to match. Liposuction doesn’t fix that. Layer three of the read below does.
Most necks show aging in more than one layer at once, so Dr. Grandhige doesn’t start with a procedure name. He reads four layers in order, and each one has its own answer. You can run a rough version of every check at a mirror before you book.
Tip your chin down and roll the soft tissue under your jaw between your thumb and finger. Then pull the corners of your mouth down hard, which tightens the platysma, and pinch again. Whatever you can still pinch sits above the muscle, and that’s what liposuction removes. Fullness you can feel behind the tensed muscle but can’t grab sits below it. For plenty of patients, layer one is the only layer that needs work.
Clench your teeth, grimace, and look at the front of your neck. Two vertical cords running down from the jaw are platysmal bands. Liposuction does nothing to them, and taking fat from around active bands can leave them more visible than before.
Where bands are present, the answer is MyEllevate, a permanent suspension suture, with platysmaplasty to bring the muscle edges back together. The suture is nonabsorbable and can be removed later, unlike barbed threads that dissolve on their own timetable. The lift is there the day it’s placed.
Pinch the skin at the side of your neck, hold for two seconds, and let go. Skin that snaps back will settle over a new contour by itself. Skin that drifts back slowly needs help in the same visit, or the new contour reads as slack.
Reasonable elasticity usually holds through a skin age of roughly 25 to 55, and skin age isn’t the number on your license. Ruskin patients come from both ends of that range. Younger families in the newer SouthShore subdivisions mostly sit inside it. Sun City Center, the 55-and-over community next door, starts at the far edge. Skin age doesn’t always match a birth certificate, so the pinch test decides, not the calendar.
When skin needs help, Dr. Grandhige adds Renuvion, a helium plasma radiofrequency device, or FaceTite, a radiofrequency-assisted lipolysis device. Both are 510(k)-cleared. What separates them is depth of access. They work at the deepest layer of the skin and in the fibroseptal network beneath it, the connective tissue between skin and muscle that holds a contour in place. Lasers and microneedling work down from the surface and can’t reach that foundation. Patients who need fat out and skin tightened in one sitting are looking at FaceTite in Ruskin.
Skin that’s gone past what tightening can reach needs excision. Formation says so at the consultation and points those patients to the surgeons who do that work well.
Stand sideways to a mirror and check where your chin sits against your lower lip. A chin that sits back, or a jaw angle that reads soft, makes a neck look full even when there’s little fat to take. Suction doesn’t change bone.
That frame gets built with Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed to extend the structure your jawline hangs from. On the right anatomy, this is the layer behind a superhero jawline. Done backward, with suction where the frame was the problem, it hollows the area under the jaw and leaves the weak profile in place. Patients whose read lands here usually start with jawline fillers in Ruskin.
One published series gives specific nerve-injury numbers for neck liposuction, and they’re low.
A 987-patient neck liposuction series was treated under tumescent local anesthesia by three dermatologic surgeons. It recorded five cases of temporary marginal mandibular nerve dysfunction, one submandibular gland ptosis and one arterial bleed. It recorded no seromas, skin necrosis, scarring or hyperpigmentation. Per patient, that’s about 1 in 200 for a temporary nerve problem and about 1 in 1,000 for each of the other two.
The nerve figure is easier to picture than it sounds. The marginal mandibular nerve moves the lower lip, so temporary weakness shows up as a smile that pulls unevenly on one side. In that series, every case was temporary.
The limits matter as much as the numbers. This is a retrospective series from three practices, not a randomized trial. It describes those surgeons and that technique, not a national average, and it can’t tell you anything about the hands you’re considering. It’s still a useful match for an awake neck case, since those patients were treated under local anesthesia too.
Ask any surgeon you’re considering for their own complication history. Then ask whether they do the neck awake or asleep, and why.
Plan on about 30 miles and roughly 40 minutes each way off-peak, and plan on someone else driving. The oral sedative rules out driving yourself home.
The direct route runs east on College Avenue to I-75, north past Apollo Beach and Riverview to the Selmon Expressway, then west to Exit 4 at Willow Avenue in Hyde Park. The office on South Howard Avenue is a few minutes from the ramp. The Selmon is all-electronic, so it’s a SunPass or a bill by plate. Skipping the toll means looping through I-4, I-275 and the downtown interchange locals call Malfunction Junction, which adds miles.
Timing matters more than distance. Northbound I-75 through Riverview and Brandon slows hard in the morning rush. A late-morning slot gets you in after the worst of it and usually home before the afternoon build. From roughly November through April, winter residents add traffic on College Avenue toward Sun City Center and on US-41 through Ruskin, so build in a cushion.
The short distance pays off after the procedure. Dr. Grandhige follows patients for a full year, and nearly all of them stay with it for the whole year. From Ruskin, each visit takes an afternoon.
Heat is the one local factor no route avoids. The chin compression garment stays on 12 to 24 hours a day through the first week, and that week feels different in July than in January.
If you aren’t sure the neck is the right starting point, the wider read of neck and jawline contouring in Ruskin covers the jaw, chin and lower face together.
You’re awake the whole time. It’s a shorter procedure, and you go home the same day.
An oral sedative comes first, then local anesthetic under the chin and along the jaw. Dr. Gopal Grandhige works through very small incisions under the chin and behind the ears. His own physician assistant, who assists on every surgery he performs, works alongside him. He treats the work as sculpture rather than fat removal. Every pass is planned against your jawbone, your muscle position and the way light falls across your lower face. He sharpens the jawline while protecting your natural angles, and he leaves some fat in place on purpose. Over-resection leads to irregular contours and a neck that ages poorly.
When layer three calls for it, Renuvion or FaceTite follows in the same visit. You leave in a chin compression garment.
State rules sort in-office surgery by anesthesia level. The Florida Board of Medicine’s office surgery rules require registration and annual inspection, or accreditation, once liposuction removes more than 1,000 cc of fat or a procedure moves into the deeper sedation levels. Any office should be able to tell you which level your procedure falls under. Formation’s larger operations run in Quad A accredited facilities with the same anesthesia provider and staff on every case. A neck case doesn’t need that setting, which is why you go home to Ruskin the same day.
Your health and diet get assessed before anything is scheduled, and what that assessment finds changes the plan. Nutrition, supplements and peptides are adjusted in the window right before and right after so your body builds the most structure it can while it heals. Protein intake, inflammation control and lymphatic flow are dialed in as part of the procedure, not handed over on a printout on your way out. Lymphatic treatment runs through providers Dr. Grandhige has vetted personally on their outcomes.
Formation calls this the 60/40 Recovery Standard. Surgery is 60 percent of your result and post-operative care is the other 40, and poor aftercare can undo a well-performed operation. It’s why two people with the same procedure on the same day can look different six months later. The same reasoning runs through Formation’s philosophy as a whole.
Tightness and swelling under the chin in the first days are expected. The result keeps changing for months as swelling settles and the skin tightens.
The cannula and the radiofrequency handpiece are identical at every practice between Ruskin and downtown Tampa. Any practice can buy them. The thinking around them isn’t identical, and that’s what you’re choosing between.
First few days. Mild swelling, bruising and tightness, managed with the chin compression garment.
One week. Most patients are back at work and back to social plans, still wearing the garment 12 to 24 hours a day. Training resumes at about one week.
Two to four weeks. Swelling keeps settling, and the fat removal becomes visible.
Three months. Skin tightening shows and the jawline sharpens.
Six to nine months. The final result settles as tightening finishes.
Plan backward from the date that matters. Four weeks gets you to visible fat removal and three months to visible tightening. If you want the tightened line in place by the start of summer, late winter is about the latest a procedure can happen. That timing also puts your garment week in the mildest stretch of the year.
Which one surprised you the most? 👀
Liposuction is one of those procedures people have heard a lot about, but not everything they’ve heard is accurate.
From what liposuction can actually achieve to how long results take to develop, there are plenty of misconceptions that can lead to unrealistic expectations.

This is not your typical liposuction. 👀
Most people think liposuction is simply about removing unwanted fat. But high-definition liposuction takes a different approach.
Before surgery, Dr. Gopal carefully maps out the patient’s natural anatomy, marking the chest, abdomen, and waist to guide where fat will be removed and where it should be preserved.
Why does that matter?
Because removing fat evenly isn’t necessarily going to create definition. HD liposuction uses strategic fat removal to highlight the underlying muscle structure and create a more sculpted, athletic appearance.
And those lines? They’re part of the planning that makes this level of precision possible.
The photos show an actual patient immediately after surgery. Swelling is still present, and the final results will develop as healing progresses.

Does fat move after lipo?
It’s a common concern, especially when someone notices weight gain in an area that wasn’t treated.
Here’s what actually happens.
The fat cells removed during liposuction are gone permanently. But if someone gains weight afterward, the remaining fat cells can still grow, including in areas that weren’t treated.
The fat didn’t move. The body gained weight.
That’s why Dr. Gopal emphasizes being at a weight patients can realistically maintain before surgery. Not the lowest weight they can reach, but one they can sustain long after recovery.

Want a more sculpted body?
High-definition liposuction takes body contouring a step further. Instead of simply removing excess fat, it focuses on sculpting around the body’s natural muscle structure to create more visible definition and a more athletic contour.
Dr. Gopal Grandhige specializes in HD liposuction, customizing each procedure based on the patient’s anatomy, body shape, and desired level of definition.
Because the goal isn’t to give everyone the same body. It’s to create a result that works with their natural anatomy.

Don’t plan lipo 2 months before vacation.
If the goal is to enjoy a more sculpted, defined body next summer, the planning needs to start months before that.
Fall is one of our busiest seasons at Formation, and there’s a reason for it. Many patients are already thinking about how they want to look and feel next summer.
But getting there takes time.
At Formation, Dr. Gopal requires at least four weeks of preparation before surgery. That time is spent preparing patients physically and mentally for the procedure and recovery.
After surgery, swelling needs to go down, the skin needs to contract, and the tissues need time to settle into their new contour.
It can take 6–9 months for results to look and feel natural.
That’s why Dr. Gopal recommends planning liposuction 6–12 months ahead, especially when there’s a specific season or vacation in mind.
If next summer is the goal, now is the time to start the conversation.

How defined would you go?
Not everyone wants the same look from HD liposuction. Some prefer a smooth, subtle contour, while others want their abs to stand out.
Here’s how the three levels of definition differ:
01. Low definition: A leaner, natural-looking abdomen with subtle definition and light contouring around the underlying muscles.
02. Medium definition: More visible separation between the abdominal muscles, with added definition through the abs and obliques without a sharply etched six-pack.
03. High definition: More pronounced sculpting around the abs and obliques to highlight the underlying muscle structure and create a more chiseled, athletic look.
Dr. Gopal tailors his HD liposuction technique to the level of definition you’re looking for, while considering what’s achievable for your body.
Which would you choose: 1, 2 or 3? Tell us in the comments!

This isn’t traditional lipo. It’s HD lipo.
For this patient, getting smaller isn’t the goal.
He already works out. He already has the muscle. Dr. Gopal’s job is to use HD liposuction to bring out more of that existing definition and shape the areas around it so the body looks more athletic and defined.
That’s why the markings matter. Before surgery even begins, Dr. Gopal is mapping out how the abs, chest, arms and surrounding areas should work together in the final result.

This is the part of the job that never gets old.
There’s something special about knowing how different a patient’s life might feel on the other side.
Not just because their body may look different, but because of everything that can come with it.
Feeling more comfortable in their own skin.
Saying yes to plans without thinking twice.
Looking forward to summer instead of dreading it.
Plastic surgery can change a body. But sometimes, what matters most is how differently someone gets to experience their life afterward.
And that’s a pretty incredible part of what we get to do at Formation Tampa.

Three small incisions instead of one long one.
For some patients with diastasis recti, repairing the separated abdominal muscles doesn’t necessarily require a traditional tummy tuck.
A tummy tuck makes sense when there’s loose skin that needs to be removed at the same time. But some patients are lean, don’t have much loose skin, and still have that persistent pooch from muscle separation.
For the right patient, Dr. Gopal can repair the diastasis using a minimally invasive robotic approach through three small incisions, about 8 mm each, hidden along the bikini line.
The key is choosing the right procedure for the right anatomy. If there’s significant loose skin or excess fat, repairing the muscles alone may actually make that looseness more noticeable.
That’s why the consultation matters. It’s not just about seeing a belly pooch. It’s about understanding what’s causing it and what actually needs to be addressed.

Thank you to our patients for continuing to trust Dr. Gopal and the Formation team with your care.
Start with the complimentary virtual evaluation. Patient coordinator Hanna reviews your goals and outlines a plan before you spend a morning on I-75. The in-office consultation with Dr. Grandhige carries a fee and works as a full surgical assessment. It covers an anatomical evaluation, a read of all four layers, a direct conversation about limits, and guidance on timing. That fee can be applied toward any procedure or product at the practice.
Bring your goals, your questions and your medical history.
Plenty of patients tell the staff what worries them before they’d say it to a surgeon. The staff passes those concerns along so Dr. Grandhige can answer them without you having to ask. The fear he hears most is that the result won’t look natural. It’s often framed as whether friends or coworkers will be able to tell.
Dr. Grandhige is a board-certified general surgeon who specializes in aesthetic surgery, certified through the American Board of Surgery. After medical school at the University of Michigan, he completed a general surgery residency. He followed it with fellowship training in burn surgery, critical care, and minimally invasive metabolic and foregut surgery. He has practiced in Tampa for more than 15 years across several thousand surgical cases, and neck and jawline contouring is one of the three areas he’s known for. He designs every surgical plan himself and limits his volume so he stays involved through recovery.
One patient, a professional in his forties, came in because his neck made him look heavier and older than he felt, and he hated how he looked in pictures. After targeted neck liposuction that preserved his natural jaw angles, colleagues told him he looked more rested. He felt his face finally matched the rest of his body, and he started getting in pictures again.
Dr. Grandhige will also tell you no. Some patients leave with a plan that starts somewhere other than surgery.
No. It's done awake in the office with an oral sedative and local anesthetic. It's a shorter procedure, and you go home the same day.
The office on South Howard Avenue in South Tampa is about 30 miles from Ruskin, roughly 40 minutes off-peak by I-75 and the Selmon Expressway. You'll need a driver, because the oral sedative rules out driving yourself home.
Most patients are back at work and back to training at about one week. The chin compression garment stays on 12 to 24 hours a day through that first week.
No. It removes fat only. When skin needs help, Renuvion or FaceTite is added in the same visit. Skin past what tightening can reach needs excision, and Formation refers those patients to surgeons who do that work.
Rapid weight loss can leave less fat under the chin and skin that hasn't tightened to match, and liposuction doesn't fix loose skin. The in-office read checks all four layers and tells you which one is driving what you see.
Those are platysmal bands, and liposuction doesn't treat them. Removing fat around active bands can make them stand out more. Where bands are present, the answer is MyEllevate with platysmaplasty.
Fat cells that are removed don't come back, so the result holds as long as your weight stays fairly stable. Your neck keeps aging, but taking out the excess fat can slow how quickly laxity shows up later.
The in-office consultation with Dr. Grandhige carries a fee and is a full surgical assessment, and that fee can be applied toward any procedure or product at Formation. The complimentary virtual evaluation with patient coordinator Hanna is the place to start if you want a plan before making the drive.
Fat, muscle, skin and frame each need a different answer, and treating the wrong layer is how people end up looking worse than when they started. It’s also why two Ruskin patients who describe the same complaint can leave with two different plans.
The drive up I-75 is about 40 minutes. The consultation is where you learn which of the four layers is driving what you see in the mirror. You’ll also learn whether the answer is a procedure now, a different procedure, or waiting.
