A PDO thread lift places absorbable barbed sutures under the skin to reposition tissue along the jawline and neck. The lift is there the day it’s placed, and the body builds collagen along the thread tracks over the months that follow. Threads work on one of the four layers that make a neck look older. So the useful question isn’t whether threads work. It’s whether your neck is a layer-three problem.
Formation is in South Tampa, not Apollo Beach. If you live in 33572, this is a drive up Big Bend Road to I-75, and we’d rather say that in the first paragraph than three scrolls down. What follows is the same read Dr. Grandhige runs in the room, so you can get most of the way to an answer before you spend the gas.
Polydioxanone is a suture material surgeons have used for decades. In a PDO thread lift, the thread is barbed, so it grips tissue and holds it in a new position instead of just closing a wound. Two things happen. Tissue moves that day. Then the thread dissolves over roughly six to nine months while collagen forms along the track it left.
That collagen is the part that outlasts the thread. The mechanical lift fades as the suture breaks down. Most patients see the result hold somewhere in the 12 to 18 month range before it softens.
One regulatory detail worth knowing, because marketing copy blurs it. PDO threads are 510(k)-cleared as Class II devices for soft tissue approximation, which is the surgical suture indication. Cosmetic lifting isn’t the cleared indication. And cleared is not the same as approved: clearance means the FDA found the device substantially equivalent to something already on the market. That doesn’t make threads unsafe. It means “FDA-approved for a facelift” is a sentence nobody should be writing.
Threads don’t remove fat. They don’t repair muscle. They don’t build a jaw.
Most necks show aging in more than one layer at once. That’s why single-treatment answers disappoint people. Dr. Grandhige reads four layers in every neck and jawline assessment, and each one has its own solution. Threads answer part of one of them.
If you can pinch a pad under your chin and your jawline disappears when you look down at your phone, fat is the primary issue. No suture removes it. Threads placed over a submental fat pad pull skin across a bulge and can make the bulge read more clearly.
The layer-one answer is removal. Formation handles this with in-office neck liposuction under local anesthesia and an oral sedative, through small incisions under the chin and behind the ears. If fat is what’s blurring your jaw, neck and jawline contouring will do more for you than any thread will.
Stand at a mirror and clench your teeth. If two vertical cords stand up in your neck, those are platysmal bands, and they’re muscle. Barbed threads placed in the skin above a lax platysma don’t reach the structure causing the problem.
Where bands are the driver, the answer is MyEllevate, a permanent suspension suture, combined with platysmaplasty to address the muscle directly. For bands that are more about tone than laxity, Botox can soften the pull without touching the anatomy. This is where threads live. Skin that has lost recoil but still has some spring left responds to being repositioned and held. The elasticity window runs roughly from skin age 25 to 55. Inside it, threads have something to work with. Outside it, you’re asking a suture to carry a load skin can no longer share.
Here’s the test we use. Pinch the skin at your jawline and release. If it snaps back, threads have a real target. If it drifts back slowly or stays creased, that’s a different conversation.
Some people don’t have a skin problem. They have a frame problem. A chin that sits behind the plane of the lower lip makes the neck look full no matter how tight the skin gets. No amount of tightening builds a superhero jawline on a recessed chin.
Frame is addressed with structural filler: Radiesse, a calcium hydroxylapatite, where firm projection is the goal, or Juvederm, a hyaluronic acid, where reversibility matters more. A page covering structural filler for chin and jaw projection is in progress.
Three approaches compete for the same patient, and they aren’t interchangeable. Run yourself through this before you book anything.
Route one, threads. You have mild to moderate laxity, decent recoil, no bands, no significant fat pad, and you want the change visible now with a couple of days of downtime. Threads fit. Accept that the result is measured in months, not years.
Route two, energy. You have laxity plus tissue that needs remodeling rather than repositioning. FaceTite and Renuvion reach the deepest layer of the skin and the fibroseptal network beneath it. Lasers and microneedling work from the surface down and cannot get to that foundation. Energy takes three months to show and six to nine to finish, but it’s addressing structure rather than borrowing tension.
Route three, neither. You have real skin excess, standing folds at rest, or bands that hold when your jaw relaxes. Tightening and contouring have a ceiling and you’re above it. Formation says so and refers you out. Dr. Grandhige doesn’t perform facelifts or neck lifts, and we stay in contact with the surgeons who do that work well so we can point you to the right one rather than sell you a smaller procedure that won’t get there.
Most Apollo Beach patients who come in asking for threads land on route one or route two. A meaningful number land on route three, and hearing it early saves them a year.
Thread lift marketing tends to describe the procedure as nearly risk-free. The literature is more specific.
A 2025 meta-analysis of thread lifting complications pooled 26 studies covering 2,827 patients. Bruising came in at 26%. Skin dimpling or asymmetry at 7%. Thread exposure at 5%. Infection at 2%.
Now the part that belongs next to those numbers. That analysis is a preprint and hasn’t completed peer review. Heterogeneity was high, with an Iยฒ of 92% for bruising and 76% for dimpling, meaning results scattered widely between studies. It was limited to English-language publications, funnel plots suggested publication bias, and it covered thread lifting broadly rather than PDO alone. Treat the figures as a range, not a rate.
The literature also documents tissue fibrosis along thread tracks. That’s scar tissue in a plane a surgeon may need later, which is why sequencing matters if a surgical lift is anywhere on your ten-year horizon. Threads first isn’t automatically wrong. It’s a decision that should be made on purpose.

Two local factors change the math on this procedure, and neither shows up on a generic Tampa Bay page.
The first is sun. Apollo Beach is built on canals, and the people who live there are on the water. Cumulative UV exposure degrades elastin, and elastin is what determines whether skin recoils when a thread pulls it. A 48-year-old who has spent twenty years on a boat in Tampa Bay can have less usable recoil than a 55-year-old who works indoors. Chronological age and skin age separate here more than they do in most markets, and we assess skin age, not birthdays.
The second is the age spread. Apollo Beach skews toward working families. Ten minutes south on US-41, Sun City Center and Kings Point are 55-and-over communities. That matters because the largest PDO study to date, a multicenter review of 1,500 patients across 105 clinics using barbed cog threads, found age to be an independent predictor of dissatisfaction, with an odds ratio of 1.06 per year. Thread count mattered too. Overall dissatisfaction was 3.4%, though that study counted only patients who came back reporting an insufficient result, so it undercounts anyone who was quietly unhappy and didn’t return.
Translated: the further past that elasticity window you are, the more likely threads underdeliver, and stacking more threads to compensate raises the odds rather than lowering them. If you’re calling from Sun City Center, there’s a real chance route two or route three fits you better, and we’d rather tell you that on a video call.
The durable half of a thread lift is collagen your body makes. So we don’t treat the appointment as the whole procedure.
Before threads go in, health and diet get assessed. Protein intake, inflammation, sleep, and medication that affects bruising all get reviewed, and nutrition, supplements, and peptides get adjusted in the window immediately before and after so the body builds the most structure it can. A patient who is under-fed on protein and running high inflammation lays down less collagen than the same patient dialed in, with the same threads placed the same way.
Recovery gets the same treatment. Formation runs a 60/40 standard on the neck and jawline: what happens during placement is 60% of the result, and what happens in the weeks after is the other 40%. Bad aftercare can undo good placement.
The thread itself is a commodity. Any practice in Tampa Bay can buy the same box. The thinking around it is not identical, and that’s the part worth driving for. You can read more about how Formation approaches care if you want the longer version.
Day of. The lift is visible immediately. Expect tightness, some swelling, and possible bruising at entry points.
Days two to three. Most patients are back at work. Bruising, if it shows, is peaking around now.
Weeks one to two. Swelling settles. No strenuous exercise and no facial massage for two full weeks. This is the one place where our thread timeline runs longer than our standard neck protocol, which puts patients back to training at one week after neck liposuction with energy tightening. Barbed threads need their anchor points to stabilize before you load the face with impact and repeated movement, so we hold you back an extra week on purpose.
Weeks three to six. The mechanical lift softens slightly as swelling resolves and the collagen response begins.
Months three to six. Skin quality improves as collagen matures along the thread tracks.
Months twelve to eighteen. The threads are long gone. The collagen benefit persists while the lift gradually releases. This is the decision point for whether to repeat, switch routes, or stop.

Clinical questions only. These are the ones that separate providers.
Ask who physically places the threads and what their training in facial anatomy is. Ask them to identify which of the four layers is driving your specific concern, and listen for whether they name one or dodge into generalities. Ask what they’d do if threads aren’t the right answer for you, because a provider who only offers threads will only find thread problems. Ask how they decide thread count for your anatomy rather than from a set package. And ask whether they’ve read the complication literature, because a provider who quotes you a zero-risk procedure hasn’t.
The mechanical lift typically holds 12 to 18 months as the threads dissolve. Collagen laid down along the thread tracks can improve skin quality beyond that window, though it doesn't maintain the lift on its own.
Possibly, but the odds shift. The largest PDO study found age to be an independent predictor of dissatisfaction. Past the roughly 25 to 55 elasticity window, energy-based tightening or a surgical referral often serves you better. We assess skin recoil rather than birth year.
No. A double chin is a fat problem in layer one, and no suture removes fat. Threads placed over a submental fat pad can make the fullness read more clearly. Neck liposuction addresses it directly.
Those are platysmal bands, and they're muscle. Threads sit above them and won't reach the cause. MyEllevate with platysmaplasty addresses the muscle layer directly.
It's done under local anesthesia. Most patients describe pressure and tugging rather than pain. Soreness for a day or two afterward is normal.
Two weeks, and that's longer than we hold patients after neck liposuction with energy tightening. Barbed threads need time for their anchor points to stabilize before you add impact and repeated facial movement.
The literature documents tissue fibrosis along thread tracks. If a surgical lift is a realistic possibility for you in the next decade, sequencing is worth discussing before threads go in rather than after.

A PDO thread lift is a good answer to a specific question: skin with recoil left in it that needs repositioning, in someone who wants the change visible now and can live with a result measured in months. For that person, it works, and the research supports it with numbers that are honest about bruising and dimpling.
For the person whose jawline is buried under fat, or whose platysmal bands stand up when they clench, or whose chin sits too far back to frame anything, threads are the wrong tool applied confidently. That’s the failure mode we see most, and it usually traces back to nobody reading all four layers before reaching for the one product they sell.
