PRP works for genetic hair loss, but it only works on part of the problem. Pooled trial data show it adds hair density in men and women with pattern hair loss. It doesn’t touch the hormone driving the condition in the first place. That gap explains why some patients see real change and others see almost nothing.
Pattern baldness is the most studied use of PRP. It’s also where expectations go sideways fastest. Patients walk in hoping for regrowth on a bald crown. What the evidence supports is a modest gain in how many hairs sit in a given patch of scalp, and only where living follicles remain.
PRP for genetic hair loss is a treatment that concentrates platelets from your own blood and injects them into the scalp. The platelets release growth factors that signal the cells at the base of each follicle. The goal is to hold onto and thicken the hair you still have.
Nothing foreign goes in. Blood gets drawn from your arm, spun in a centrifuge to separate the platelet layer, and injected across the thinning zones. The whole visit runs under an hour.
Genetic hair loss is androgen-driven, which makes it a different animal from stress shedding or patchy autoimmune loss. Your follicles inherit a sensitivity to dihydrotestosterone, or DHT. DHT shortens the growing phase of the hair cycle.
Each cycle, the follicle gets a little smaller. The hair it produces gets shorter and finer. Eventually the follicle stops producing anything visible. That process is called miniaturization, and it runs on a genetic clock that doesn’t care how you feel about it.
This matters for treatment selection. Stress-related shedding often resolves on its own. Genetic loss doesn’t. It progresses, which is why thinning across the crown tends to get worse without intervention rather than better.
The strongest evidence says PRP adds density in pattern hair loss, with moderate confidence. A 2025 systematic review in Dermatology and Therapy pooled 43 randomized trials covering 1,877 participants. Activated PRP increased hair density and lowered recurrence compared with placebo. Patient satisfaction improved too.
Read the limitation, though. It’s the part that changes decisions.
The same authors reported that preparation protocols varied so widely across trials, and reporting was so incomplete, that they couldn’t pin down which PRP formulation drove which result. They called for standardized trials before anyone treats those numbers as settled.
Something else worth knowing from that dataset. Roughly 73 percent of participants were men. Women are underrepresented in the evidence base, so the confidence for female pattern loss is thinner than the headline suggests. Response also varies by zone, and the frontal hairline is generally more stubborn than the crown.
More hairs does not mean thicker hairs. That 2025 review found no meaningful effect on hair thickness at all. PRP appears to recruit and hold follicles rather than fatten the individual shaft.
Combination data tell a similar story. When PRP was added to topical minoxidil, density beat minoxidil alone at one month, three months, and the five to six month mark, with gains between roughly 11 and 22 hairs per measured area.
Now the honest part. Those authors graded their own certainty as low to very low. Only five trials qualified, three carried a high risk of bias, and nobody followed patients past six months.
The same paper raises a question most clinics skip. There’s no validated threshold for how many extra hairs a patient actually notices in the mirror. So a statistically real gain of 11 to 22 hairs might or might not register as visible improvement to you.
We say this out loud at consultation. PRP does not create new follicles where none survive, and it does not restore a scalp that’s already smooth.
PRP supplies growth signals. It does nothing about DHT. Push growth factors while the androgen pathway keeps miniaturizing follicles, and you’re filling a bucket with a hole in it.
The body compensates. Pressure one pathway hard enough and it routes around you through another, which is why single-lever plans underperform. Go after genetic hair loss from several angles at once and you close those escape routes.
That’s the argument for stacking. Medical therapy addresses the hormone. PRP addresses the growth signal. Microneedling addresses delivery. Reviewing other hair loss treatments alongside PRP is not hedging, it’s how the pathways get covered.
Peptides and PRP fine-tune the work. Your lifestyle does the work. The habits that feed the same pathway PRP is nudging include protein intake sufficient for keratin production, resistance training to support anabolic signaling, sleep that lets repair happen, iron and vitamin D levels checked rather than assumed, and scalp inflammation kept down.
Skip those and you’re paying for a treatment to compensate for something free.
No. This gets stated wrong constantly, including by clinics that should know better. The centrifuges and kits used to prepare PRP receive 510(k) clearance as devices, which is a finding of substantial equivalence to something already on the market. That’s cleared rather than approved, and it’s a different standard entirely.
The therapy itself is off-label for pattern hair loss. Off-label is legal and common in medicine. It just means the informed consent conversation matters more, not less.

If an area has been bald for years and the scalp is smooth and shiny, the follicles are gone. Growth factors have nothing to act on. PRP is a poor use of your time there, and a hair transplant evaluation is the more honest conversation.
PRP also isn’t appropriate if you have a bleeding disorder, an active autoimmune condition, or a platelet abnormality. And if you can’t commit to maintenance, the gains fade. Patients who quit after one or two sessions almost always end up wondering why PRP isn’t working when the answer is that the protocol never finished.

Pattern loss also involves low-grade inflammation around the follicle, which PRP helps quiet.
