They cut low on purpose. The goal is a scar your underwear or swimsuit bottom can cover. For a full procedure, the surgeon makes a horizontal cut between the pubic hairline and the belly button, then lifts the skin to repair the muscle beneath it.
At Formation, our plastic surgeon, Dr. Ahmad Saad, marks that line while you’re standing up, not lying down. Skin falls differently when you’re upright. I’ve sat in on these markings, and the patients who bring their actual swimwear walk away with the best-hidden scars every time. Some surgeons even ask you to bring your usual underwear or bathing suit so the line lands right where clothing sits.
But “where do they cut” is almost the wrong first question. Let me put that better. The cut location follows from one thing: how much skin has to come off. More loose skin means a longer cut, and sometimes a higher one. People with a lot of upper-belly skin may have to accept a scar that rides higher than they hoped. A surgeon who hides that fact to win your booking is one to walk away from.
Not every tummy tuck uses the same cut. Four patterns cover almost everyone, and the technique chosen depends on your anatomy and how much skin needs to go.
This is the workhorse. The surgeon makes a hip-to-hip cut below the navel and a second cut around the belly button so it can be moved back into place after the loose skin is pulled down. The upper skin gets drawn down like a window shade, the extra is trimmed, and a fresh opening for the navel is made. If you’ve had kids or lost a lot of weight, this is usually the honest answer. Most people researching a full tummy tuck want a flat result, and the full version is the one that gets them there. It does more work, so it asks for a longer scar in return.
One short cut, low, above the pubic bone. No cut around the navel. The scar runs about 4 to 8 inches, and the belly button stays put. A mini tummy tuck only treats skin below the belly button, so it fits a narrow group of patients.
Now the unpopular part. The mini gets requested far more than it actually suits anyone. Loose skin usually spreads above and below the navel, which a mini can’t reach, so only a small number of people are real candidates. If a clinic pushes you toward a mini for a belly that clearly needs a full, walk out the door.
The cut goes further. It wraps around the hips toward the sides of the lower back, so the tail ends of the scar show when you’re viewed from behind, and the navel is freed and repositioned. This one is built for people after major weight loss, where loose skin circles the whole midsection. If you’ve dropped 80 or 100 pounds, this is often the cut that matches your skin. For someone with mild looseness, it’s overkill, and you’d be trading a much longer scar for very little extra benefit.
A fleur-de-lis adds a vertical cut up the center on top of the horizontal one. That vertical scar is harder to hide, so it’s saved for people who need the most skin removed and accept the trade. After very large weight loss, the body sometimes needs this T-shaped pattern to avoid leaving a bunched, uneven belly. It isn’t a beauty upgrade. It’s a structural fix for skin that won’t lie flat any other way.
Usually no, if the cut is placed well. A standard tummy tuck scar sits low enough that briefs, bikini bottoms, and most swim trunks cover it without effort. If you live in a swimsuit half the year, like plenty of people here do, that’s a conversation to have before surgery, not after.
This is the part people get wrong. They obsess over how low the scar is and ignore how the scar heals. A low scar that’s stretched, raised, and wide looks worse than a slightly higher scar that’s flat and thin. Placement matters. Closure and tension matter more. Picture two people with the same surgery. One scar, pulled tight under pressure, spreads to a centimeter wide. The other, closed with less tension, stays pencil-thin. Same cut, very different result.
The belly-button scar is the one patients forget. With a full tummy tuck, there’s a small ring scar around the navel. Skilled surgeons tuck it inside the rim so it nearly disappears. Ask to see real healed photos, not stock images, before you book anything.
You’ll have a long horizontal scar low on the abdomen, and with a full tummy tuck, a small scar around the navel. The skin gets closed with stitches, surgical glue, tape, or clips, depending on the surgeon’s method.
Scars are permanent. Anyone who promises you a scar-free tummy tuck is selling something. What a skilled surgeon controls is how the scar settles: its width, its color, and how flat it lies against the skin.
Raised, thickened scars aren’t rare, either. Studies put thick (hypertrophic) scars in roughly 39 to 68 percent of surgical patients, and keloids, which grow past the original wound, in about 5 to 15 percent. Your genes, your skin tone, and how the wound is closed all push that risk up or down. Darker skin tones carry a higher keloid risk, so flag any family history of bad scarring at your consult.
Plan for a full year, sometimes longer. Scar maturation can keep going for up to a year after surgery, and a scar that looks worse around the 6 to 10 week mark can still end up looking much better by 6 to 12 months.

Patience is the cheapest scar treatment there is. People who panic at week eight and start scrubbing or picking almost always end up with a worse-looking line than the ones who left it alone.
A few habits move the needle more than any pricey cream.
Silicone is the one with real backing. Silicone gel and sheets have been a first-line scar treatment for decades, worn most of the day for several months once the skin is fully closed. Don’t start them on an open wound. Wait for your surgeon’s clearance first.
The rest is basic, but it works:
Genetics and your surgeon’s closure technique still set the ceiling. These habits decide where you land under it.
