Semax. And not because it works better.
That’s the short answer to what’s better, Dihexa or Semax, and it will disappoint anyone hunting for a stack recommendation. Semax has three decades of clinical use behind it in Russia and a federal advisory vote in its favor from July 2026. Dihexa has rodent data, two retracted mechanism papers, and a related drug that failed in people.
Neither is FDA-approved. Neither can be legally compounded in the United States as of August 2026. Formation doesn’t carry either, and that fact is the most useful thing on this page.
Semax is a seven-amino-acid peptide derived from ACTH that raises BDNF and NGF signaling in the brain. Dihexa is a synthetic angiotensin IV analogue proposed to build new synapses through the HGF/c-Met pathway. Both get studied for cognition. Only one has ever been given to a human being under a published protocol.
What this article won’t do is hand you doses or a place to buy either compound.
Semax wins this comparison on evidence quality, not effect size. It has a human track record. Dihexa has none.
Semax has been an approved pharmaceutical in Russia since the mid-1990s. Those clinical series are mostly Russian-language and small, and none would satisfy an FDA reviewer. But real people were in them.
Dihexa has zero registered human trials. Not one. Its reputation rests on rodent work from a single laboratory and a potency figure that came out of a cell culture dish.
I have this conversation in the Tampa office most weeks, usually with someone holding up a phone. The thread always compares mechanisms. It never compares evidence quality.
Actually, let me correct the framing. Calling Semax the winner implies two products on a shelf waiting for you to pick one. They aren’t. Both sit outside legal compounding here, which makes this less a comparison than a lesson in reading a peptide claim. The advertised benefits of Dihexa peptides circulate faster than the corrections.
Semax has real human exposure, which is more than most research peptides can claim. It’s also thin by Western standards.
Russia has used it for ischemic stroke, cerebrovascular insufficiency, and recovery after brain injury. Published series report better neurological and cognitive scores against standard care, with transient nasal irritation the most common complaint from intranasal use.
Now the caveats. No large Western Phase 3 trial has ever run. Human pharmacokinetic data is limited. FDA reviewers flagged possible anti-thrombotic activity and an amphetamine-potentiation signal in 2026.
The mechanism holds up better than the clinical record. Semax raises brain-derived neurotrophic factor and nerve growth factor and modulates monoamine signaling, and 2024 work added antidepressant-like effects in chronically stressed rats.
People raise Selank in the same breath, since both came from the same Russian tradition. Semax leans toward focus and drive, Selank leans calming, and that’s why people take Selank for anxiety rather than cognition.
Because a university investigation found the images in it were falsified.
In April 2025, the Journal of Pharmacology and Experimental Therapeutics retracted two papers that built the entire scientific case for Dihexa. Kawas 2012 established it as an HGF-dimerization mimetic. Benoist 2014 supplied the causal evidence that its effects ran through HGF/c-Met. A Washington State University investigation found falsified and fabricated data in the figures. One author’s doctorate was revoked. A third paper from that lab, McCoy 2013, still carries a 2021 expression of concern.
Then there’s the human data problem. What exists is the record of a related compound. Fosgonimeton modulates the same HGF pathway, and its Phase 2/3 LIFT-AD trial ran 26 weeks across 287 people with mild-to-moderate Alzheimer’s. It missed its primary endpoint at p=0.70 and missed function alone at p=0.61. The company later settled False Claims Act allegations tied to the same research-integrity findings.
An independent Chinese group partially replicated Dihexa’s cognitive rescue in Alzheimer’s-model mice in 2021, through PI3K/AKT rather than the retracted HGF story. That’s the strongest outside evidence in existence. A mouse study.
Here’s what nobody selling Dihexa mentions. The famous claim that it’s ten million times more potent than BDNF came from a cell-culture assay in a paper that no longer exists.
Neither compound can be legally compounded in the United States today. Everything else is timeline.
On April 15, 2026, the FDA pulled twelve peptides off Category 2, the designation for substances that raise significant safety concerns. Semax and Dihexa were both on it. Plenty of clinics reported that as legalization. It wasn’t. Coming off Category 2 removes a prohibition. It does not put a substance on the 503A Bulks List, which is what authorizes a pharmacy to prepare something.
Seven of those twelve went before the Pharmacy Compounding Advisory Committee at its July 2026 advisory meeting. FDA staff recommended against all seven, citing weak chemical characterization and thin human effectiveness data. The committee overrode staff on six. Semax passed 8-5, and DSIP was the only rejection at 6-7. Dihexa wasn’t on that agenda, and it’s slated for separate review before the end of February 2027.
Advisory votes are non-binding. The Secretary of Health and Human Services still has to sign off, and final rulemaking has to be complete before a single vial is legal. That distinction applies to every compound in the batch, which is why April’s news was so widely misread.
Both aim at the same hallmark of aging, altered intercellular communication, through different signaling systems.
Semax addresses altered intercellular communication and chronic inflammation. Raising BDNF and NGF restores growth-factor signaling between neurons that fades with age, and the Russian stroke work showed Semax shifting immune gene expression in ischemic brain tissue, which touches inflammaging.
Dihexa’s proposed target is altered intercellular communication and nothing else. The synaptogenesis story is a claim about restoring cell-to-cell signaling in the hippocampus, and with the mechanism paper retracted it stays a hypothesis.
Cognitive decline isn’t one hallmark. It’s mitochondrial dysfunction, chronic inflammation, loss of proteostasis, and disabled macroautophagy pulling the same direction at once. A compound touching one axis does a fraction of the job, which is why mitochondrial and inflammatory support belong in the same peptide protocol as anything aimed at brain signaling.
Every compound in this comparison sits on the building side of a seesaw, and almost nobody talks about the other side.
Longevity isn’t maximizing growth. It also isn’t maximizing deprivation. It’s the movement between them. Day and night. Feeding and fasting. Training and recovery. The body evolved around cycles, and most health mistakes come from parking permanently on one side.
BDNF, NGF, HGF, synaptogenesis. All of it is building. Now name the renewal side of brain health. Autophagy. Mitophagy. Clearing misfolded protein. Deep sleep, when the glymphatic system flushes metabolic waste out of neural tissue.
The nootropic space is almost entirely growth-side, and that’s the position I’ll defend. Someone stacking growth-factor compounds on six hours of sleep is pressing the accelerator and the parking brake at once.
The lifestyle work that feeds this pathway is specific. Zone 2 cardio and short high-intensity intervals raise circulating BDNF. Resistance training plus protein covers the building side. A twelve-hour overnight fast and consistent deep sleep handle renewal. Heat exposure raises heat shock protein expression, which supports proteostasis. Do those and a peptide has something to fine-tune. Skip them and it has nothing to work with, whether the compound is a secretagogue like Sermorelin or a neuropeptide.
Push hard on a single pathway and the body routes around you.
That isn’t a metaphor. It’s regulatory biology. Receptors downregulate. Feedback loops tighten. Compensatory pathways pick up the slack until the same input stops working.
Which is the strongest argument against this article’s own framing. Choosing between Dihexa and Semax assumes the answer is one molecule. In practice, cognitive complaints get worked through several channels at once. Sleep architecture. Thyroid and sex hormones. Blood sugar stability. Inflammatory load, where a compound like KPV is one lever among several.
A patient who treats undiagnosed sleep apnea will out-perform that same patient on any nootropic peptide.

So the useful answer to what’s better, Dihexa or Semax, is that neither belongs in your body this month. That changes when final rulemaking lands. Until then, anyone selling you either one is selling an unregulated chemical with a story attached. Nobody knows, because it has never been tested in one. Dihexa has no registered human clinical trials.
