“NAD+ peptide” usually refers to therapies built around raising NAD+ levels, sometimes paired with actual peptide compounds in the same treatment plan. NAD+ itself works as an electron carrier for the reactions that convert food into cellular energy, and it activates sirtuins, a family of proteins tied to cellular aging and repair. Clinics frequently bundle NAD+ protocols with real peptides, similar to why people use Selank alongside other protocols for stress resilience, because the goals overlap even when the compounds don’t.
NAD+ levels decline as you age, an effect some researchers estimate at roughly half every couple of decades. That decline tracks with fatigue, slower metabolism, and several cellular processes tied to aging. NAD+ support, through precursors, IV therapy, or targeted peptides, aims to work with that decline rather than erase it.
NAD+ isn’t a peptide, plain and simple. A peptide is a short chain of amino acids linked by peptide bonds. NAD+ is a dinucleotide, with no amino acid chain and no peptide bond anywhere in its structure. Most of the industry glosses over this. Calling it a “peptide” is a marketing shortcut, not a chemistry fact.
That confusion isn’t harmless, either. It’s part of why patients sometimes end up sourcing compounds from providers who blur the line between legitimate medical-grade material and unregulated product. If you’re vetting a clinic, it helps to know the red flags in peptides before you commit to a protocol anywhere.
NAD+ and its reduced form, NADH, shuttle electrons through the reactions your mitochondria use to generate ATP, the energy currency your cells run on. Without enough NAD+, that shuttle slows down, and so does everything downstream of it.
NAD+ also fuels PARP enzymes, which repair damaged DNA, and sirtuins, which regulate inflammation and cellular stress responses. This is a different mechanism than growth hormone secretagogues like the one covered in how Sermorelin works, which act on a separate hormonal pathway. Know the difference before assuming one protocol can substitute for another.
We don’t treat “anti-aging” as one vague goal at Formation. We map it to specific, named mechanisms.
Hallmarks addressed by NAD+ support: mitochondrial dysfunction and genomic instability. NAD+ replenishment feeds the electron transport chain directly, which supports mitochondrial output as it drops with age. It also fuels the PARP enzymes that repair DNA, which counter the genetic damage behind genomic instability. A recent review of NAD+ precursor supplementation in humans found the mechanistic case strong but the tissue-specific clinical evidence still limited to a small number of studies. That gap matters, and I’d rather say it plainly than oversell what the human data shows.
At Formation, NAD+ protocols are administered by Jeff Atlas, PA-C, not by Dr. Gopal Grandhige himself. Jeff’s earlier career was in orthopaedics, and that background carries over into how he handles dosing and monitoring, especially for patients also working through joint or connective tissue recovery. Dr. Grandhige, a board-certified general surgeon who specializes in aesthetic surgery, oversees the clinic’s overall treatment philosophy as the owner of Formation, while the clinic’s plastic surgeon, Dr. Ahmad Saad, handles the surgical side of the practice separately from regenerative protocols like this one.
Formation only administers human-grade compounds. Research-grade peptides, the kind sold online without pharmaceutical oversight, are illegal to prescribe, purchase, administer, or dispense, and we don’t touch them. Our protocols are reviewed under SSRP and ISSCA certification standards, the two organizations that certify peptide, exosome, and stem cell practices. Not every patient is a candidate, either. The same logic behind avoiding Thymosin Alpha in the wrong patient applies before any NAD+ protocol here.
Neither wins outright, but oral precursors carry the stronger safety data and more expert support. That cuts against how the industry sells it. IV NAD+ gets marketed as the superior method almost everywhere. Eric Verdin, M.D., president of the Buck Institute for Research on Aging, said the opposite in a 2025 conversation with Peter Attia, calling IV NAD+ not worth the money next to oral precursors. The field hasn’t fully settled this, either. An active NAD+ trial is still studying vascular effects in older adults, which tells you the answer isn’t final.
Oral precursors (NMN/NR):
IV or injection NAD+:
Neither format is automatically “better.” It depends on your goals, your tolerance, and what your provider actually tracks as an outcome.
Longevity doesn’t come from maximizing anything. It comes from moving between two states: building and renewal. Growth mode includes protein intake, resistance training, and repair. Renewal mode includes fasting, autophagy, and cellular cleanup. Your body evolved around this oscillation, the same way it evolved around day and night or feeding and fasting.
Most health mistakes come from getting stuck on one side of that seesaw. Chasing NAD+ nonstop without ever supporting the renewal side is one version of that mistake. We call this framework the Rhythm of Longevity, and it shapes how we sequence NAD+ support alongside recovery options like BPC-157 when a patient’s goals call for it.
There’s also a compensation problem I should name plainly. Push one biological pathway hard enough and the body often routes around it through a different pathway, which blunts your results. That’s why we typically address a goal through more than one angle rather than leaning on a single peptide and hoping it carries the whole outcome.
The regulatory picture changed a lot in the past year. In September 2025, the FDA confirmed NMN is lawful for use in dietary supplements, reversing its 2022 position after a multi-year legal fight led by the Natural Products Association. That reversal restored consumer access to NMN products that had disappeared from major retailers.
Anyone promising guaranteed anti-aging results from NAD+ therapy in 2026 is overstating what the current evidence supports. The honest answer is that NAD+ shows clear biological activity and a much murkier record on measurable human outcomes.
Peptides and NAD+ protocols optimize a system. They don’t replace it. Your daily habits do the actual work, and the protocol just makes that work more efficient.
Sleep quality affects NAD+ turnover more than most patients expect. Resistance training pushes the building side of the seesaw, which pairs naturally with growth-pathway support such as ibutamoren for fat loss for patients already working that angle. Time-restricted eating and periodic fasting support the renewal side. They switch on autophagy and give cellular cleanup room to work. Alcohol limitation matters more than people want to hear, since it directly competes with NAD+ availability in the liver.
NAD+ peptide support works best as one piece of a coordinated plan, not a stand-alone fix. Get the lifestyle side right first. The protocol just makes it count for more.
No. NAD+ itself isn't FDA-approved as a treatment for aging or disease, and the term "peptide" is often used loosely here since NAD+ isn't chemically a peptide at all. NMN, a common NAD+ precursor, was confirmed lawful as a dietary supplement by the FDA in September 2025, but that's a supplement classification, not a drug approval.
NAD+ is a coenzyme, not a peptide, since it lacks the amino acid chain that defines a true peptide. "NAD+ peptide" is a marketing term that usually describes NAD+ therapy bundled with actual peptide compounds in the same treatment plan.
Not based on current evidence. Human trials show clear biological activity from NAD+ augmentation, but effectiveness for anti-aging outcomes specifically remains unsettled. It supports cellular processes tied to aging. It doesn't reverse the aging process itself.
This varies by person and by delivery method. Oral precursors build up gradually with daily use, while IV or injection protocols tend to produce a shorter, more immediate effect that requires repeat sessions to maintain.
Oral precursors like NMN and NR have a strong safety record at doses studied up to 2,000mg. IV and injection forms carry a higher chance of nausea, flushing, or a temporary rise in heart rate during administration, according to patient-reported data.
Yes, and it's common in regenerative medicine settings. Formation pairs NAD+ support with other peptide protocols based on individual goals, since addressing a single pathway alone often produces a weaker result than a coordinated approach.
Candidacy depends on medical history, current medications, and treatment goals, which is why a real consultation matters more than a generic answer here. It's the same screening logic we walk through for other peptides before starting any protocol.
The last one got 11/10.
Some of the biggest factors affecting liposuction results have nothing to do with what happens in the operating room.
Dr. Gopal rates 13 factors that can influence the final result, from working out before surgery and maintaining a stable weight to compression garments, skin elasticity, and lymphatic massage.
A few of his ratings might surprise you.
For example, he rates skin elasticity a 10/10 because even after the fat is removed, the skin still needs to contract and settle over the new contour.
And lymphatic massage? He gives it an 11/10 because of the role it plays in his recovery protocol, helping manage swelling and supporting how the skin settles over the newly sculpted areas.
The takeaway: great liposuction results depend on more than the surgery itself.
Patient preparation, anatomy, recovery, and long-term weight maintenance all play a role.

Six to seven hours.
That’s how long an HD liposuction procedure can take with Dr. Gopal’s approach.
He’s not simply removing fat. He’s evaluating every square inch, considering the underlying anatomy, and shaping how each contour transitions into the next.
It’s detailed, physically demanding work.
So when Dr. Gopal says, “Someone’s gotta sweat for the fat to come off,” he means it.

Will neck lipo give you the jawline you want?
It depends…
Someone can have fullness under the chin and assume the answer is neck liposuction. But what looks like “fat” from the outside can actually come from several different structures.
Some fat sits directly beneath the skin and can respond very well to liposuction.
But fullness can also come from deeper fat beneath the platysma, the salivary glands under the jaw, muscles under the chin, or even the natural projection of the chin and jaw itself.
That distinction can completely change what neck liposuction can realistically accomplish.
This is why evaluating the anatomy matters before deciding on a procedure.
For the right anatomy, neck liposuction can create a significant change in the neck and jawline.
But when deeper structures are contributing to the fullness, liposuction alone may not create the contour someone is expecting.
A good neck lipo consultation should figure that out first.

Want more definition than workouts gave you?
Traditional liposuction focuses primarily on removing fat. HD liposuction goes further. It requires understanding the underlying muscle anatomy, where to remove fat, where to preserve it, and how each contour works together to create natural-looking definition.
That’s the art of it. The goal isn’t to give every patient the same abs or the same body. It’s to work with the anatomy that’s already there and determine how much definition will look right for that individual.
Dr. Gopal Grandhige specializes in high-definition liposuction and advanced body contouring, using nearly two decades of surgical experience to create results that look sculpted, athletic, and intentional.

Defined abs with HD liposuction.
Unlike traditional liposuction, where the primary goal is removing excess fat, HD liposuction is about strategically removing fat to reveal and enhance the underlying muscle structure.
Those markings you see before surgery help Dr. Gopal map out the patient’s anatomy and plan where to remove fat and where to preserve it to create more visible definition.
The second clip was taken immediately after surgery, so this is not the final result. Swelling and bruising will change significantly as the body heals and the final contour develops over the months that follow.

Skin quality changes everything.
Neck liposuction can create a more defined jawline and neck contour by removing unwanted fat, but the result also depends heavily on skin quality and elasticity.
When skin has good elasticity, neck lipo alone may be enough for the skin to contract around the new contour. With some skin laxity, Dr. Gopal may combine liposuction with a skin-tightening treatment such as Renuvion or FaceTite. With significant laxity, a neck lift may be the more appropriate option.
That’s why evaluating the skin is an important part of determining the right approach before neck liposuction.

Liposuction can change more than the body.
I’ve watched patients become more confident, make changes in their careers, show up differently in their relationships, and start making different choices for themselves.
That’s when I know I’ve done my job well.
Because high-definition liposuction isn’t only about creating a leaner, more defined physique. Sometimes the physical transformation becomes the catalyst for changes that reach far beyond the operating room.

Male liposuction requires a different approach.
The contours that create a more feminine shape aren’t necessarily the same ones that create a strong, athletic male physique.
With male high-definition liposuction, Dr. Gopal considers how the chest, abdomen, waist, ribs and back work together—and how the underlying muscle anatomy should guide where definition and shadow are created.
That’s also why the chest matters. A highly defined abdomen and back paired with a rounder, less-defined chest can make the overall result feel out of proportion.
The goal is a cohesive, athletic contour that works with the patient’s natural anatomy.

6 months post high-definition liposuction with moderate definition.
The goal wasn’t just less fat. It was a leaner, more athletic contour with visible definition through the abdomen and torso, without pushing the result to the highest level of definition.
High-definition liposuction strategically sculpts around the underlying anatomy and can be customized based on a patient’s anatomy, muscle structure, and the degree of definition they want to achieve.
For this patient, moderate definition created the balance Dr. Gopal was looking for.
Results vary by patient and depend on individual anatomy, skin quality, and treatment plan.
