NAD+ peptide therapy is a way to refill a molecule your cells depend on to make energy and repair themselves. That molecule, NAD+, sits inside every cell you have. It helps turn food into usable fuel, backs up DNA repair, and switches on proteins tied to healthy aging. In many tissues, your NAD+ supply drops as you get older, and that dip lines up with lower energy, slower recovery, and a slower metabolism. People use NAD+ to push those levels back up.
What is NAD+? NAD+, short for nicotinamide adenine dinucleotide, is a coenzyme found in every living cell. It carries electrons during energy production and helps enzymes repair DNA, run metabolism, and manage inflammation. Your body makes it from vitamin B3 and other building blocks, and levels in your tissues tend to fall with age.
One clarification comes first, because it changes how you should read every ad you see.
No, NAD+ is not a peptide, and any honest provider will tell you so. A peptide is a short chain of amino acids. Insulin is one. NAD+ is a coenzyme, a different kind of molecule entirely.
So why does everyone say NAD+ peptide? Because wellness and IV clinics often give NAD+ by injection or drip, right alongside actual peptides, and the phrase stuck as a search term. At Formation, a regenerative medicine clinic in Tampa, NAD+ sits inside the regenerative program as its own service.
The label matters less than what the molecule does. Call it whatever you want. The real question is whether it earns a spot in your plan.
NAD+ has two main jobs: it helps make energy, and it helps run repair. On the energy side, your mitochondria, the tiny engines inside your cells, use NAD+ to turn food into ATP, the fuel that runs everything from a heartbeat to a heavy squat. Low NAD+ leaves those engines sputtering.
On the repair side, NAD+ feeds two sets of enzymes. Sirtuins, sometimes called longevity proteins, read your metabolic state and adjust how cells burn fuel, handle stress, and calm inflammation. PARP enzymes patch damaged DNA. The Cleveland Clinic puts it plainly: without NAD+, cells cannot produce energy well, and it supports muscle, brain, and heart function.
Strip NAD+ away and both jobs slow at once. That metabolic role is also why NAD+ keeps coming up in talk about weight and metabolism.
In most tissues, yes, NAD+ falls with age. But the story marketers tell is cleaner than the science. Direct measurements in skin, brain, muscle, and liver show NAD+ dropping over the decades, and some research points to it falling by roughly half by middle age. That part holds up.
Then a 2026 analysis in Nature Metabolism complicated things. Across seven groups of people, more than 300 in total, whole-blood NAD+ barely moved with age, and elite athletes matched sedentary people. The takeaway is not that decline is fake. It is that blood may be a poor mirror for what happens inside your organs, where levels still trend down.
So the honest read: tissue decline is well supported, but the scary blood number some clinics quote was never really measured.
NAD+ drops for a few reasons at once. DNA damage piles up and PARP enzymes burn through NAD+ fixing it. An enzyme called CD38 climbs with age and inflammation and eats NAD+ directly. And the recycling machinery slows down.
NAD+ touches at least five of the twelve hallmarks of aging, the cellular drivers researchers use to explain why bodies break down over time.
Hallmarks addressed: mitochondrial dysfunction, deregulated nutrient-sensing, genomic instability, cellular senescence, and chronic inflammation.
How each one works:
One molecule, five levers. That is why it draws so much attention.
Longevity is not about maxing out growth, and it is not about starving yourself thin. It is about moving between the two.
Picture a seesaw. One side is building: protein, resistance training, growth signals like IGF-1 and testosterone, mTOR switched on, recovery and repair. The other side is renewal: fasting, eating less, autophagy and mitophagy (your cells clearing out their own junk), AMPK active, damaged cells swept away. Health lives in the swing between them, the same way your body already runs on day and night, feeding and fasting, effort and rest.
Most mistakes come from parking on one side. Always eating, never fasting. Always training, never recovering.
NAD+ sits right at the hinge. It helps power the building side, since making energy needs it, and it powers the renewal side through sirtuin-driven cleanup and repair. That balance is the core of the clinic’s approach to alignment: match your inputs to where your body actually is, not to whatever is trending.
Most people want NAD+ for four things: more energy, faster recovery, sharper focus, and better body composition. Some of that has data behind it. Some of it is hope, and knowing which is which is the whole point.
The clearest human evidence sits with oral NAD+ precursors like NMN and NR, since NAD+ itself is hard for the body to absorb by mouth. A 2024 systematic review of ten trials, 489 people across conditions like chronic fatigue and prediabetes, found these compounds were well tolerated, with mostly minor side effects and some gains in fatigue, sleep, and insulin sensitivity. A larger 2026 review of more than a hundred studies agreed on one thing and hedged on another: NAD+ boosting is biologically active and generally well tolerated, but its clinical payoff for anti-aging and wellness stays inconclusive.
This is the part clinics gloss over. Almost none of that evidence tested IV or injected NAD+ in healthy people. The few solid IV trials are in specific medical conditions, not general wellness. So the biology is real, the precursor safety data are real, and the anti-aging promises are mostly still promises.
My honest take is simple. NAD+ can help. It is not a miracle, and any clinic guaranteeing one is selling you something.
Peptides blew up on social media. That does not make a molecule a fad.
Peptide culture right now runs on influencer sales funnels, gray-market vials, and people experimenting on themselves. That is a trend cycle, not a care model. The molecules underneath are not the fad. The way they are being sold is.
Remember that insulin is a peptide. Nobody calls insulin a trend. It is a molecule your own body makes, one that saves lives when it is dosed right and does real harm when it is not. Same logic runs across the board. Wanting more energy, better recovery, and a leaner body is a fine reason to be curious. The trouble starts when that curiosity meets hype instead of guidance.
A vial is not a plan. Self-experimenting off a research-chemical website is not the same as care a clinician steers, which is the standard Formation holds for peptides used in recovery and for NAD+ alike. The wider peptide space is under real regulatory pressure too. In July 2026, an FDA advisory panel spent two days weighing which peptides should even be legal to compound.
Because most NAD+ sold online is labeled not for human use, and that is not a technicality. It is a safety line.
In 2024, the FDA reminded clinics that food-grade NAD+ is unsuitable for sterile compounding and warned that patients who received it reported severe chills, shaking, vomiting, and fatigue, some needing medical care, all consistent with endotoxin contamination. Several compounded NAD+ injections were pulled from the market in 2024 and 2025 over contamination and endotoxin levels. This is not hypothetical.
The split is simple:
Formation only uses human-grade NAD+ from licensed compounding pharmacies, and does not offer research-grade at all. That same standard runs across the regenerative program, which holds SSRP and ISSCA certifications, the bodies that vet protocols for peptides, exosomes, and stem cells.
If a provider cannot tell you where their NAD+ comes from, treat that as your answer.
At Formation, NAD+ is treated as a medical service with a set process and a specific person doing the injections, not a walk-up menu item.
The injections are handled by Jeff Atlas, PA-C, not by the surgeon who owns the clinic. Jeff came to Formation from orthopaedics, where hands-on injection work was the daily job, and he brings that precision to every NAD+ appointment. The program is led by Dr. Gopal Grandhige, a board-certified general surgeon and the founder of Formation.
A NAD+ plan runs in five steps:
Formation’s main clinic is in Tampa, with services offered in other locations too. You can reach the team at (813) 922-2920.
NAD+ does not do the work. Your life does. NAD+ just helps the work you are already putting in pay off better. If you want the pathway to respond, these habits pull in the same direction:
Now the part most people miss. The body compensates. Push one pathway and it pushes back through another, so a single lever tends to get canceled out. CD38 can climb and burn NAD+ right back down while you try to raise it. That is why we chase a goal from several angles at once: raise the inputs, cut the drains, back the enzymes, and keep moving. One-trick plans underperform for a reason.
NAD+ peptide therapy refills a molecule your cells lean on for energy and repair. It can help. But it works as one tool inside a plan, never as the plan itself. The vial is not the strategy. What decides your result is whether the NAD+ is human-grade, whether a clinician is actually steering it, and whether your training, food, and sleep pull the same way. That is the whole game. If you want to know whether NAD+ is a fit for you, that starts with an in-person evaluation, not an order form.
NAD+ peptide therapy refills NAD+, a coenzyme your cells use to make energy and repair DNA. Restoring it may support metabolism, recovery, and cellular repair. It works best as one part of a medically guided plan, not a standalone fix. A 2024 review of ten trials with 489 people found NAD+ compounds were generally well tolerated.
No. NAD+ is a coenzyme, not a peptide, which is a short chain of amino acids like insulin. It gets grouped with peptide therapy because clinics often give it by injection or IV alongside peptides, which is how the term NAD+ peptide caught on.
In tissues like skin, brain, and muscle, yes, and some research points to it falling by roughly half by middle age. A 2026 Nature Metabolism analysis found whole-blood NAD+ stayed flat across ages, which suggests blood is a poor mirror for what happens inside your organs. Most experts still read the tissue evidence as a real, gradual decline.
NAD+ therapy is considered safe when a qualified clinician supervises it and the NAD+ is human-grade from a licensed compounding pharmacy. A 2024 review of ten trials reported a low rate of mostly minor side effects. Research-grade products sold online, labeled not for human use, skip safety testing, and the FDA has linked contaminated NAD+ to severe reactions.
Most NAD+ sold online is research-grade and labeled not for human use, so it is not tested for purity, potency, or sterility, and the vial may not match the label. Using research chemicals as medicine also falls outside the law. A clinic using human-grade, pharmacy-compounded NAD+ gives you a verified product under medical oversight.
Some people notice more energy or clearer focus within the first few sessions, while changes tied to metabolism and body composition take weeks of consistent treatment. Results lean heavily on sleep, training, and nutrition running alongside the therapy. NAD+ speeds up the work you are already doing; it does not replace it.
At Formation in Tampa, Jeff Atlas, PA-C gives the NAD+ injections and IV drips, so the treatment is not waiting on the clinic's surgeon-owner. Jeff came from orthopaedics and brings that injection experience. Dr. Gopal Grandhige, a board-certified general surgeon and the founder of Formation, leads the regenerative program.
The last one got 11/10.
Some of the biggest factors affecting liposuction results have nothing to do with what happens in the operating room.
Dr. Gopal rates 13 factors that can influence the final result, from working out before surgery and maintaining a stable weight to compression garments, skin elasticity, and lymphatic massage.
A few of his ratings might surprise you.
For example, he rates skin elasticity a 10/10 because even after the fat is removed, the skin still needs to contract and settle over the new contour.
And lymphatic massage? He gives it an 11/10 because of the role it plays in his recovery protocol, helping manage swelling and supporting how the skin settles over the newly sculpted areas.
The takeaway: great liposuction results depend on more than the surgery itself.
Patient preparation, anatomy, recovery, and long-term weight maintenance all play a role.

Six to seven hours.
That’s how long an HD liposuction procedure can take with Dr. Gopal’s approach.
He’s not simply removing fat. He’s evaluating every square inch, considering the underlying anatomy, and shaping how each contour transitions into the next.
It’s detailed, physically demanding work.
So when Dr. Gopal says, “Someone’s gotta sweat for the fat to come off,” he means it.

Will neck lipo give you the jawline you want?
It depends…
Someone can have fullness under the chin and assume the answer is neck liposuction. But what looks like “fat” from the outside can actually come from several different structures.
Some fat sits directly beneath the skin and can respond very well to liposuction.
But fullness can also come from deeper fat beneath the platysma, the salivary glands under the jaw, muscles under the chin, or even the natural projection of the chin and jaw itself.
That distinction can completely change what neck liposuction can realistically accomplish.
This is why evaluating the anatomy matters before deciding on a procedure.
For the right anatomy, neck liposuction can create a significant change in the neck and jawline.
But when deeper structures are contributing to the fullness, liposuction alone may not create the contour someone is expecting.
A good neck lipo consultation should figure that out first.

Want more definition than workouts gave you?
Traditional liposuction focuses primarily on removing fat. HD liposuction goes further. It requires understanding the underlying muscle anatomy, where to remove fat, where to preserve it, and how each contour works together to create natural-looking definition.
That’s the art of it. The goal isn’t to give every patient the same abs or the same body. It’s to work with the anatomy that’s already there and determine how much definition will look right for that individual.
Dr. Gopal Grandhige specializes in high-definition liposuction and advanced body contouring, using nearly two decades of surgical experience to create results that look sculpted, athletic, and intentional.

Defined abs with HD liposuction.
Unlike traditional liposuction, where the primary goal is removing excess fat, HD liposuction is about strategically removing fat to reveal and enhance the underlying muscle structure.
Those markings you see before surgery help Dr. Gopal map out the patient’s anatomy and plan where to remove fat and where to preserve it to create more visible definition.
The second clip was taken immediately after surgery, so this is not the final result. Swelling and bruising will change significantly as the body heals and the final contour develops over the months that follow.

Skin quality changes everything.
Neck liposuction can create a more defined jawline and neck contour by removing unwanted fat, but the result also depends heavily on skin quality and elasticity.
When skin has good elasticity, neck lipo alone may be enough for the skin to contract around the new contour. With some skin laxity, Dr. Gopal may combine liposuction with a skin-tightening treatment such as Renuvion or FaceTite. With significant laxity, a neck lift may be the more appropriate option.
That’s why evaluating the skin is an important part of determining the right approach before neck liposuction.

Liposuction can change more than the body.
I’ve watched patients become more confident, make changes in their careers, show up differently in their relationships, and start making different choices for themselves.
That’s when I know I’ve done my job well.
Because high-definition liposuction isn’t only about creating a leaner, more defined physique. Sometimes the physical transformation becomes the catalyst for changes that reach far beyond the operating room.

Male liposuction requires a different approach.
The contours that create a more feminine shape aren’t necessarily the same ones that create a strong, athletic male physique.
With male high-definition liposuction, Dr. Gopal considers how the chest, abdomen, waist, ribs and back work together—and how the underlying muscle anatomy should guide where definition and shadow are created.
That’s also why the chest matters. A highly defined abdomen and back paired with a rounder, less-defined chest can make the overall result feel out of proportion.
The goal is a cohesive, athletic contour that works with the patient’s natural anatomy.

6 months post high-definition liposuction with moderate definition.
The goal wasn’t just less fat. It was a leaner, more athletic contour with visible definition through the abdomen and torso, without pushing the result to the highest level of definition.
High-definition liposuction strategically sculpts around the underlying anatomy and can be customized based on a patient’s anatomy, muscle structure, and the degree of definition they want to achieve.
For this patient, moderate definition created the balance Dr. Gopal was looking for.
Results vary by patient and depend on individual anatomy, skin quality, and treatment plan.
