Tirzepatide, on the other hand, works as a dual agonist, targeting both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors simultaneously.
Depending on the formulation, tirzepatide serves different purposes. As Mounjaro, it is prescribed for diabetes management. As Zepbound, it is FDA-approved specifically for weight loss.
Tirzepatide and semaglutide both belong to the GLP-1 agonist drug class. Medications in this class work by mimicking the GLP-1 hormone naturally found in the body, which sends signals to both the brain and the pancreas to influence blood sugar levels, digestion and feelings of fullness.
Those signals prompt the pancreas to produce more insulin, helping keep blood sugar in check. GLP-1 also slows gastric emptying, meaning food stays in the stomach longer and keeps you feeling satisfied well after a meal. On top of that, it communicates with the brain in a way that can reduce what’s often called “food noise,” which refers to the persistent, intrusive thoughts about eating that many people struggle with.
Tirzepatide also includes GIP, or glucose-dependent insulinotropic polypeptide, an additional hormone that works alongside GLP-1 to further support insulin production. Mounjaro and Zepbound are both manufactured by Eli Lilly, while Wegovy and Ozempic come from Novo Nordisk. Depending on the brand, you may find semaglutide positioned as either a weight loss or type 2 diabetes treatment. The same applies to tirzepatide.
It’s also worth noting that two medications sharing the same active ingredient and manufacturer are not automatically interchangeable. Each drug carries its own formulation, dosing protocol and approved indication, so what works for one patient may not be the right fit for another.
Both semaglutide and tirzepatide have shown strong results for weight loss. But when comparing the two, which one actually performs better for weight management?
According to Eli Lilly, the manufacturer of the tirzepatide weight loss formulation, people taking the 15 mg dose lost up to 48 pounds, representing roughly a 21% reduction in body weight.
One meta-analysis put the highest dose of tirzepatide (15 mg) head-to-head against the highest dose of semaglutide (2 mg). Tirzepatide came out ahead with a 17.8% reduction in body weight compared to 12.4% for semaglutide.
A separate cohort study also compared the two medications and found that tirzepatide led to 4.3% greater weight loss at six months and 6.9% greater weight loss at 12 months. The dual mechanism targeting both GLP-1 and GIP receptors is likely what drives tirzepatide’s stronger results in both average weight loss and blood sugar regulation.
One important factor to consider when reviewing semaglutide research is the dosage used in each study. Many earlier clinical trials were conducted using the 1 mg dose, but the current maximum has since increased to 2 mg. Research confirms that the higher dose improves efficacy, particularly for glycemic control and weight loss.
It’s worth noting that while semaglutide is not formally classified as a weight loss drug, it can contribute to weight reduction. Adults with type 2 diabetes using it have lost up to 14 pounds based on manufacturer data. Ultimately, which medication is right for you comes down to your doctor’s guidance. As one registered dietitian put it, both medications are strong choices for managing type 2 diabetes, but the decision between tirzepatide and semaglutide depends on each patient’s individual needs, weight management goals and how they’ve responded to previous treatments.
One practical factor worth keeping in mind: physicians sometimes prescribe based on what’s actually available at the time. Both tirzepatide and Ozempic have experienced recurring supply shortages due to high demand and limited production capacity, so availability can play a role in what gets prescribed.
Both medications carry a range of potential side effects worth knowing before you start treatment.
Since both are administered as weekly injections, allergic reactions at the injection site are possible with either drug. Common injection areas include the upper arm and abdomen. Always clean and disinfect the site thoroughly before each injection to reduce irritation and infection risk.
