A skin flap is living tissue that a surgeon moves from one part of the body to another while keeping its blood supply attached. That blood supply is the whole point. It lets the moved tissue stay alive and rebuild a wound that a simple skin graft could not.

Flaps sort into three broad groups, based on how far the tissue travels and how its blood supply is handled.
Local flaps use tissue right next to the wound. The surgeon slides or rotates skin into the gap while it stays connected to its original blood supply. These are common on the face after skin cancer, and they give the best color and texture match because the skin came from inches away.
A regional flap moves tissue from a nearby zone on a stalk of blood vessels called a pedicle. The stalk keeps the tissue alive during and after the move. In some staged cases, the surgeon divides that pedicle a couple of weeks later, once the flap has built new connections in its new home.
A free flap is fully detached, then reattached at the new site by reconnecting the blood vessels under a microscope. This is microsurgery. Free flaps handle the largest and most complex wounds and can move skin, fat, muscle, or even bone. They also carry the highest demands for monitoring.
Surgeons move a patient’s own fat for aesthetic shaping too, a separate procedure called fat transfer. That isn’t a flap, though, because the fat doesn’t carry its own vessels.
Flap choice drives risk. In nasal reconstruction after skin cancer, pooled results put overall complications near 11%, and regional flaps ran highest at about 26%. Those numbers pool many small studies with different defects, so read them as a pattern, not a promise.
Recovery depends on the flap. Many local flaps go home the same day, and one 2025 review of outpatient flap surgery found major complications under 5% for even sizable local flaps. Free flaps often need a hospital stay with close checks for the first 2 to 3 days, because that’s the window when blood flow problems show up and can be fixed.

The risks you can move are the ones that matter most. Smoking, diabetes, a higher BMI, high blood pressure, and long operating times all raise the chance of trouble. Now the contrarian part. Bigger reconstruction isn’t automatically better. A free flap can add bulk you didn’t want and a second surgical site you also have to heal.
