Plastic surgery can remove stretch marks, but only the ones sitting in skin that gets cut away and thrown out. That’s the honest answer, and most clinics won’t give it to you that plainly. A stretch mark inside the lower abdominal skin removed during a tummy tuck is gone for good. A stretch mark above your navel gets repositioned lower on your torso and stays exactly as visible as it was. Lasers and microneedling can soften what’s left. Neither erases it.
I’ve had this conversation hundreds of times in Tampa. It usually starts with a patient holding up a phone, showing a before-and-after that promised far more than any operation can deliver.
Stretch mark removal through plastic surgery means excising skin that contains the marks, most often during abdominoplasty, a thigh lift, or a lower body lift. Marks in the removed tissue never come back. Marks outside the excision zone remain, though their position on the body shifts. No operation erases stretch marks from skin it doesn’t take out.
Stretch marks are scars that form in the dermis, the layer of skin sitting under the surface you can see. They don’t fade completely because the collagen and elastin fibers that tore never rebuild in their original pattern.
Skin stretches or shrinks faster than it can adapt, the support fibers in the dermis tear, and the thinner tissue that heals over that tear shows through the surface. Pregnancy does it. So do growth spurts, fast weight gain, fast weight loss, long-term steroid use, and a handful of medical conditions like Cushing’s syndrome.
The largest review of the literature on this subject, published in Dermatologic Surgery in 2024, pooled 151 studies and 4,806 treatment outcomes. About 83% of those patients were women, with an average age of 30 at diagnosis. That profile matches almost exactly who sits in my consultation room asking about this, often within two years of a pregnancy or a big drop in weight.
Color tells you more than size.
Fresh marks look pink, red, or purple because the torn dermis lets blood vessels show through. Older marks go pale, sometimes silvery white, and by then the vessels have receded and the collagen has settled. Fresh marks respond to treatment. Mature white ones barely budge. Anyone selling you the same protocol for both isn’t paying attention.
No non-surgical treatment removes a stretch mark. Energy devices and needling improve color and texture, and the ceiling on that improvement is documented well enough that nobody in this field should be surprised by it.
In that same 2024 review, complete clearance happened in roughly 3% of cases. About 90% of outcomes landed at partial improvement. Energy-based devices accounted for 56% of all treatments studied, injection-based treatments topped the categories for complete response at around 7%, and topicals came in near the bottom. Read those numbers again. The best-performing category in the largest dataset we have clears fewer than one in ten cases.
That’s not an argument against treatment. Partial improvement is real, and patients are often happy with it. It’s an argument against the way it gets sold. Device makers publish improvement percentages that describe how much better a mark looks, then let patients hear the word removal. Those are two different claims.
I’m skipping over-the-counter creams and at-home rollers here. They’re mostly harmless, and there’s nothing to weigh. Prescription tretinoin has modest support for fresh marks. The rest of that shelf does nothing a moisturizer wouldn’t. If your real goal is to tighten skin without surgery, that’s a separate conversation about laxity, not about scars.
Yes, mostly the color. Lasers tuned to red and brown pigment pull the pink out of fresh marks and even out the tone of older ones so they blend with surrounding skin.
Fractional CO2 laser sits at the aggressive end of that range. A 2025 meta-analysis in Lasers in Medical Science pooled six randomized trials and found CO2 laser improved appearance in more than 70% of patients, which sounds excellent until you read the safety data. Compared with microneedling, the CO2 group carried a much higher pigmentation risk, with a relative risk above eight for post-inflammatory hyperpigmentation. Eight times the risk, on a cosmetic scar, in a state where people are outdoors year-round.
Microneedling works on the raised, dented, or uneven marks rather than the discolored ones. Fine needles create controlled injuries at a set depth, the body rebuilds collagen in response, and the surface smooths out over several sessions.
Adding radiofrequency energy to those needles reaches deeper into the dermis, which is why radiofrequency microneedling tends to outperform plain needling on wider, older marks. In that head-to-head 2025 analysis, improvement scores and patient satisfaction were statistically indistinguishable between microneedling and CO2. Same result, less pigment risk. Adding platelet-rich plasma pushes the response higher in several studies, which tracks with injection-based treatments posting the highest complete-response rate.
For the abdomen, a full abdominoplasty removes roughly 40% to 60% of existing stretch marks. That figure holds up across surgeon reporting and the clinical literature, and it’s the number I quote in consultation because it sets expectations that survive the first post-op photo.
The geometry explains it. Surgeons performed just over 171,000 tummy tucks in 2024 by ASPS counts, and a standard full abdominoplasty takes out a horizontal ellipse of skin running from around the pubic line up to the belly button. Stretch marks living inside that ellipse leave with the tissue. Scar revision volume rose 4% the same year, to 54,280 procedures, which tells you how many people come back later to deal with what got left behind.
The excision zone is fixed by anatomy, not by preference. Skin between the pubic bone and the navel comes out, the upper abdominal skin gets pulled down to close the gap, and a scar sits low across the hips.
So marks on your lower belly are gone. Marks above the navel get relocated to somewhere below it. Actually, relocated isn’t quite the right framing. They get stretched and flattened as the upper skin is pulled taut, so they often look thinner and less obvious than before, just sitting in a lower position. Better, not gone. Patients who came in for excess abdominal skin and muscle repair almost always count that as a win. Patients who came in purely for stretch marks feel misled, which is why I won’t operate on the second group.
Nobody should have abdominoplasty for stretch marks alone. Full stop. The right candidate needs the surgery for another reason, and mark removal is a side benefit that arrives free with the excision.
That means loose skin that won’t retract, separated abdominal muscles, or a skin apron after major weight change. Demand in this category keeps climbing. ASPS logged thigh lifts up 3% and arm lifts up 2% in 2024, growth the society ties to patients addressing excess skin after weight loss medication. Plenty of those patients arrive after major weight loss carrying both problems at once, which is when surgery makes sense on the merits.
If you’re at a stable weight with tight skin and flat muscles, and the only thing bothering you is a set of white marks across your hips, surgery is the wrong tool. Energy-based treatment is the honest recommendation, with honest limits attached to it.
Postpartum patients get the worst advice in this category, and the reason is that timing gets skipped. Most pregnancy stretch marks show up in the third trimester, arriving red or purple. That is the exact window where treatment works best.
That creates real tension. Fresh marks respond to energy-based treatment far better than mature ones do, but abdominoplasty should wait until you’re finished having children. Another pregnancy stretches repaired muscle and tightened skin all over again, and patients who go through that often end up weighing a second tummy tuck years later. I’ve watched that happen twice this year alone.
My recommendation splits the difference. If your family is complete, wait until your weight has held steady for six months and go straight to surgical planning, because the marks will still be there and the excision handles them. If another child is possible, treat the fresh marks non-surgically now while they’re still red, accept that you’re improving appearance rather than removing anything, and revisit surgery later.
That second path frustrates people who wanted one decision. It’s still the right call. Operating on an abdomen with another pregnancy ahead of it throws away a good result.
Skin tone belongs in this conversation and almost never makes it in. Patients with Fitzpatrick IV through VI skin carry a much higher risk of post-inflammatory hyperpigmentation from ablative laser, which can leave a darker patch that bothers them more than the original mark ever did.
That eightfold pigmentation risk in the CO2 data isn’t spread evenly across patients. It concentrates in darker skin. For those patients I steer toward needling before anything ablative, accept a slower timeline, and treat one small test area first. Losing three months to a careful protocol beats spending a year fading pigment you caused.
The pattern goes like this.
A patient spends eighteen months on serial laser and needling courses aimed at mature white marks across a lower belly that also carries loose skin and a two-finger muscle separation. The marks improve slightly. The skin still hangs. Then they book an abdominoplasty, which excises half those same marks in one operation.
Sequence matters more than modality. If abdominoplasty is already on your horizon for any other reason, do that first, then treat the marks above your navel six months later. Doing it the other way around means treating tissue that was headed for the bin.
The trade-off is real, and I’ll name it. Surgery gives you a permanent horizontal scar in exchange for permanent mark removal. Some patients look at scar placement diagrams and decide a fine line low on the hips beats a field of striae. Others don’t, and that’s a reasonable call. I’ve never once talked somebody into that trade, and I don’t try.

The excised marks cannot return, because that skin is gone. New marks can form later if a future pregnancy or major weight change stretches the remaining skin faster than it can adapt.
