Most lumps from PDO threads clear on their own. The ones that don’t respond to one of three things. Guided manual release, a low-dose anti-inflammatory injection, or taking the thread out. Which one you need depends on what the lump is actually made of, and that’s the part patients almost never get right by feel alone.
I’ve evaluated plenty of people at Formation in Tampa who arrived at week two certain something had gone wrong. Usually nothing had. In a few cases something had, and another eight weeks of waiting would have made it harder to fix.
This article skips filler lumps. Hyaluronidase dissolves hyaluronic acid. It does nothing to polydioxanone. If someone offers you hyaluronidase for a thread lump, ask why.
Getting rid of lumps from PDO threads means matching the treatment to the cause. Swelling and bruising settle in one to three weeks with no help. Tension dimpling responds to manual release. Inflammatory nodules respond to a low-dose steroid. Threads placed too shallow or bunched at a barb usually come out.
Six causes, and they aren’t equally serious. Five resolve on their own. One needs treatment the same day.
Your skin reacts to being threaded. Puffiness, firmness, and tenderness show up within hours. This is the most common cause by a wide margin and the least worrying. It fades over one to two weeks.
A thread placed too close to the surface reads as a ridge or a thin line you can trace with a fingertip. Thin skin shows it more. That’s a placement problem, not a healing problem, so it won’t fade on schedule.
The cogs on barbed threads grip tissue on purpose. Pull too hard, or pull against tissue that’s already tight, and those cogs gather into a firm bead. This is the lump patients describe as a knot.
Threads lift in a direction. Too much tension in that direction puckers skin into a dimple. Dimpling is the most reported thread complication in the literature, and it’s the one that responds best to early intervention.
Blood pools, the spot feels soft and looks discolored, and both fade together. Nothing to do.
Red, hot, growing, painful, draining, or paired with a fever. Call someone. Today. This is the one item on the list where waiting costs you something real.
Yes, and the published numbers are messier than clinics admit. A 2026 meta-analysis in Frontiers in Surgery pooled 26 studies covering 2,827 patients and reported swelling in 34% of cases, visible or palpable threads in 10%, and dimpling in 7%. The preprint version of that same analysis put swelling at 16% and palpable threads at 6%. Same authors, same studies, very different headline figures.
Real-world numbers land in similar territory. A 2025 multicenter review of more than 110,000 cases found dimpling in roughly 12% of the patients it tracked for complications, with palpable threads in about 2%.
Marketing rounds all of that down to “rare.” It isn’t rare. It’s usually temporary, which is a different claim.
A normal lump softens week over week for the first three weeks. A lump that holds steady or grows past week six isn’t doing that. Six weeks is my cutoff for calling something in. Four is fine too if it’s bothering you.
Treatment escalates. We start with the least invasive option that could plausibly work and stop as soon as the lump softens.
Jeff Atlas, PA-C does the hands-on work. He came out of orthopaedics, which matters more than it sounds, because releasing a bunched thread is a tissue-plane problem before it’s a cosmetic one. I handle the assessment and decide whether anything should be touched at all.
Don’t massage it on your own. This is the most common way patients turn a two-week problem into a two-month one. Rubbing hard on a threaded plane can deepen a dimple or shift a thread off its vector.
One thing I’ll argue with colleagues about: the standard reassurance that threads dissolve so lumps always resolve is about two-thirds true. PDO absorbs over roughly six to eight months. A true granuloma, an infection, or a thread bunched at a cog doesn’t politely wait that out. Documented complications include extrusion and skin erosion, and those get worse with time.
Also skip side-sleeping on the treated area the first week, hold off on heavy training, and keep big mouth movements modest for a few days. Boring advice. It works.
Five signals mean stop waiting and get seen.
Any one of these is enough. You don’t need all five. None of this means a PDO thread lift was the wrong choice for you. It means this particular lump needs eyes on it, not another week of hoping.
Most lump problems are placement problems, which means they get prevented at the consultation, not the follow-up.
Thick tissue hides material. Thin tissue shows everything. Matching thread type and depth to the tissue in front of you is the whole job, and it’s why I turn some people down for threads and point them toward energy-based tightening instead.
Ask these five things before you agree to anything.
Question five separates providers. Anyone who says yes to everyone is selling.
Minor bumps and puffiness run one to two weeks. Dimpling and puckering run two to six weeks. A bunched knot runs one to three months and often needs help. Full absorption of the polydioxanone takes about six to eight months, which is why a hard nodule can outlast the swelling by a long stretch.
The collagen response is why the wait is tolerable. New collagen builds through months two to six, and skin that felt uneven at week four often reads smooth by month three with nobody touching it.
Median patient age for thread lifts climbed from 34 to 42 between 2020 and 2024 in that same multicenter review. Older skin holds swelling longer. If you’re 45, your week three doesn’t look like a 30-year-old’s week three, and almost nobody warns you.
A lump is a diagnosis before it’s a treatment. Massage the wrong lump and you deepen it. Inject the wrong lump and you thin the skin over it. Wait on the wrong lump and you end up pulling a thread out of inflamed tissue instead of quiet tissue.
If you’re in Tampa and something under your skin isn’t settling, get it looked at by someone who names what it is before touching it, whether or not they did your original PDO thread treatment. Getting rid of lumps from PDO threads is rarely the hard part. Identifying them correctly is.
You mostly don't, and that's the honest answer. Swelling and bruising resolve on their own within one to three weeks with no intervention. Anything firm, ridged, or still present at week four needs a provider to identify it first, because the treatment for a dimple and the treatment for a nodule are opposite.
Puffiness and minor bumps last one to two weeks. Dimpling and puckering last two to six weeks. A bunched knot can persist one to three months, and full absorption of the polydioxanone itself takes roughly six to eight months, which is why a hard nodule sometimes outlasts everything else.
Only if your provider specifically told you to, and only the way they showed you. Unguided massage on a threaded plane can deepen a dimple or move a thread off its vector, which turns a short problem into a long one.
A low-dose anti-inflammatory injection is a standard first-line treatment for firm inflammatory nodules, and it often softens them in one or two sessions. It gets used sparingly because repeated steroid in one location thins the skin. Since a 2026 meta-analysis of 2,827 patients found visible or palpable threads in 10% of cases, this comes up more often than clinic marketing suggests.
No. Hyaluronidase breaks down hyaluronic acid filler and has no effect on polydioxanone. Providers occasionally use it on surrounding filler when both were placed in the same area, but it won't touch the thread itself.
Barbed and cog threads create mechanical lift, and that tension is what produces most dimpling and bunching, so yes, they carry more of this risk than smooth mono threads. Dimpling turned up in roughly 12% of tracked patients in a 2025 review of more than 110,000 facial thread lifting cases.
Removal gets considered for thread extrusion, infection, a true granuloma, or a bunched thread that hasn't released after conservative treatment. A micro-incision or cannula technique takes it out, ideally once inflammation has settled rather than during an active flare.
