PRP vs laser therapy for hair loss isn’t one treatment beating the other. They do different jobs on different timelines. PRP puts concentrated growth factors into the scalp in a clinic, usually a month or so apart. Laser therapy is red light you wear at home, several sessions a week, indefinitely. PRP works faster and reaches deeper. Laser asks for nothing but consistency.
Neither one rebuilds a follicle that’s already gone. That single fact settles more of this comparison than anything else, and almost nobody writing about it says so plainly.
So the real question isn’t which treatment is stronger. It’s how much living follicle you still have, and what you’re willing to keep doing for the next five years.
PRP is your own blood, spun down and put back. A vial is drawn, placed in a centrifuge, and separated so the platelet layer concentrates. That plasma gets injected across thinning areas of the scalp, where platelet growth factors signal the follicle and the tissue around it to behave younger than it has been.

Hallmarks addressed: stem cell exhaustion, altered intercellular communication, and chronic inflammation. Concentrated platelets deliver growth factor signaling to dermal papilla cells that have gone quiet, and they act on the low-grade inflammation that sits around miniaturizing follicles in pattern hair loss.
Low-level laser therapy is red light in a narrow band, absorbed by mitochondria inside the follicle cells. The working theory is that light at those wavelengths raises cellular energy production, improves scalp circulation, and holds follicles in their growing phase longer. It’s a nudge, applied often, over a long time.
The evidence has gotten better recently. A 12-month prospective trial published in Dermatologic Therapy followed 68 adults using a helmet device three times a week for 20 minutes. Hair density rose from about 99 hairs per square centimeter to 124, roughly a 25 percent gain, with shaft thickness up around 15 percent. Nearly 59 percent were rated improved by investigators, and more than 98 percent held steady or improved. No device-related adverse events showed up across the full year.
The authors printed their own limitation alongside it. The trial was open-label with no sham control, so placebo effects can’t be ruled out, and it tested a single device at a single setting.
Hallmarks addressed: mitochondrial dysfunction and altered intercellular communication.
Depth. That’s the difference worth caring about.
PRP is placed where the follicle lives. Light has to travel through skin to get there, which is why laser protocols run for months before anything measurable happens. One is delivered, the other is transmitted.
The second difference is who does the work. PRP is a clinic procedure with a person making decisions about placement. A laser cap does the same thing on your head that it does on everyone else’s. Preparation varies too, which is why PRP and PRF produce different concentrations from the same draw.

For measurable density gain in a defined window, PRP is the stronger tool. For holding ground over years without effort, laser therapy is hard to beat. Both sit below medication in the evidence hierarchy, and pretending otherwise does patients no favors.
A 2025 meta-analysis in Aesthetic Plastic Surgery pooled nine randomized trials covering 451 patients and compared PRP against 5 percent topical minoxidil. Hair density came out statistically similar between them. Patient satisfaction favored PRP with an odds ratio of 2.77, and negative hair pull tests ran 82.75 percent with PRP against 52.94 percent with minoxidil. The authors flagged high heterogeneity across the included studies and called for larger standardized trials, which is the correct way to read that result.
Put those two datasets next to each other and the pattern is clear. PRP moves shedding and patient-perceived improvement more than it moves raw counts. Laser moves counts slowly and steadily, then keeps them there. Anyone claiming a clean win for either side is selling something.
That’s also why PRP works better as part of a plan than as the plan.
Candidacy is where these two diverge hardest, and the FDA record makes it unusually easy to check.
Home laser devices are cleared under product code OAP, a Class II designation reviewed through the 510(k) pathway. The FDA classification defines the device around promoting hair growth in men with androgenetic alopecia at Norwood-Hamilton IIa through V. Individual clearances extend that to women at Ludwig-Savin I through II and Fitzpatrick skin types I through IV. That’s early to moderate pattern loss, not a scalp that’s already bare. And clearance means substantial equivalence to a device already on the market, not proof that it works for you.
PRP has a wider practical range but the same biological ceiling. It suits people with visible thinning who still have miniaturized follicles producing fine hair, people who can’t tolerate finasteride or minoxidil, and people already on medication who want more from it. In bald areas where the follicle is gone, expectations have to change before treatment starts.
One thing worth saying out loud is that PRP for hair loss isn’t FDA approved. It’s off-label use of your own blood, which is legal and routine, and any clinic implying otherwise is being loose with language.
Diagnosis comes first either way. Thyroid disease, iron deficiency, postpartum shedding and autoimmune hair loss all look like pattern loss in a mirror and respond to entirely different things. Treating the wrong condition with the right tool wastes a year.
Yes, and the biology argues for it.
Pattern hair loss runs through several pathways at once. DHT sensitivity shrinks the follicle. Circulation and mitochondrial output fall. Growth factor signaling drops off. Low-grade inflammation settles in around the follicle. Push hard on one pathway and the body routes around it through the others, which is why single-agent protocols plateau and combinations keep climbing.
That’s the same principle behind everything we do in regenerative medicine. Pressure one lane, the body compensates in another lane, and the result comes out weaker than it should have.
A workable stack looks like medication for the DHT pathway, laser for the mitochondrial and circulatory side, and PRP for the growth factor signal. Each covers a different failure. None of them create new follicles, and that limit applies no matter how many you combine.
Both treatments are maintenance, not repair. Stop either one and the follicle returns to whatever it was doing before you started.
PRP runs as an initial series spaced several weeks apart, then periodic sessions after that. Most people see nothing for the first two months, then notice shedding slow before they notice thickness. Laser needs several 20-minute sessions a week without gaps. In the 12-month trial, density changes reached statistical significance at 16 weeks and thickness took until 24, which tells you what patience the protocol requires.
The mistake I see most often has nothing to do with which treatment someone picked. It’s stopping after a strong first year, then coming back 18 months later surprised that the gain left with it. That’s also why when to start matters less than whether you’re prepared to keep going.
Photograph your scalp before anything begins. Same light, same angle, same part. Hair changes too slowly for memory to track, and without a baseline you’ll spend the year arguing with yourself about whether it’s working.
Hair follows a rhythm. It builds, it rests, it sheds, it builds again. Pattern hair loss is that rhythm falling out of balance, with the building phase shortening cycle after cycle until the follicle produces something too fine to count.
