A PRP injection for knee arthritis uses a concentrated dose of your own platelets to calm joint inflammation and nudge tissue toward repair. It won’t rebuild a worn knee from scratch. For the right patient, it can lower pain and improve movement for roughly 6 to 12 months. That’s the honest version, and it’s the one most clinics skip.
I’ve watched people pin an entire recovery on a single shot, then wonder why their knee still aches. PRP is a tool. Not a cure. And where you get it done matters as much as whether you get it done at all.
A PRP injection for knee arthritis is a shot of platelet-rich plasma made from your own blood. A provider draws a small sample, spins it in a centrifuge to concentrate the platelets, then injects that plasma into the knee joint to reduce pain and support healing.
Your platelets do more than clot. They hold growth factors, tiny signals that tell nearby cells to repair damaged tissue. PRP is a biologic made from your own blood, not a synthetic drug. Concentrate those platelets and you get a denser dose of those signals in one spot.
So this isn’t a medication. It’s your own biology, spun down and placed where the joint hurts. At Formation, a regenerative medicine clinic in Tampa, that plasma never leaves the person it came from.
PRP works by flooding the joint with growth factors that dial down inflammation and signal local cells to repair. It doesn’t grow new cartilage on command. It changes the chemistry of a joint stuck in a low-grade inflammatory loop.
An arthritic knee isn’t just worn down. It’s inflamed. That constant, low-level inflammation, sometimes called inflammaging, keeps breaking tissue down faster than the joint can rebuild. A high concentration of growth factors can shift that balance for a while.
Hallmarks of aging addressed: chronic inflammation and altered cell signaling. PRP works on the messages cells send each other, quieting the inflammatory chatter that wears a joint down over time.
There’s a catch most patients never hear. Not all PRP is equal. Platelet concentration, whether the prep is leukocyte-rich or leukocyte-poor, and how it’s spun all change what ends up in the syringe. A 2025 meta-analysis in the American Journal of Sports Medicine found that higher platelet doses tracked with better pain and function scores. Two clinics can both call it PRP and hand you very different products.
For mild to moderate knee arthritis, yes, PRP helps a meaningful share of patients. Mayo Clinic specialists report a 60 to 70 percent success rate, meaning at least half the usual pain and function relief, lasting 6 to 12 months. For bone-on-bone, end-stage knees, the results fall off hard.
The evidence has firmed up over the last two years. In 2024, an ESSKA-ICRS expert panel said PRP is appropriate for patients up to age 80 with early to moderate arthritis who’ve already tried conservative care and gotten nowhere. In April 2026, the American Academy of Physical Medicine and Rehabilitation went further, suggesting doctors consider PRP earlier rather than waiting until a knee is destroyed.
Head to head, PRP tends to beat hyaluronic acid gel shots and steroid injections at the 6 to 12 month mark for pain and function. Steroids can calm a flare fast, but the relief fades and repeated steroids may thin cartilage. PRP plays a longer game.
One number frustrates me. A 2025 study found only about 6.5 percent of knee arthritis patients were ever offered PRP, while steroids and gel shots get handed out constantly. Worth adding that the FDA has cleared the machines used to prepare PRP but hasn’t approved PRP itself for knee arthritis, so it’s offered off-label. Legal and routine for biologics, but it’s why patient selection matters so much.
No, not reliably. This is where the industry oversells hard. PRP can improve symptoms and, in some studies, reduce swelling deep in the bone. But imaging rarely shows real cartilage regrowth.
The framing most patients walk in with gets it backward. PRP isn’t a cartilage factory. It’s a way to quiet a joint down and buy back function. Sell it as regrowth and you’re setting a patient up to feel cheated.
PRP is low risk because it’s made from your own blood, so allergic reactions are rare. The real risks are minor and short-lived. A sore, swollen, stiff knee for a few days, with a small chance of infection or a reaction to the needle itself.
Studies put side effects a bit higher than a placebo shot, around 18.7 percent versus 9.1 percent, but on par with steroid or gel injections. Most of that is temporary soreness. Serious problems show up in well under 1 percent of cases. Mayo’s team reported over 1,100 PRP patients with no serious adverse events.
The bigger risk isn’t medical. It’s wasting time on the wrong product from the wrong provider. Ordering unregulated research-grade molecules or chasing the cheapest injection you can find is where people get burned. Research-grade material is not made for human use, and it’s illegal to prescribe, dispense, or inject. Formation won’t touch it.
A PRP knee injection is a quick, in-office visit, usually under an hour. Blood draw, spin, inject. No general anesthesia, no hospital.
First comes a small blood draw from your arm, like routine bloodwork. Next, the sample spins in a centrifuge for several minutes to separate and concentrate the platelets. Last, the knee gets numbed and the plasma goes into the joint, often with ultrasound so the needle lands exactly where it should.

Dr. Gopal Grandhige, a board-certified general surgeon who owns and runs the clinic, oversees the medical side. You’re not getting a rushed shot from someone who does them between other things.
Quality control is the other half. Formation uses only human-grade compounds from licensed pharmacies for its regenerative work and holds SSRP and ISSCA certifications, the bodies that vet peptide, exosome, and stem cell protocols. That’s the difference between a supervised medical treatment and a gamble.
PRP isn’t instant. Most people feel a bit worse for a few days, then start noticing relief around 2 to 6 weeks in, with peak benefit by about 3 months. Results tend to last 6 to 12 months, sometimes longer.
Recovery itself is easy. Ice the knee, rest it for a day or two, and skip anti-inflammatory painkillers like ibuprofen for a couple of weeks. Those drugs blunt the exact inflammation PRP is trying to use. Most people walk out and drive home, though a stiff knee that first day is normal.

Think of your joint health like a seesaw. On one side sits building, the strength work and protein that tell tissue to get stronger. On the other sits renewal, the rest and sleep your body uses to clean up and recover. Health lives in moving between the two, not camping on one side. PRP doesn’t replace that rhythm. It makes the effort you’re already putting in go further.
This is the part I push hardest on. The body compensates through many pathways. Push on inflammation alone and it finds another route to keep the joint irritated. So a real knee plan hits several angles at once. Quad and glute strengthening to unload the joint. Weight management, since every extra pound multiplies force through the knee. Sleep and nutrition to support repair. Some patients also fold in peptide therapy for tissue support, always under supervision.
If you’re weighing this, the smart move is an honest assessment, not a sales pitch. Push for a straight answer on whether your knee is the kind that responds before you commit to anything.
A PRP injection for knee arthritis isn’t magic, and anyone promising a rebuilt knee is selling you something. What it is, in the right knee and the right hands, is a low-risk way to cut pain and stretch out the time before you’d ever think about surgery. Get it done where the product is controlled, the provider knows joints, and the plan includes the work only you can do.
Not specifically. The FDA has cleared the devices used to prepare PRP, but it hasn't approved PRP itself as a treatment for knee arthritis, so clinics offer it off-label. That's legal and common for biologic treatments, and it's part of why patient selection carries so much weight.
It depends on your knee, but a series of three injections spaced a few weeks apart tends to outperform a single shot for arthritis. Some people get lasting relief from one. Your provider should base the number on how you respond, not a preset package.
Not well. PRP works best for mild to moderate arthritis, roughly grades 0 through 3. For end-stage, bone-on-bone knees at grade 4, the evidence is weak, and a joint replacement conversation usually makes more sense. A 2024 ESSKA panel drew the same line.
Most studies report relief lasting 6 to 12 months, and some patients get longer. There's no firm consensus, and results depend on the arthritis severity and the quality of the PRP. Repeat injections can extend the benefit over time.
For pain and function at the 6 to 12 month mark, PRP often edges out both hyaluronic acid gel shots and cortisone. Cortisone acts faster on a flare but fades sooner, and repeated cortisone may harm cartilage. PRP trades speed for durability.
Take it easy for the first day or two, then ease back in. Skip anti-inflammatory painkillers for about two weeks, since they can blunt the effect. Follow the rehab plan your provider gives you, because the strengthening work is what makes the injection pay off.
Usually because of low platelet concentration, poor patient selection, or skipping rehab. A weak prep, a bone-on-bone knee, or treating the shot as a standalone fix all lead to disappointment. Higher platelet doses and the right candidate change the odds.
