Yes. Plastic surgery removes eye bags, but only the kind caused by fat that has pushed forward under the eye. That answer is narrower than most people expect, and the gap is where things go wrong.
Plenty of the people who come in convinced they need eyelid surgery are looking at something else in the mirror. Fluid. Pigment. A hollow that reads as a shadow in every photo. Cutting fat out of those eyes makes them look older, not younger. So before anyone talks about incisions, we figure out what the bag actually is.
This covers what lower eyelid surgery can and can’t do, how to tell which problem you have, and when a nonsurgical option is the smarter call. It doesn’t cover upper eyelid work, brow position, or crow’s feet. Those are separate problems with separate fixes.
Lower eyelid surgery removes eye bags permanently when fat is the cause. Fat that’s taken out or repositioned doesn’t come back.
Eye bag surgery, also called lower blepharoplasty, removes or repositions the fat pads sitting under the eye and trims loose lower lid skin. It corrects bulges made of fat. It doesn’t treat swelling from fluid, darkness from pigment, or hollowness in the tear trough, which are three different problems that look similar in a bathroom mirror.
The procedure is steady, not trendy. Surgeons reported 120,755 eyelid surgeries in 2024 in the ASPS procedural statistics report, essentially flat against 120,747 the year before. That count covers procedures performed by ASPS member surgeons only, so the true national number runs higher.
Flat demand tells you something useful. Eyelid surgery isn’t riding a social media wave. It holds its place because the anatomy it corrects doesn’t respond to anything else.
“Bags” is one word covering at least four separate conditions. Each has its own fix. Picking the wrong one is the most common reason people end up unhappy.
Orbital fat cushions your eye and sits behind a thin wall of tissue. As that wall weakens with age, fat presses forward into the lower lid.
The tell is consistency. The bulge is there in the morning, there at night, and still there after a week of eight-hour sleep. It’s structural, and it’s the one surgery fixes.
If the puffiness is worst when you wake up and better by lunch, you’re dealing with fluid. Salt, alcohol, allergies, and sleeping flat all push fluid into that tissue overnight.
Mayo Clinic recommends a cool compress, sleeping with your head slightly raised, and limiting salt and fluids before bed. None of that is exciting. All of it works better than surgery, because surgery does nothing for fluid.
Press gently under the lid with a clean finger. If the area feels like it’s missing volume rather than carrying extra, you’re looking at a hollow. The shadow it casts photographs like a bulge.
Removing fat from a hollow eye deepens the hollow. That mistake is common and hard to undo.
Some under-eye darkness is melanin, and some is blood vessels showing through skin thinner than anywhere else on the body. No amount of surgery lightens either one.
Lower eyelid surgery works through one of two incisions, and the choice comes down to whether skin needs to come off.
When the problem is fat alone, the incision goes inside the lower lid and nothing shows on the outside. When loose skin also needs trimming, the incision sits just under the lash line. Surgeons can also reposition tissue rather than remove it, moving fat down into the hollow below instead of discarding it.
That second approach has become the preference for a reason. Fat removal is permanent, and taking too much ages an eye rather than refreshing it.

I’ve used the same anesthesia team on every one of my cases for years, which matters more than most patients realize.
Lower blepharoplasty with fat removal doesn’t eliminate dark circles, crow’s feet, or fine lines. Cleveland Clinic states that plainly, and it’s worth reading twice if a consultation left you with a different impression.
Picture the patient who has both a fat bulge and pigment. Surgery takes the bulge. The darkness stays. That isn’t a bad surgical result. It’s an incomplete plan.
Crow’s feet come from muscle movement, which is why Botox under the eyes and around them addresses lines that no incision reaches. Different problem. Different tool.
Most under-eye complaints have more than one cause. Patients rarely expect that, and it changes how a plan gets built.
Sequencing matters. I’d rather correct structure first and see what’s actually left once swelling resolves than stack three treatments at once and lose track of what did the work. So hyaluronic acid filler and neuromodulators come after the shape settles, not before.
Some patients need one thing. Some need three. The plan follows the anatomy rather than a package.
Skip surgery when the tissue still has recoil and the bulge is mild.
Radiofrequency tightening reaches the deepest layer of the skin and the fibroseptal network underneath it, which is where laxity actually lives. Lasers and microneedling work from the top down and can’t get to that foundation. That makes FaceTite under the eyes a fit for the patient with crepey lower lid skin and no real fat bulge. It doesn’t remove fat, and nobody should tell you otherwise.
The line falls at recoil. Skin with recoil responds to energy. Skin that hangs at rest doesn’t, because energy can’t shrink a fold that’s already loose. Past that point, tissue has to come off.
Some lower lids need volume added, not taken away.
When the lid is hollow and the cheek beneath it has lost support, the eye sits in a shadow that fat removal only deepens. This is where fat transfer under the eyes comes in, moving your own tissue into the deficit to rebuild the shelf the eye rests on.
Your own fat behaves differently from filler. Some of it doesn’t survive the move, which is why a second session is occasionally part of the plan rather than a sign something failed. Filler is reversible and predictable. Transferred fat lasts longer and asks for more downtime.
A good candidate has a bulge that doesn’t change through the day, healthy eyes, and expectations set at refreshed rather than transformed.
Eye disease changes the math. Glaucoma, dry eye, thyroid conditions, and any history of retinal problems belong on the table before anyone discusses incisions. So does a bleeding history.
Part of my job is telling people no.
Some patients leave with a plan that starts with allergy treatment and a different pillow. Others need volume added, not fat taken away. When someone genuinely needs a surgical eye lift, we book it. When they don’t, saying so early saves them a procedure they’d spend years regretting.
Health and diet get assessed before treatment. Nutrition, supplements, and peptides are adjusted in the window right before and right after, so the body builds as much structure as it can while it’s healing. The instrument is identical at every practice in Tampa. The thinking around it is not.

Swelling and bruising show up in both lids. Cold compresses help. Stitches in the upper lid come out around a week, and lower lid incisions placed inside the lid don’t need any. Dry eye is common afterward and usually clears inside two weeks. Longer than that warrants a phone call.
Real complications are rare, and rare isn’t zero. Bleeding, infection, scarring, an eyelid that folds the wrong direction, trouble closing the eye completely, and vision loss all sit on the published risk list. Any surgeon who skips that conversation is selling rather than planning.
Results hold up. Lower eyelid surgery rarely needs repeating. Aging continues around it, which is a different statement from the result failing. That reading of tissue comes out of my surgical background in general surgery, burn surgery, and critical care.

Fillers work for hollowness, not for bags. Hyaluronic acid placed in the tear trough restores volume where volume is missing, which softens shadows. Adding filler to an eye that already has a fat bulge tends to make the area look heavier.
No. Blepharoplasty with fat removal does not eliminate dark circles, crow's feet, or fine lines. Darkness from melanin or from visible blood vessels needs a different approach, which is why a patient with both a bulge and pigment usually needs a combined plan.
There is no fixed age. What matters is whether the bulge is fat rather than fluid, whether the eyes are healthy, and whether expectations are realistic. Most patients who pursue it are in their forties or older, because that is when the tissue wall holding orbital fat back typically weakens.
