PDO thread puckering is the small dimpling or bunching of skin that shows up after a thread lift, caused by the threads gathering tissue as they pull it into a lifted position. It looks like tiny folds, creases, or indentations along the treated area, often near the entry points. In most cases it is mild and fades on its own within days.
I have watched patients catch sight of a pucker in the car mirror an hour after treatment and start to spiral. Take a breath. A little gathering is part of how the lift works, not a sign something went wrong. The threads have to grab tissue to hold it up, and grabbed tissue bunches before it relaxes.
What bothers me is how often clinics gloss over this. The glossy marketing shows a flawless jawline at the one-month mark and skips the part where you might look slightly crinkled for a week. So this is what you will see, why it happens, and where normal settling ends and a real problem begins.
Right after a thread lift, puckering usually appears as small puckers or dimples that follow the direction of the lift. The skin can look gathered, like the top of a drawstring bag, with little ridges that catch the light when you move.
These are dissolvable sutures sitting under the skin, not a cut or a wound. Cleveland Clinic describes a thread lift as a non-surgical procedure that makes subtle changes with minimal downtime, so the early gathered look comes from tension, not trauma.
At rest the dimpling might be barely there. Smile, chew, or turn your head and it gets more obvious, because the muscle movement tugs on the same tissue the threads are holding.
Color matters too. Mild redness, slight swelling, and a few pinpoint bruises near the insertion sites are normal company for early puckering. The dimples themselves should look like surface texture, not deep pits, and they should not hurt beyond mild tenderness.
Two patients can get the same treatment and pucker differently. Thin skin shows gathering more. Heavier tissue hides it. A conservative lift puckers less than an aggressive one. None of that means one result is bad. It means the skin is doing what skin does under tension.
Puckering clusters where the lift does the most work and where your face moves the most. The cheeks and midface gather often because that is where threads anchor a sagging area. The jawline and the corners of the mouth come next, since they shift every time you talk or smile.
The lower face and jowls are the classic trouble zone. That skin is mobile and frequently asked to hold the biggest lift, so it bunches more readily. If a heavy jawline is your real concern, threads may not be the strongest tool, and ways to reshape your jawline without aggressive threading are worth weighing first.
The neck and under-eye area pucker less but show it more, because the skin there is thin. A pucker that would hide on a cheek can look obvious under the eye, which is why careful placement matters most in those spots.
Puckering happens because barbed threads work by physically gathering and holding tissue, and gathered tissue folds. The barbs along each thread act like tiny anchors. When your provider pulls the thread to lift, those anchors bunch the skin slightly, the same way pulling a thread through fabric ruckles the cloth.

A real problem looks and behaves differently. Persistent, sharp dimples that do not budge after a couple of weeks, growing asymmetry, threads you can see or feel as ridges under the skin, or pain that worsens instead of fading. Those belong in the documented complication column, not the normal one. A 2026 meta-analysis of thread lift complications grouped dimpling and visible threads among the more commonly reported issues, and the pattern is almost always technique or aftercare, not the threads themselves.
Put plainly, temporary puckering is the cost of admission for a lift that gathers tissue. Puckering that sticks around is feedback that something in the placement or the healing needs attention.
Mild puckering typically settles within one to two weeks as swelling drops and the threads relax into place. Gentle, provider-approved massage can speed that along for surface dimples.
Most people are back to normal life within a few days, but the skin is still settling under the surface. If you are planning to fly after PDO threads, give the tissue a few quiet days first so cabin pressure and travel stress do not pile onto fresh swelling and make early puckering look worse than it is.
Because the threads dissolve and the body absorbs them over months, even a stubborn pucker is temporary rather than permanent. That is one of the real upsides of threads over a scalpel. A surgical mistake can be forever. A thread that gathered a little will not be.
The first fix is usually time plus light massage, not a return trip to the table. For most surface puckers, your provider will show you how to gently press and smooth the area, and the dimpling eases as days pass.
What you do at home matters more than people expect. Sleep on your back so you are not mashing fresh threads into a pillow. Skip hard chewing, big facial expressions, and strenuous workouts for the first week or two. Avoid facials, saunas, and anything that yanks on the skin.
When a pucker refuses to settle, that is a conversation, not a panic. A skilled provider can release a tethered thread, massage out a deeper dimple, or in stubborn cases remove and reposition. There is no prize for toughing it out. The same threads that are easy to adjust at week one get harder to manage once tissue heals around them. Early is easy. Reach out the moment something feels off, not after a month of hoping.
