PDO thread lifting is a minimally invasive cosmetic procedure that uses dissolvable polydioxanone sutures to lift mildly sagging skin and trigger new collagen growth. A provider inserts the fine threads under the skin with a thin needle. The threads dissolve over six to nine months, while the collagen they kick-start keeps firming the area for a year or longer.
It’s not surgery, and it’s not a facelift substitute. Most clinic pages skip the honest part. The lift you see on day one is partly swelling. The real change comes from collagen, and it’s quieter than the marketing photos suggest. I’ve watched patients walk in expecting facelift results and leave happy once they understand what threads actually do.
I’m not going to talk pricing here, because it swings so much by provider, thread count, and treatment area that any number would mislead you. What I will cover is what threads do, who they fit, the risks, and what current research says about how long the effect holds.
PDO threads are the oldest of the three suture materials used in thread lifts, and they lean toward firming and refreshing skin rather than heavy lifting. The other two, PLA and PCL, dissolve slower and pull a little harder.
PDO stands for polydioxanone, a colorless surgical polyester. Surgeons have used it for stitches since the 1980s, and the same material has long been cleared by the FDA for surgical use, which is part of why its safety record runs so long. It’s worth knowing that most thread lifts use these devices off-label for cosmetic lifting, which is common and legal, though it’s a fair thing to ask your provider about. Once a thread sits under the skin, it dissolves in roughly six months, and your body treats its presence as a small injury worth repairing. Cells called fibroblasts answer by laying down fresh collagen, the protein that keeps skin firm and springy. Falling collagen is a big reason skin starts to droop as you age.
Threads come in three builds, and the one you need depends on the job. Mono threads are smooth and don’t lift much; they sit in the skin and push collagen, so they handle texture and mild tightening. Cog threads have tiny barbs that grip tissue like soft fishhooks, and these do the real lifting, repositioning a sagging cheek or jowl. Screw threads are one or two strands twisted together, used to add a bit of volume back to hollow spots.
PDO dissolves fastest. PLA and PCL last longer and tend to spark more collagen.
PLA (polylactic acid) takes around twelve months to absorb. PCL (polycaprolactone) runs longer, roughly twelve to fifteen months. Because they linger, they keep prompting collagen for a longer stretch and hold a firmer lift. A Japanese review of more than 106,000 cases found PDO showed up most in patients in their twenties and thirties, while the slower threads were used more often in older patients. So PDO is a fine entry point for early laxity, but if your skin needs more pull, ask whether a longer-lasting thread fits better. For sagging well past that point, you’re looking at facial surgery, not threads.
PDO thread lifting carries far less risk than a surgical facelift, and serious problems are rare in trained hands. Most side effects are minor and short-lived.
In a 2021 meta-analysis of facial thread-lifting published in Aesthetic Plastic Surgery, swelling was the most common issue at about 35%, followed by skin dimpling near 10%, paresthesia (a pins-and-needles feeling) around 6%, visible threads at roughly 4%, and infection close to 2%. That same research on thread-lift complications found dissolvable threads like PDO were safer than permanent ones, with paresthesia dropping from about 12% to 3% and thread extrusion from roughly 8% to under 2%.
Age matters too. Patients over 50 had a higher dimpling rate, near 16% versus under 6% in younger patients, plus more infections. That doesn’t make threads dangerous. It means provider skill and patient selection do most of the work. The worst outcomes I’ve seen, asymmetry or a dimpled cheek, almost always trace back to an undertrained injector working too fast, not to the threads themselves. Minor bruising, mild soreness, and the rare snapped thread round out the list.
PDO threads work best on the lower face and neck, where mild sagging tends to show up first. Cheeks, jawline, jowls, the neck, and the area around the brows are the usual targets.
They’re a poor match for heavy, advanced sagging. Threads shine in the in-between stage, when you’re past what good skincare can hold but not yet ready for a knife. The neck is a common request, and threads can sharpen a soft jaw-to-neck line, though deep banding there usually needs more. Because the lift is modest, we often pair threads with other treatments in the same plan. A round of dermal fillers can put lost volume back into the cheeks, which gives the lifted skin something to sit on.
The visible lift from PDO thread lifting usually holds six to twelve months, and the collagen benefit can stretch a year or more. Anyone promising two-plus years is selling you the best case.
A lot of older advice oversells this, and you’ll still see pages claiming two years or longer. A 2019 study found skin sagging improved right after the procedure, dipped noticeably by six months, and was hard to spot at the one-year mark. Patient-reported data tells a similar story: high satisfaction early, then a slide as the threads dissolve. One meta-analysis pegged short-term satisfaction near 98%, falling to about 88% over the long term.
Younger skin with more elasticity tends to hold results longer, sometimes three to four years with touch-ups. Older skin may only get a year. To keep the effect going, many patients book a maintenance session or add radiofrequency skin tightening between thread treatments.
A PDO thread lift takes under 45 minutes in the office, with no general anesthesia and no scalpel.
For about five days before, you’ll likely be asked to skip alcohol, tobacco, and anything that thins the blood or raises bruising, such as ibuprofen, aspirin, fish oil, and green tea extract. On the day, your provider walks you through the risks and your recovery before starting. The procedure itself usually runs like this:

Recovery is light. Most people return to normal life the same day, with some swelling and bruising for the first day or two.
For the first week, don’t rub or massage your face, since pressure can shift a thread before it settles. Hold off on pursing your lips hard, smoking, and drinking through a straw for a few weeks. For one to two weeks, skip sleeping on your side, hard workouts, and saunas. Prop your head on a pillow so you don’t roll onto your face overnight. Small habits, but they protect the result.
The best candidate has mild to moderate sagging, decent skin quality, and realistic expectations. If your skin is very loose or very thin, threads tend to disappoint.
I turn people away from threads more often than you’d guess. Heavy jowls, deep neck banding, and advanced laxity all need surgery, and threads stacked on top of them waste money and goodwill. Thin skin can let the threads show. Heavy smokers and slow healers raise the odds of trouble.
The single biggest factor in your result isn’t the thread brand. It’s the hands placing it. Ask your provider how many of these they’ve done, ask to see before-and-afters on someone your age and bone structure, and ask whether the threads are cleared for the exact use you’re getting. A board-certified surgeon working in an accredited setting gives you the best odds, plus a real plan if something needs fixing later. If a place quotes you a price over the phone before anyone has looked at your face, treat that as a warning sign.
People ask whether thread lifts are “worth it.” That’s the wrong question, though. The better one is whether your sagging is mild enough for threads to do real work. On RealSelf, the procedure carries roughly a 75% “Worth It” rating, which tells you it satisfies most people matched to it well and frustrates the ones who aren’t.
Demand for low-downtime options keeps climbing. The 2024 ASPS report counted more than 28 million minimally invasive procedures, up year over year, as patients chase results without the recovery of surgery. PDO thread lifting fits that mood, as long as you go in clear-eyed about what threads can and can’t do. If your laxity is mild, it buys you a natural refresh and time before you ever consider a facelift. If your sagging is past that point, a frank surgeon will tell you so. Match the procedure to your real anatomy, pick skilled hands, and it’s a smart, modest step. Oversold and misapplied, it’s just an expensive few months of swelling.

Most patients feel pressure and pinching rather than sharp pain, since the area is numbed with local anesthetic first. Mild soreness, swelling, and bruising are common for one to two weeks. Over-the-counter pain relief usually covers it.
PDO thread lifting is considered safe when a trained provider performs it, and serious complications are rare. People with heavy, advanced sagging, very thin skin, or unrealistic expectations are usually poor candidates. Threads cannot replace a facelift, and placing them over severe laxity tends to waste money. Heavy smokers and slow healers also face higher complication rates.

Rapid weight loss from GLP-1 medications can leave facial skin loose, and the 2024 ASPS report found 41% of GLP-1 patients were considering a nonsurgical procedure. Threads can address mild laxity, though significant sagging may still need surgery.
