Neck liposuction risks include bleeding, bruising, infection, uneven results, numbness, swelling, and (rarely) serious issues like blood clots, anesthesia reactions, or skin necrosis. Most patients walk away with nothing worse than a week of puffiness and some color changes under the chin. But the procedure isn’t risk-free, and pretending otherwise sets people up for a recovery they didn’t sign up for. I’ve watched hundreds of patients go through this. The ones who do well are usually the ones who came in with realistic expectations of what double chin surgery can actually deliver.
A NIH-indexed submental complications review puts the overall complication rate for this procedure between 0.3% and 6%. For the broader liposuction category, a meta-analysis of more than 29,000 patients pegged total complications at roughly 2.62%, with contour irregularities leading the list at 2.35%. Those numbers are low. They’re not zero. And the size of the gap between “low risk” and “your specific risk” comes down almost entirely to who’s holding the cannula and how healthy you are walking in.
Neck liposuction risks fall into two groups. Common side effects like bruising, swelling, numbness, and minor asymmetry resolve within weeks. Rare but serious complications include blood clots, infection, skin necrosis, fat embolism, and damage to nearby structures. Submental-specific complication rates run between 0.3% and 6%, depending on technique and surgeon experience.
Yes, when a board-certified surgeon performs it on a healthy candidate. The procedure has been around for decades, and the techniques used in 2026 are gentler than what was standard ten years ago. In 2024, ASPS data showed 24,000 submental and chin liposuction procedures, up 1% from the year before. The full liposuction category sat at 349,728 procedures. That’s a lot of safety data.
But “safe in general” doesn’t mean “safe for you.” Risk climbs with combined procedures, with patients who have diabetes or clotting disorders, and with providers who don’t actually train in facial anatomy. I’ve seen patients show up for revision work who picked their original surgeon based on Instagram before-and-afters. That’s not a strategy. That’s a coin flip.
Almost every patient experiences at least one of the issues below. The real question isn’t whether you’ll have side effects. It’s whether they’ll resolve it cleanly.
Light bleeding happens during the procedure itself, and bruising shows up within 24 to 48 hours afterward. Most of it clears by day 10 to 14, fading from purple to yellow to gone. Excessive bleeding is rare but possible if you take blood thinners, supplements like fish oil or vitamin E, or if you ignore pre-op instructions. Compression garments help more than people give them credit for.
Infections happen in well under 1% of cases when the incisions are kept clean. Watch for redness that spreads, warmth, fever, or pus from the entry sites. Antibiotics handle most cases caught early. Spotting lipo infection warning signs within the first ten days is what separates a quick fix from a hospital trip.
Sometimes one side looks slightly different from the other. Sometimes it’s swelling that resolves on its own. Sometimes it’s uneven fat removal that needs a small revision touch-up at six months. Anatomy isn’t symmetric to begin with. Most “asymmetry” patients complain about in the first month it is just lopsided swelling.
Swelling peaks at days 3 to 5, then drops steadily over the next two weeks. If a pocket of fluid (a seroma) builds up under the chin and doesn’t reabsorb, your surgeon drains it with a needle. Mayo Clinic data lists contour irregularities and fluid pockets among the most commonly reported post-op issues. Wearing your compression garment as directed makes seromas significantly less likely.
Almost everyone feels numb under the chin for a few weeks after surgery. Small sensory nerves get irritated during the procedure, and they need time to wake back up. Most sensation returns within 6 to 12 weeks. A small percentage of patients keep mild numbness in a patch for several months. True permanent numbness from neck lipo is rare.
Rippling, dimpling, or uneven texture happens when too much fat is removed, when skin elasticity is poor, or when the patient is older with thinner tissue. This is the risk that gets oversold by lipo-only providers. If your skin doesn’t have the snap to retract over the new contour, no amount of fat removal will give you a sharp jawline. You’ll need PDO threads to tighten skin or an energy-based device to finish the job.
Bruising typically resolves in 7 to 14 days. The color shifts from dark purple or blue in the first few days, to green and yellow, then fades out. Bruising lasting past three weeks isn’t normal and should be flagged to your surgeon, especially if it comes with pain that’s getting worse instead of better.

That last one trips up more patients than anything else. People forget that bending to pick up a laundry basket counts as straining.
Probably not. The incisions are tiny, usually 2 to 3 millimeters, placed under the chin and sometimes behind the earlobes. After six months, most patients can’t find them without a mirror and good lighting. Scars fade most dramatically in the first three months but keep maturing for up to a year. The exact placement matters. Knowing where the incisions are made for your procedure is one of the questions you should ask at consultation. Cleveland Clinic guidance on this category of surgery confirms that incision care during the first weeks is the strongest predictor of how visible scarring will be long-term.

These are the ones that send patients to the ER. They’re rare. They’re also the reason why surgeon credentialing matters more than price or location.
Deep vein thrombosis is uncommon in neck-only liposuction because the procedure is short and patients aren’t immobile for long. The risk climbs when neck lipo is combined with a longer body procedure. Walking the day of surgery cuts clot risk substantially.
Most neck liposuction in 2026 is done under local anesthesia with light sedation, which carries lower risk than general anesthesia. Reactions are rare. A solid pre-op health screen catches the patients who shouldn’t have certain medications.
Skin necrosis (tissue death) is exceedingly rare in neck lipo and almost always tied to smoking, severe vascular disease, or overly aggressive technique. If you smoke and your surgeon doesn’t ask you to stop for four to six weeks before surgery, find a different surgeon.
A fat embolism happens when fat enters the bloodstream. It’s a documented risk in any liposuction procedure but vanishingly rare in neck-only cases because the volume of fat removed is small.
The neck holds the marginal mandibular nerve, blood vessels, and the platysma muscle. Damage is rare and tied directly to surgeon experience. This is why you don’t get neck lipo from a non-facial-trained provider.
Most risk doesn’t come from the procedure itself. It comes from patient and provider variables that are largely in your control.
Risk stacks. Adding a neck lift, jawline filler, or advanced liposuction in another body area on the same day increases anesthesia time, blood loss, and recovery demands. Combinations make sense in the right patient. They don’t make sense for everyone.
Diabetes, autoimmune disease, smoking, blood thinners, and uncontrolled blood pressure all raise the risk profile. Body mass index matters too. ASPS 2024 data shows the largest group of liposuction patients was 40 to 54 years old, with women still making up the majority. Health, not age, is the bigger factor.
Skipping follow-ups. Taking off the compression garment after three days. Hitting the gym at day five. Sleeping flat instead of elevated. Each one nudges risk up. Recovery isn’t optional.
The biggest single variable. A board-certified surgeon for liposuction with hundreds of neck cases under their belt isn’t the same as a med spa provider with a weekend training certificate. Verify ABMS certification. Ask how many neck cases they do per year. Ask about their revision rate. Most patients never ask. They should.
“Cobra neck” deformity. It happens when too much fat is taken from the central neck, leaving a depression that looks worse than the original double chin. It’s harder to fix than it is to prevent, and revisions can run for months. Surgeons consistently say they wish patients understood this: aggressive fat removal isn’t a better outcome. Conservative is the right word here. You can always take a tiny bit more later. You can’t put it back.
This is the contrarian point most lipo content misses. The industry keeps repeating that neck lipo is quick and easy with no downtime. Final results don’t show up until three to six months out, after all the swelling resolves. Anyone selling you a quick fix is selling you the wrong expectation.

Most swelling resolves within two to three weeks, with peak puffiness happening at days 3 to 5. Subtle residual swelling can stick around for up to three months, and final results take 3 to 6 months to fully reveal. If swelling worsens after week one or comes with sharp pain, contact your surgeon.
Yes, in most cases. Revisions for asymmetry or small contour issues are usually possible after 6 months of healing. "Cobra neck" deformities from over-resection are harder to correct and may need fat grafting plus skin tightening. Revision rates for experienced neck lipo surgeons typically run under 5%.
Often yes for residual fat, but the bigger question is skin laxity. Rapid weight loss from GLP-1 medications frequently leaves loose skin, which lipo alone won't fix. ASPS data shows rising demand for neck lift procedures alongside lipo for this exact reason.
The neck is a low-volume area, usually under 100 milliliters of fat total. Conservative removal is the right call because over-resection causes more deformities than under-resection. Good surgeons remove enough to define the jawline and stop short of creating a depression.
Watch for sudden chest pain or shortness of breath (possible blood clot), spreading redness with fever (infection), darkening skin or slow healing (possible necrosis), or sudden severe asymmetry. These are rare but need immediate medical attention. Routine bruising and swelling that follow expected timelines are not emergencies.
In some states, yes, but the risk profile climbs sharply. Med spa providers without surgical training are more likely to cause contour deformities, undertreat or overtreat, and miss complications early. Submental anatomy is unforgiving. Verify ABMS board certification before booking.
Only modestly. Lipo removes fat but doesn't tighten skin meaningfully. Patients with good skin elasticity see clean results. Patients with skin laxity from age or weight loss typically need lipo combined with a skin-tightening procedure, or they end up disappointed with the outcome.
