The jaw and neck blur for four reasons, and most people have more than one going on at once. Knowing which one is yours decides the treatment.
Bone and fat shrink. Your face loses volume from the inside, so the skin that used to drape over a firm structure starts to fold. Collagen and elastin drop off too, so skin gets looser and springs back less. Fat can also collect under the chin, which softens the line between jaw and neck into one round curve. And the platysma, a flat sheet of muscle across the front of the neck, can tug downward and form vertical cords.
There’s a newer driver showing up in my consults: fast weight loss. Patients dropping size quickly on weight-loss medications often deflate in the face and neck before anywhere else, and the skin doesn’t shrink at the same speed. Surgeons across the country are reporting the same pattern. The treatment you need depends entirely on which of these is the main culprit, so guessing is expensive.
Five approaches cover almost every non-surgical case I see. Here’s the quick version, and how they differ:
The honest distinction is this: fillers and fat-dissolving shots change shape now. Collagen builders and skin tightening change quality slowly. Surgery still does the heaviest lifting. For context on scale, ASPS counted roughly 24,000 submental and chin liposuction cases in 2024, so plenty of people still pick the surgical route when the problem is big enough. Fillers are the fastest way to draw a cleaner jaw. Jeff places a thick gel along the mandible and chin, and the edge that aging blurred comes back the same day. This is the workhorse of jawline contouring, and it’s popular for a reason: more than three million Americans get some form of jawline filler and other dermal fillers each year, according to Cleveland Clinic.
Two main types do the job. Hyaluronic acid gels give a firm, liftable edge and dissolve if you ever want them gone. Calcium-based fillers run thicker and also nudge your body to make collagen, so they pull double duty. Hyaluronic acid in the jaw typically holds six to twelve months, sometimes up to two years depending on the product and your metabolism.
Here’s where I push back on the internet. Filler is not a fix for loose skin. If your real problem is sag, packing in syringes just makes a heavy face heavier. I’ve sat across from patients who spent a small fortune chasing definition with filler when the skin needed tightening instead. Get the diagnosis right first.
Yes, a fat-dissolving injection can shrink a pad of fat under the chin, but only that. The active ingredient is deoxycholic acid, a molecule your body already uses to break down dietary fat. Injected under the chin, it kills fat cells, and your body clears them over a few weeks. Gone cells don’t come back, so the result holds as long as your weight stays steady.
It usually takes a few sessions about a month apart, and the swelling after the first one is real. Plan for a puffy week.
Now the contrarian part, and it’s important. Dissolving fat does nothing for skin, and on the wrong patient it backfires. Take fat out from under loose skin and you can unmask a saggier neck than you started with. This is why I assess skin quality before anyone gets near a needle. If the skin won’t snap back, fat-dissolving alone is the wrong call, and neck liposuction or a tightening approach belongs in the conversation. The injectable fat-reduction category actually lost ground in 2024 by ASPS-tracked figures, and my read is simple: it got oversold for years and underdelivered on necks it was never going to fix.
If your neck shows two vertical cords when you tense it, that’s the platysma muscle, and a neuromodulator relaxes it. Soften that downward pull and the jawline reads sharper, the cords flatten, and the neck looks calmer.
This isn’t off-label guesswork anymore. In October 2024 the FDA approved the first neuromodulator specifically for moderate to severe platysma bands. In the trials behind that approval, roughly 65% and 62% of patients said they were satisfied with their neck and jaw at two weeks, against about 12% on placebo. That’s a real signal, not marketing.
A few units along the jaw and down the bands takes ten or fifteen minutes, and there’s basically no downtime. Results settle over a couple of weeks and last around three to four months. The catch: this works on muscle and pull, not on skin or fat. If your concern is a fatty fullness or loose drape, relaxing the muscle won’t touch it. Adding structure is a separate job, and whether jawline fillers work for your case is its own conversation.
Some injectables don’t add volume so much as wake up your own collagen. Calcium-based and poly-L-lactic acid products act as a scaffold under the skin, and over weeks to months your body lays down new collagen around them. The change is slow and subtle, which is the point for patients who don’t want to look suddenly different.
These suit thinning, crepey skin and early jowling better than a sharp structural fix does. You’ll usually need a short series, and the payoff arrives gradually, with results that can run a couple of years. I tell patients to think in seasons, not days.
This is also one of the better tools to lift jowls without surgery when the sagging is mild. Set the expectation honestly, though. A biostimulator firms and thickens skin. It will not haul up a heavy jowl that’s already collapsed past a certain point. When someone shows me that degree of laxity, I say so.
Non-surgical skin tightening works, within limits, and the limits are the whole story. Radiofrequency devices heat the layers under your skin to trigger collagen and shrink loose tissue. The effects can be modest and sometimes don’t last, and I’d rather you hear that from me than feel cheated later.
For the right candidate, the result is real. For someone with a lot of hanging skin, an external device alone won’t deliver, and chasing it is throwing money at the wrong problem.
There’s a middle option worth knowing. A minimally invasive radiofrequency treatment delivers the heat under the skin through a tiny entry point, which tightens more than a surface device but stops short of a lift. Patients ask us all the time whether FaceTite on the neck is right for them, and for moderate laxity it often bridges the gap between injectables and surgery. It’s the kind of call I’d rather make in person after feeling how the skin moves.
Single treatments fix single problems. Real faces rarely have just one. That’s why I plan most cases as a small stack rather than one hero procedure.
A common combination: dissolve a little fat under the chin, then add filler along the jaw to rebuild the edge the fat was blurring. Another: relax the neck bands with a neuromodulator while a biostimulator firms the skin over several months. Each piece covers a different layer, and the combined effect beats any one of them alone.
Post-weight-loss patients are the clearest example. When someone walks in with loose skin after weight loss on the neck plus lost volume in the face, no single injection solves both. We sequence treatments and, when the skin is too far gone, I’ll tell you surgery is the smarter spend. The goal is your result, not a longer invoice.
You’re likely a good candidate if your skin still has decent spring, your concerns are mild to moderate, and your weight is steady. People who do best want refinement, not a dramatic overhaul, and they’re fine with a few visits to get there.
You’re probably not a fit if you have heavy, hanging skin or a large amount of submental fat. At that point, injectables and devices become a slow, costly way to underwhelm yourself.
I keep this honest on purpose. The FDA points out that non-surgical results are often temporary and that not everyone responds, which lines up with what I see in clinic. During your consult I’ll feel the skin, pinch the fat, watch the muscle move, and tell you plainly whether non-surgical work will get you where you want to go or whether you’d be happier with a procedure. Nobody leaves my room sold on something that won’t work for them.
Recovery splits cleanly by treatment type, so here’s what to actually expect.
Fillers and neuromodulators are the easy ones. Maybe some redness, a little swelling, the odd bruise. Most people go back to work the same day. Filler looks good immediately, though small swelling can shift the final look for a few days.
This one earns real downtime. Expect noticeable swelling under the chin for several days, sometimes a week, especially after the first round. Numbness and tenderness are normal. Schedule it when you can hide out a bit.
A surface radiofrequency session is low-key, with mild redness that fades fast. The minimally invasive version asks for more: some swelling and bruising, and a short recovery before you’re camera-ready. People also ask whether a procedure will neck liposuction leave scars behind, and the answer depends on the technique and the hands doing it.
Longevity tracks with what the treatment does. Fat-dissolving results are effectively permanent, since dead fat cells don’t regrow, as long as your weight holds. Hyaluronic acid filler in the jaw runs six to twelve months and sometimes up to two years per Cleveland Clinic. Neuromodulators fade in three to four months. Collagen builders can last a couple of years because the improvement is your own tissue.
So there’s a trade. The fast, structural treatments need touch-ups more often. The slow, collagen-driven ones cost you patience up front but hold longer.
One question I get constantly is whether jawline filler migrates out of place over time. With sound product choice and careful placement it’s uncommon, but it’s a fair thing to ask before anyone injects you. A good injector plans for it.
This is the part people undervalue, and it’s the part that protects you. Two faces with the same complaint can need opposite treatments, and only someone who reads anatomy well will catch that.

Now the warning. Plenty of places will hand you the cheapest syringe and send you out the door. That’s how necks get worse, not better. Credentials and judgment aren’t interchangeable, and the difference between a cosmetic surgeon vs plastic surgeon is worth understanding before you sit in anyone’s chair. Ask who’s actually holding the needle and what their training is. If the answer is vague, leave.
Sometimes the right answer is a scalpel, and I’ll say so even though you came in asking about injections. If you have significant hanging skin, deep jowls, or a large fat pad, a neck lift, liposuction, or a combination will outperform anything non-surgical, and it’ll cost you less frustration over time.
Surgery asks for real recovery and a bigger commitment, no question. But for advanced aging it delivers a change that injectables physically cannot match. The volume hasn’t dropped for a reason; ASPS still counted about 22,445 neck lifts in 2024.
Some patients start non-surgical and move to surgery years later. Others go straight to the procedure because it’s the honest fit. Either path is fine. What’s not fine is spending a year and a pile of money on treatments that were never going to reach your goal. That’s the conversation I’d rather have on day one.
