Yes. Melanotan can darken birthmarks, and it changes how scars look too. It doesn’t tan one patch at a time. It signals the pigment cells across your whole body to make more melanin, so anything that already holds pigment, including most birthmarks and moles, tends to get darker. Scars are the wild card. Some blend into the tan, others turn blotchy, and pale ones may not change at all.
Melanotan is a lab-made peptide that copies a hormone your body uses to make skin pigment. Sold online as Melanotan I or Melanotan II, it’s marketed for tanning. Melanotan II has no FDA approval for any use, and most versions sold to consumers are research-grade products labeled “not for human use.”
Patients ask me about this after seeing tanning nasal sprays and “tan jabs” on TikTok. The pitch sounds harmless. The pigment problem it creates is the part nobody films. In our clinic, peptides multiply your effort. They don’t replace judgment.
Melanotan copies alpha-MSH, the natural signal that tells your melanocytes to make more brown-black pigment. That’s the whole mechanism, and it isn’t selective. It turns pigment cells up wherever they live, not just where you’d like a tan.
That’s why birthmarks and scars come into play. Both can hold melanocytes, and when the drug turns those cells up, existing pigment deepens and new spots can appear. Melanotan II was among the peptides reclassified in 2026, though that shift changed its paperwork, not its risk profile.
Yes, pigmented birthmarks usually darken with Melanotan. Freckles, café-au-lait spots, and pigmented moles all hold melanin, so they answer the same signal your tan does. Reports on the drug describe existing moles growing larger and darker alongside brand-new moles.
People treat birthmarks and moles as one thing. That’s not quite right, and the difference matters. Most “birthmarks” patients ask me about are pigmented, and those behave like moles here. Vascular birthmarks, like port-wine stains and hemangiomas, come from blood vessels instead of pigment cells, so Melanotan doesn’t darken them the same way.
Sometimes. Skin pigment often fades toward baseline within a few weeks to a month after the last dose. New moles and freckles are the ones that can stick around, which is a problem I’ll come back to.
Scars react unevenly, and that’s exactly the trouble. How a scar responds depends on its age and your skin type.
Newer scars and darker skin tones lean toward hyperpigmentation, so a scar can turn darker than the skin around it. Older, pale scars are the opposite story. Users have reported that a mature, white surgical scar, like a tummy tuck line that’s lost its melanin, stayed pale while the normal skin around it browned. You end up with a patchy contrast you never wanted. That’s the reverse of medically supervised peptides that support healing, where the whole point is even, monitored tissue repair.
This is where a cosmetic annoyance turns medical: darkening your moles hides the warning signs of skin cancer.
A changing mole is one of the strongest signals of melanoma, with a history of a changing mole tied to roughly a 400% higher relative risk in medical reviews. Now picture Melanotan darkening 30 or 40 moles at once, which is a normal number to carry. Your dermatologist is hunting for the one that changed for a bad reason among 40 that all just changed for a peptide. Published work on changes in pigmented lesions after Melanotan use is a big reason skin doctors push back on it.
The side effects aren’t only skin-deep either. Case reports and regulators link Melanotan II to nausea, kidney trouble, a painful prolonged erection, and even brain swelling. This isn’t a targeted tanning aid. It’s a systemic drug bought from strangers.
Stop and get it examined. A spot that shifts in color, size, shape, or border deserves a professional skin check, not a wait-and-see.
If you’ve used a tanning peptide, say so, because that context changes how a clinician reads your skin. Baseline photos and a full-body exam help. And if you’re weighing any peptide with a provider, ask the questions people skip: What baseline mole mapping will you do? How will this interact with future laser or aesthetic treatments? What’s the plan if new atypical moles show up? A program run with real medical oversight has answers ready. A vendor shipping vials does not. Self-injecting from unregulated sources also risks infection and abscesses, which can leave scars of their own.
Peptides aren’t good or bad. Insulin is a peptide. Insulin isn’t a trend. It’s a molecule your body makes that saves lives used correctly and harms people used wrong. Melanotan sits at the misuse end: a pigment signal aimed at looks, bought online, injected without oversight.
Social media turned peptides into a DIY purchase, pushed by influencer sales funnels and gray-market suppliers. That’s a trend cycle, not a care model. A vial is not a strategy. Copying a creator’s protocol isn’t clinical care. Regulators have called these tanning products far from safe, and they warn that no tan, real or fake, protects your skin from UV damage. One analysis of tanning kits even turned up products with over 100 unidentified ingredients in the vial. You have no idea what you’re injecting.
And peptides only fine-tune. They don’t do the work for you. Your sleep, protein, and training carry the load, and the right peptides optimize the effort you put in. The body also compensates through many pathways, so pushing one lever tends to trigger a counter-move elsewhere. That’s why real programs work several pathways at once instead of hammering a single one.

We don’t offer Melanotan for tanning, and we won’t. In April 2026 the FDA pulled Melanotan II from its significant safety concerns list, but that wasn’t approval, and it didn’t clear the drug for compounding. It stays unapproved for human use, it’s headed for a compounding advisory review by early 2027, and pharmacies still aren’t authorized to make it. Research-grade “not for human use” products aren’t legal to administer to people, full stop.
For contrast, the only Melanotan-family drug with real FDA approval is afamelanotide, cleared in October 2019 as Scenesse for a rare light-sensitivity disorder and given by a trained clinician. There’s no approved cosmetic tanning version anywhere.
So can Melanotan darken birthmarks or scars? Yes, and it darkens the moles you’d rely on to catch skin cancer along with them. The safer route to color is a topical self-tanner plus sun protection, not an unregulated needle.
