A 2024 systematic review in the Aesthetic Surgery Journal pooled 29,368 liposuction patients across 39 studies and put the infection rate at 0.020%. That’s two patients in ten thousand. Contour irregularity, at 2.35%, showed up more than a hundred times as often.
I’ve been operating for over 15 years. What keeps me careful with a submental case isn’t bacteria. It’s skin that won’t retract.
Infection after neck liposuction is a bacterial invasion of the small incision sites or the tissue space under the chin. It usually appears within the first ten days as spreading redness, warmth, fever, or drainage that has color to it. Rates in modern surgical series sit well under 1%. Sterile technique, an accredited facility, and close early follow-up are what keep that number low.
Infection is rare, and the figure most websites quote is close to useless.
You’ll see a range of 0.3% to 6% repeated everywhere for submental liposuction complications. That’s a twentyfold spread. It’s wide because those studies lump a bruise and a hospital admission into the same bucket across four decades of changing technique. Nobody should make a decision on a number that vague.
Better data exists:
The pattern is consistent. When infection does appear, it clusters around combined procedures and infusion-assisted technique. That 2025 analysis found infection at 0.54% with infusion-assisted methods, roughly 27 times the pooled rate. A neck done alone, through two or three incisions a few millimeters wide, sits at the safe end of that spectrum.
Real infection gets worse. Normal recovery gets better. That’s the whole distinction, and most patients can apply it themselves once someone explains it.
The neck liposuction procedure uses two or three incisions placed under the chin and behind the ears, each only a few millimeters across. Those are the sites to watch. Not the whole neck.
That last one matters more than people realize, and I’ll come back to it.
Most patients who call convinced they have an infection have a seroma or ordinary inflammation.
Swelling under the chin peaks around day three, then declines. It’s often lopsided, because tissue planes drain unevenly. A seroma is a pocket of clear fluid, so it feels soft and shifts when you press it. Infection feels firm, hot, and tender in a way that doesn’t shift. The 2024 meta-analysis put seroma at 0.65%, roughly 32 times more common than infection.
This confusion is why I have patients come in for lymphatic treatment starting at the first post-op visit rather than waiting. If a fluid collection forms, we drain it in the office. And I get eyes on the incisions early instead of relying on a phone description. The same logic applies to double chin liposuction, where the treated area is small and a few days of unchecked fluid changes the final contour.
Now the part that gets left out of every other article on this topic.
The infection worth understanding isn’t the one that shows up on day four. It’s the one that shows up on day 69.
In January 2024, the CDC published an investigation into 15 patients across nine states who developed Mycobacterium abscessus infections after cosmetic surgery at a single Florida clinic. Every one of them had liposuction. The median time from surgery to first symptom was 69 days, with a range of 33 to 119 days. Six of the 15 needed intravenous antibiotics. Treatment ran two to six months.
These are nontuberculous mycobacteria. They live in water and soil, they resist standard sterilization when protocols slip, and they don’t respond to the antibiotics a general practitioner reaches for first. The Florida investigation found gaps in environmental cleaning, protective equipment use, and surgical device disinfection at the practice involved.
Most risk articles say infection is nearly impossible with tiny incisions. That advice is outdated, and it’s the reason delayed cases get missed. If you develop a nodule six weeks after surgery and two rounds of antibiotics do nothing, that’s the conversation to have with your surgeon. Not another prescription.
It appears to, and this is where practice-level choices matter more than patient-level ones.
Historical tumescent series performed under pure local anesthesia have reported some of the lowest serious infection rates in the literature. The 9,002-case Dermatologic Surgery series is the cleanest example. Shorter operative time, no airway instrumentation, no general anesthesia recovery period.

I’m not claiming general anesthesia is dangerous. For a full abdominoplasty it’s the right call. For a neck, adding it buys you nothing.
Four things move the number, and only two are within your control.
Infection control is structural, not something you add on the day of surgery.
Every surgical case at Formation is performed in a Quad A accredited facility, which carries the most rigorous inspection standard for outpatient surgery. Florida tightened this further with HB 1561, effective May 2024, adding registration and financial responsibility requirements for offices performing higher-volume liposuction. Ask any surgeon you consult which accreditation their facility holds. If the answer takes more than five seconds, that’s information.
I use the same anesthesia provider and the same staff on every case. My physician assistant is employed by the practice and assists on all my surgeries. Familiar teams catch deviations that rotating teams don’t. The CDC’s Florida investigation traced back to a temporary location running the same staff and the same protocols as the permanent one, which tells you protocols and people travel together.
Surgical volume here is limited on purpose. Fewer cases means more attention per patient, and my background as a board-certified general surgeon with fellowship training in burn surgery and critical care shapes how I think about wound healing long before anyone reaches the operating room.
Call. Don’t wait, and don’t ask a forum.
That fifth item is the one people sit on. Don’t. Delayed presentation is the defining feature of atypical infection, and early recognition shortens treatment dramatically.
For context on how routine this procedure has become, liposuction was the most performed cosmetic surgical procedure in the United States in 2024 according to ASPS procedural statistics, and the South Atlantic region accounted for 28% of all cosmetic procedures nationally. Volume that high in Florida means the average outcome is good and the outliers are real.
So can you have an infection after neck liposuction? Yes. At a rate of roughly two in ten thousand in pooled data, with the odds shifting based on facility accreditation, anesthesia choice, and whether you’re stacking procedures. Those are things you can evaluate before you book.
Pick the facility carefully. Understand exactly what you’re consenting to. Then stop worrying about bacteria and start asking about skin elasticity, because that’s what actually determines whether you like the result. Standard bacterial infections usually resolve with oral antibiotics when treated promptly. Atypical mycobacterial infections require different drugs and much longer courses. In the CDC Florida cluster, 40% of patients needed intravenous antibiotics and treatment ran two to six months, with some requiring drainage or debridement.
Ask directly which accrediting body certifies the facility and when it was last inspected. Quad A accreditation is the most rigorous standard for outpatient surgery. Florida also strengthened its rules under HB 1561, effective May 2024, adding registration and financial responsibility requirements for offices performing higher-volume liposuction.
The last one got 11/10.
Some of the biggest factors affecting liposuction results have nothing to do with what happens in the operating room.

Will neck lipo give you the jawline you want?
It depends…
Someone can have fullness under the chin and assume the answer is neck liposuction. But what looks like “fat” from the outside can actually come from several different structures.
