The U.S. ran roughly 1.59 million cosmetic surgical procedures in 2024, according to the ASPS 2024 statistics report released last June. That’s a lot of room for unqualified providers to hide. I’ve seen revisions on both, and they’re costly in money and downtime.

I’ll push back on the industry habit of promising “natural results” without defining what that means for a specific anatomy. Natural for whom? Ask for specifics, not slogans.
Hospital privileges aren’t a status symbol. They’re a third-party check on competence. Hospitals don’t grant them lightly, and they pull them fast when something goes wrong.
If a surgeon operates only in their own office and has no local hospital privileges, ask why. Sometimes the answer is fine, because the surgeon has chosen an accredited center with proper AAAHC accreditation. Sometimes it isn’t. A real consultation includes a full medical history, questions about current medications (including GLP-1 weight loss drugs, which most surgeons want paused weeks before surgery), and a clear discussion of what surgery can and can’t do for your case. Three questions to ask:

A surgeon who refuses to show photos, or only has a tight curated gallery, is hiding something. Push for cases that match your body type, your skin tone, and your age. Actually, scratch the age. Anatomy match matters more for most procedures. I’ve seen patients in their 50s heal better than patients in their 30s because of skin quality, not chronology. The procedure is half the decision. The post-op plan is the other half.
Ask who handles your follow-ups, who you call at 2 a.m. when something hurts, and what the revision policy looks like if you’re not happy with the result. Some practitioners advertise board certified using cosmetic boards that require minimal training, so the specific board name matters more than the phrase itself.
For minimally invasive treatments performed by qualified nurse practitioners or physician assistants, yes. For actual surgical procedures, no. Surgical work requires a board-certified plastic surgeon operating in an accredited facility with board-certified anesthesia, not a retail med spa setting. Choosing the wrong setting is one of the most common red flags plastic surgeon shoppers regret missing.
Ask three. What's your revision rate for this specific procedure, who provides anesthesia and where, and can I see same-angle unfiltered photos of patients with anatomy similar to mine. Vague answers to any of those are themselves a warning sign. ASPS recommends patients also ask about medical history review and complication protocols.
Yes, even when surgery is performed in an office or surgical center. Hospital privileges mean a separate institution has vetted the surgeon's training and outcomes. Several recent cosmetic surgery mortalities documented by ASPS happened in non-accredited office settings without hospital backup. Privileges plus AAAHC facility accreditation is the safety floor for outpatient procedures.
