Two different people book this. One wants a longer-looking neck and a softer shoulder slope. The other wants the ache between the neck and shoulder to stop. Same injection, different definitions of working, and different waiting periods. Sort out which one you are before you count days.
Tension relief typically starts inside three to seven days. That’s the fast half of the answer.
Botulinum toxin begins chemical denervation of an injected muscle one to two days after the needle, and the effect builds through the first week, which is roughly what facial Botox feels like when it starts working. The trapezius follows the same biology, spread across more tissue.

An ultrasound study followed twenty women with trapezius hypertrophy after a five-point injection into each side. Muscle thickness dropped by four weeks and kept dropping through twelve weeks, which was the lowest point recorded. The authors identified four to twelve weeks as the window with the most obvious reduction, and wrote that appearance was obviously different at twelve weeks compared with before treatment. Four in five patients reported being very satisfied.
That gap between week one and week twelve is where most disappointment lives. People book on a social media before-and-after, feel their shoulders soften in a few days, then check the mirror at week two and decide it failed.
One limit belongs here. That study ran on twenty patients, all women, and the authors said the result needs a larger group to confirm.
Twelve weeks. That’s the honest number for the mirror.
Nothing you would notice. That finding deserves a minute.
A randomized, double-blind trial published in 2026 split twenty patients into two arms. One arm got forty units, the other got eighty. Both showed a measurable decrease in trapezius area at twelve weeks, with p values of 0.011 and 0.005. Neither arm beat the other. Neither reported complications. Satisfaction came in at moderate for both.
Two limits belong alongside that. The trial ran on twenty patients, and the funding came from the toxin’s maker.
The direction still tells you something. More product didn’t buy a better result or a faster one. It bought more product.
Which reframes the question people usually ask. The variable that moves your timeline isn’t how much went in. It’s where it went, and how much muscle was there to begin with.
Plan on three to six months, with the muscle drifting back toward baseline after that.
In the ultrasound work described above, thickness sat below baseline as far out as forty-four weeks, but the gap narrowed steadily after twenty weeks. The numbers stayed technically improved while the visible benefit faded. Those are two different things, and a clinic quoting you the longer figure is quoting the measurement, not the mirror.
Facial Botox behaves similarly. Most people get three to six months before movement returns.
One more thing about the fade. It doesn’t switch off. The muscle regains function gradually, so you’ll usually feel tension creeping back weeks before the shoulder line changes shape again. That drift is your cue to book, not the calendar.
Likely yes, and this is where a lot of Formation patients run into it.

Three things move your dates more than anything a clinic can promise up front. None of them is the unit count, which is also why how often to treat has no single answer.
A thicker trapezius takes longer to visibly thin. Same treatment, later payoff.
An active shoulder girdle rebuilds faster. Your result may arrive on schedule and leave early.
Botulinum toxin products carry different unit strengths. A unit is not a universal measure, so a number you read about one product tells you nothing useful about another.
Onset speed varies between them too. That’s part of why two people treated the same week can report different first-week experiences and both be normal.
Botox Cosmetic isn’t approved for the trapezius. Most articles on this topic skip that.
The approved cosmetic areas are glabellar lines, lateral canthal lines, forehead lines, and platysma bands, which joined the list in 2024. Trapezius contouring isn’t there, which makes this an off-label use. Off-label isn’t illegal and it isn’t unusual, but it does mean the timing and dosing evidence hasn’t been held to the standard an approved indication faces.
Three points from the prescribing information deserve your attention. The boxed warning covers distant spread of toxin effect, with symptoms reported hours to weeks after injection. The label states that serious reactions including excessive weakness and swallowing trouble have been reported with unapproved uses. And it notes that botulinum toxins can weaken neck muscles that serve as accessory muscles of breathing.
That last point sits right where this injection goes. It matters more than the unit count.
The label also answers the repeat question better than most clinics do. Dosing more often than every three months hasn’t been clinically evaluated, and the label advises the lowest working dose at the longest feasible intervals to limit antibody formation.
Read that twice if you’re the type to chase a faster result with a bigger number. The manufacturer’s own document points the other direction.
Some people shouldn’t get this, and Dr. Grandhige would rather say so than book it.

If your bulk is trained muscle doing exactly what you asked it to do, weakening it is a trade rather than a fix. If your shoulder line reads wide because of bone structure, toxin won’t move it.
