Filler looks puffy at first. Swelling from the needle combines with the water-binding pull of hyaluronic acid, so the treated area looks fuller than the final result. Puffiness usually peaks 24 to 72 hours after injection and settles over one to two weeks. Lips and the area under the eyes stay swollen the longest.
So the face you see on day two isn’t the face you’re keeping. I’ve lost count of how many people have called Formation on a Wednesday morning sure we overfilled them on Monday. Almost none of them were overfilled.
Nobody warns you that the peak arrives later instead of right away.
For almost everyone, yes. Hyaluronic acid is a sugar molecule that holds water, which is the reason filler works and also the reason week one looks puffier than you expected.
Once the gel sits under your skin, it starts pulling fluid from the tissue around it. Meanwhile your body is already sending its own fluid to every spot the needle passed through. Stack those two things and you get a temporary layer of volume sitting on top of what filler actually does.
Different gels behave differently. Soft, low-density products used in lips draw more water and swell more in the first week. Denser gels placed deep on bone for jaw or cheek structure swell less because they sit under thicker tissue. A blunt cannula usually causes less trauma than a needle across the same area, though it doesn’t erase swelling. No product avoids it. Any brand claiming minimal swelling is selling you a marketing line, not biology.
Swelling peaks 24 to 72 hours after your appointment, not on the drive home. Day two and day three are almost always worse than day one.
Here’s the pattern I see most often. You leave looking good, because the gel is placed and your tissue hasn’t reacted yet. You wake up the next morning tighter. By day three you’re at your widest. Then it turns, and most of the visible fluid is gone somewhere between day five and day fourteen.
Bruising runs on its own clock and can outlast the puffiness by a few days, so whether filler bruises has nothing to do with how much product went in. Tenderness fades faster than either one. A week of soreness is ordinary and doesn’t mean anything went wrong with placement.
Swelling lasts one to two weeks in most areas, but the site matters more than the volume. Thin skin with good blood supply swells harder and holds fluid longer than thick skin over bone.
Lips are the worst offender, and it isn’t close. The tissue is thin, mobile, and full of small vessels. Expect a real jump on day two, sometimes double what you’ll finish with. Most of it clears by day seven, though small firmness can linger to week three. Early unevenness in the first few days is common, since one side often holds more fluid than the other.
The under-eye area drains slowly. Lymphatic flow there is poor compared with the rest of the face, so fluid pools and mornings look worse than evenings. Puffiness here can take three to four weeks to settle rather than one. This area also carries the highest risk if too much goes in, because product sitting in thin tissue can hold water for months.
Jaw and chin filler sits deep, often right on bone, under skin that’s thicker and less reactive. Most swelling here clears in five to seven days. Plenty of people still think the lower face looks wider at first, which is fluid spreading along the jaw rather than the gel itself.
Here’s my unpopular position. Most of the bad filler you see online isn’t bad injecting. It’s someone who judged their result at day three, decided it was wrong, and went back for more before the first round settled.
Demand hasn’t dropped, whatever the filler fatigue headlines say. National procedure counts from the American Society of Plastic Surgeons show 5,331,426 hyaluronic acid filler treatments in 2024, up roughly 1% from 5,294,603 the year before. Lip augmentation with filler accounted for 1,589,502 of those on its own. Across all minimally invasive treatments, 28,243,407 procedures were reported, and neuromodulators made up 9,883,711 of that total.
That’s a lot of people looking in a mirror on day two. Very few of them have a placement problem. Most have a calendar problem.
The industry made this worse by selling instant results. Filler is instant in the sense that volume appears immediately. It is not instant in the sense that what you see is what you get.
Swelling improves week over week. Overfilling doesn’t. That single test settles most cases.
Fluid behaves like fluid. It’s worse in the morning, better by evening, worse after a salty dinner, better after a good night’s sleep with your head up. It shrinks a little every few days. If your face is on that curve, you’re swollen.
Excess product behaves differently. It holds steady past week four, it looks off when you smile or talk rather than only at rest, and it doesn’t respond to sleep or hydration at all. That’s a separate problem from swelling, and it’s the pattern behind most cases where filler goes wrong.
Distortion during movement is the sign I take most seriously.
Run your own test instead of guessing. Take one photo the moment you wake up and another at 9pm, in the same room and the same light, on day three and again on day ten. Fluid shows a clear gap between morning and night. Product looks identical in all four frames.
One correction to something I used to tell patients. I’d repeat the line that filler lasts six to twelve months, so any excess would sort itself out. That framing isn’t right. An imaging follow-up published in Plastic and Reconstructive Surgery Global Open tracked hyaluronic acid still visible on MRI at 27 months in the mid-face and lateral face, and ultrasound work has found filler years after injection. Product stacks. Someone topping up every nine months for five years is not starting from zero each time.
Very little shortens the timeline. What follows keeps you from stretching it out, which matters more than most aftercare sheets admit.
I get asked about lymphatic massage constantly. For fresh filler in the first week, I’d skip it. Save it for the surgical patients where it earns its place.
Past two weeks, I stop calling it swelling and start looking for a reason. Ordinary fluid is gone by then in almost every case.
Delayed reactions are real. FDA safety guidance notes that most filler side effects show up soon after injection and clear within days to weeks, while some reactions surface weeks, months, or even years later, sometimes after an illness, a vaccine, or dental work. Firm lumps, redness that keeps returning, and one-sided fullness that appeared out of nowhere all deserve an appointment rather than another week of waiting.
A few things need same-day attention, not a two-week wait. Pain that keeps climbing instead of easing. Skin turning white, blotchy, or dusky. Any change in vision. Those point toward filler entering a blood vessel, which is rare and time-sensitive.
Two questions to ask before anyone injects you. Does the office keep hyaluronidase on site, and who exactly will see you if something goes wrong at 8pm on a Saturday. Vague answers to either one are your signal to book somewhere else.
