You can’t lose your double chin with diet and exercise because your body doesn’t accept fat loss requests by location. When you drop weight, fat leaves from everywhere at once, in an order your genes set long before you started training. Some people shed the pad under the chin early. Others carry it at every weight they’ve ever been.
I’ve been operating for more than 15 years, and this is the conversation I have most often about necks. A patient walks in lean, disciplined, sure they haven’t worked hard enough. Usually they’ve worked plenty.
Submental fullness, the medical term for a double chin, doesn’t respond to diet and exercise the way belly fat does. Weight loss reduces fat everywhere at once, and no exercise moves fat from one spot. When fat isn’t the only cause, losing weight can leave the neck looser instead of sharper.
Spot reduction isn’t real. Your body pulls fat out of storage systemically, releasing it from wherever it decides, not from above whatever muscle you just worked.
Do a thousand neck curls and you’ll build a stronger platysma. The fat sitting on top of it doesn’t notice.
Where your body gives up fat first is largely inherited. So is how many fat cells sit under your chin. Two people can lose the same amount of weight, and one keeps the double chin.
Because weight loss addresses one of four possible causes, and often not the one you have.
Getting leaner removes fat. It does nothing for a muscle that has split into visible cords, skin that has lost its recoil, or a chin that sits too far back to give your jaw a hard edge. Whether a double chin is genetic has more to do with those last three than most people expect.
Fast weight loss can make the neck look worse before it looks better. Skin needs time to contract. Lose fat quicker than skin can follow and you’ve traded fullness for looseness.
Before I tell any patient how to treat a double chin, I read the neck in four layers. Fullness and aging show up across multiple layers at once, and each one has its own answer. Fixing the wrong layer is how people end up disappointed after a technically clean procedure.

Fat is the layer patients assume they have. Sometimes they’re right. Submental liposuction takes the pad out, and the fat cells removed don’t grow back, which is why neck liposuction produces a permanent change that dieting never will.
There’s an injectable route too. Deoxycholic acid breaks down fat cells under the chin across a series of sessions. The FDA has approved one deoxycholic acid product for submental fat and nothing else, and has warned patients about unapproved fat-dissolving injectables marketed for the same purpose. Swelling is the trade-off, and it comes back with every session.
The platysma is a thin sheet of muscle running from the collarbone up over the jaw. With age it separates down the middle, and the free edges roll forward into cords. Those are platysmal bands.
Fat removal does nothing for them. Take fat out from around a band and the band gets more visible, not less. Bands drive the look people call turkey neck, and whether you can reverse turkey neck without surgery depends on which layer dominates. MyEllevate, a permanent suspension suture, paired with platysmaplasty, is the repair when bands are the complaint. Unlike barbed threads that dissolve, that suture stays put and can be removed, and the lift is there the day it’s placed.
Skin decides whether fat removal alone will look good. If your skin still has recoil, it shrinks down over the new contour. Past that point it won’t, and you get a fold that hangs at rest.
The elasticity window runs roughly from skin age 25 to 55. Inside it, energy devices earn their place. Renuvion, a helium plasma device, treats the deepest layer of the skin and the fibroseptal network beneath it, the connective tissue foundation between skin and muscle. FaceTite, a radiofrequency device, reaches that same depth. Lasers and microneedling work from the top down and can’t get to that foundation. There’s a lot you can do with skin tightening without surgery when the read is right.
Past the window, no device saves it. Those patients need skin removed, and a neck lift is the operation that does it. I refer them out to surgeons who do that work well, because sending someone to the right person beats selling a treatment that won’t hold.
Layer four is the one almost nobody arrives thinking about. A chin that sits too far back gives skin nothing to drape over, so the neck reads full even when there’s barely any fat in it.
Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, both add projection at the chin and along the jaw. Structural filler in the right spot can build a superhero jawline in someone who’d have gotten nothing out of fat removal. Sometimes the better plan is chin augmentation alongside contouring rather than either one alone.
No. Chin tucks, tongue presses, the ball-under-the-jaw routine, they strengthen muscle. That’s the whole effect.
Muscle weakness isn’t what’s causing your double chin. If anything, a stronger platysma with visible bands reads worse. I’d rather a patient spend that effort on protein and sleep, both of which change how they heal if they eventually treat this.
A systematic review published in the Journal of Cosmetic Dermatology in 2026 identified injectable deoxycholic acid as the most studied nonsurgical option for submental fat, with satisfaction across published trials falling between 67% and 86%. That review’s literature search closed in March 2025, so nothing newer sits inside it. Nowhere in that evidence base does exercise reduce fat in one area. When patients ask me about non-surgical options, the honest answer starts with which layer we’re treating.
GLP-1 medications made this question far more common, and for a lot of people they changed the answer.
The American Society of Plastic Surgeons logged 837,485 patients prescribed weight loss medication by its member surgeons in 2024, the first year it tracked that at all. The same 2024 statistics report counted 24,000 submental liposuction procedures and a 2% rise in neck lifts, while noninvasive fat reduction fell to 447,581 procedures from 745,967 the year before, a drop of 40%. Those figures come from a member survey carrying a margin of error of plus or minus 4.62 percent, so they track direction better than exact volume. The pattern still holds. People are losing weight quickly, then showing up with a neck that fat reduction alone won’t fix.
The South Atlantic region, which includes Florida, took the largest share of submental liposuction in that report at 32%.
What I see in the office matches. Someone drops weight fast, the face hollows, neck skin doesn’t retract, and a fat problem becomes a laxity problem. Taking out more fat is the wrong move.
Match the tool to the layer and the result holds. Mismatch it and you’ll be back.
Fat gets removed. Muscle gets repaired. Skin gets treated with energy from underneath. A recessed chin gets projection. Most double chin plans need two of those four, and working out which two is the job. At Formation we often pair chin and neck liposuction with radiofrequency tightening in one visit, because pulling fat out without addressing skin is how you end up loose.
I turn people away for this. Not often, but regularly. A lean patient with a shallow chin and no real fat pad gains nothing from liposuction, and taking what little fat is there can hollow the area and age them. Saying no is part of the job. I’d rather lose the booking than hand someone a result they’ll want redone.
Cooling-based fat reduction comes up constantly. A 2025 meta-analysis in Facial Plastic Surgery pooled cryolipolysis studies and reported a mean fat thickness reduction of 2.78 mm. That’s a real number and a small one. The analysis combined different devices and protocols, which limits how tightly it applies to any one treatment. ASPS also flags a risk of paradoxical fat hyperplasia with cryolipolysis, rare at under 1% but three times more common in men than women. Given how much male contouring we do, that one matters here. For a pad you can pinch, it won’t be enough.
Neck work has a short recovery and a long finish. Those aren’t the same thing.
The first few days bring swelling and tightness with a chin compression garment on. By one week most patients are back at work and back to training, wearing the garment 12 to 24 hours a day. Swelling settles across two to four weeks. Skin tightening becomes visible around three months. The final result lands somewhere between six and nine months.
Anyone judging their FaceTite results at week six is looking too early, because tissue is still remodeling.
The device on the counter is the same device everywhere. What differs is what happens in the weeks around it.

Fat loss happens systemically, so your body releases fat from everywhere at once rather than from one area you target. Genetics set the order, and for some people the submental pad is last. If the cause isn't fat at all, weight loss won't touch it.
It depends which of four causes you have. Weight loss reduces the fat layer, and nothing else. If your double chin comes from platysmal bands, loose skin, or a recessed chin, losing more weight can make the area look looser rather than sharper.
No. Chin exercises strengthen the platysma muscle, which was never the problem. A stronger muscle with visible bands can look worse. No published evidence supports exercise as a way to reduce fat in one region of the body.
Rapid weight loss doesn't give skin time to contract, so you trade fullness for laxity. This is common enough that ASPS tracked GLP-1 prescriptions for the first time in its 2024 report, logging 837,485 patients prescribed by member surgeons, alongside a 2% rise in neck lifts. Those numbers come from a member survey with a margin of error of plus or minus 4.62 percent.
Fat is soft and you can pinch it between two fingers. Loose skin shows as crepey texture and a fold that stays put when your face is at rest. Most people have some of both, and the ratio determines whether fat removal alone will look good. Only a hands-on assessment settles it.
Rarely, because inherited fat cell distribution is what put it there. Family patterns usually point to structural causes too, like chin projection or early platysma separation. Those don't respond to lifestyle change at any weight.
Swelling settles across two to four weeks and most patients are back to work and training at one week. Skin tightening becomes visible around three months. The final contour lands between six and nine months, so judging results earlier than that gives a false read.
