Jowls are the soft, sagging skin and tissue that drift below your jawline as you age. They form when collagen breaks down, facial fat shifts, and gravity pulls everything south. Most people see early signs between ages forty and fifty, though sun damage and genetics can push that earlier.
To get rid of jowls in 2026, you have real choices, and they fall into three buckets. Daily habits help slow them down. Injectables and energy-based treatments soften mild to moderate sagging. Surgery handles the heavy lifting when skin laxity is advanced. The right pick depends on how loose your skin already is, not on what’s trending on social media.
I’ve watched plenty of patients try one thing after another. Most of them start with the cheapest option, get disappointed, and then call a board-certified surgeon. So let me save you a step. I’ll walk you through what moves the needle. I’ll also tell you what’s a waste of money.
A few things this article won’t cover in depth: cream-only solutions and contour-changing apps. They don’t address deep skin laxity. Period.
Jowls form when your skin loses collagen and elastin, your face loses fat in the cheeks, and gravity drags everything south. That’s the short answer.
The longer answer involves four big drivers. Sun damage tops the list. According to the American Academy of Dermatology, about 90 percent of visible skin aging comes from UV exposure, not from how many birthdays you’ve had. Smoking and chronic dehydration shred collagen too. Repeated weight loss, especially the kind people now experience on GLP-1 medications, can leave skin hanging because the underlying fat pads shrink faster than the skin can rebound. The ASPS noted in its 2024 procedural statistics that demand for facial rejuvenation among weight-loss patients climbed sharply.
Genetics matters too. If your mom had early jowls, you probably will. Frustrating, but useful information.
One thing that doesn’t really cause jowls: “tech neck” posture. The cell phone panic gets recycled every year. It contributes far less than UV damage or genetics, and worrying about it distracts people from the bigger fixes.
Mostly no. Facial yoga, mewing, and chin tucks have a small effect on muscle tone, but they don’t tighten loose skin or replace lost fat. A widely cited Northwestern study showed modest improvement at best after months of daily practice.
Save the time and put it toward sun protection.
If you still want to try, the safer exercises involve gentle resistance against the platysma muscle in the neck. Tilt your head back, press your tongue to the roof of your mouth, and hold for five seconds. Repeat ten times. Will it reshape your jawline? No. Could it help slightly with muscle tone? Sure.
Selling jowl-fixing exercises as if they replace medical treatment is the kind of advice that gets shared on social media and leaves people frustrated when nothing changes. Be realistic about what you really want.
Skin laxity gets graded as mild, moderate, or advanced. Each grade has a sensible treatment match. Mismatching them is how people end up spending money twice.
Mild laxity responds to fillers and energy devices. Moderate laxity often needs combination treatments. Advanced laxity usually calls for surgery if you want results that last more than a year or two.
The whole point of a consultation with a board-certified specialist is to figure out where you actually fall. Most people self-assess one grade higher or lower than reality.
Fillers don’t lift skin. They replace the fat that’s gone missing in your cheeks and along the jawline. When the cheek looks fuller, the jowl looks smaller because the eye sees a smoother transition.
Hyaluronic acid fillers are the most common choice for jowl support. Calcium hydroxylapatite and poly-L-lactic acid stimulate collagen too. The trade-off is reversibility. Hyaluronic acid can be dissolved if you don’t like the result. Collagen stimulators can’t. I’d go reversible the first time you try this method.
Results usually last between nine and eighteen months, depending on the product and how your body metabolizes it.
A neck lift remains the most reliable fix for moderate to advanced jowls. The surgeon trims excess skin, repositions deeper tissues, and tightens the platysma muscle. Results last anywhere from seven to ten years for most patients.
Recovery isn’t trivial. Plan for two weeks of obvious bruising and another four to six before you’d want to be in photos. Complication rates with experienced surgeons are low, but they’re not zero. The most common issues are temporary numbness, asymmetry, and visible scarring around the ears. A neck plastic surgery consultation is where you find out if you’re even a candidate.
If you’ve already tried injectables and energy devices and your skin still drapes below the jaw, you probably are. If you haven’t tried anything yet and your laxity is mild, you’re not.
Ultrasound devices use focused energy to heat tissue several millimeters below the skin’s surface. The heat triggers new collagen production over the following three to six months.
Real-world results in clinic settings? Patients with mild to moderate laxity see a 20 to 30 percent improvement on average. People with thick, heavy skin or advanced sagging see much less, sometimes nothing. This is the treatment where expectations get out of hand fastest.
One session, no downtime, mild discomfort during. The fit works well for the right candidate and fails badly for the wrong one.
Radiofrequency treatments, including microneedling versions, heat the deeper dermis to remodel collagen and tighten connective tissue. They work especially well on the lower face because they can also melt small amounts of stubborn fat under the jawline. Many practices in Tampa now combine RF with a non-invasive jawline procedure for patients who want lift without surgery.
The FDA issued a safety alert in October 2025 about RF microneedling risks including burns, fat loss, and nerve damage when devices are used incorrectly. The takeaway isn’t to avoid the treatment. It’s to never let an undertrained provider hold the device.
Plan for three to four sessions spaced a month apart. Maintenance treatments yearly.
Threads come and go. PDO and PLLA threads can give a temporary lift, usually six to nine months, and they’re useful as a bridge for younger patients who aren’t quite ready for surgery. They’re not a permanent fix, and aggressive thread lifts have led to FTC warning letters about overstated claims.
Lasers, chemical peels, and microneedling improve skin texture and tone. They don’t address structural sagging. If your concern is mostly surface (fine lines, sun spots, dullness), they help. If your jowl hangs visibly past the jawline, they won’t touch it.
Acupuncture, lymphatic massage, and face yoga keep showing up in articles. There’s almost no clinical evidence they reduce jowls in any meaningful way.
Sunscreen, sleep, hydration, and not smoking. That’s the prevention list. Everything else is window dressing.
Sunscreen moves the needle most. Daily SPF 30 or higher with broad-spectrum protection reduces visible photoaging by a substantial margin over a lifetime, according to multiple peer-reviewed studies. Reapply if you’re outside more than two hours.
Sleep on your back when possible. Pressing the same side of your face into a pillow every night for forty years creates asymmetric folds. It’s a small effect, but it adds up.
Eat enough protein. Skin is built from amino acids, and chronic underfeeding (common with very aggressive weight-loss plans) speeds up skin laxity. The NIH dietary recommendations suggest around 0.8 grams of protein per kilogram of body weight as a baseline for adults.
And accept that you can’t prevent everything. Some sagging is age. The goal is slowing it down, not stopping it.
If I had to pick the single most effective non-surgical pairing for jowls in 2026, it’s a jawline filler treatment plus a collagen-stimulating energy device, done by someone who’s performed the combination hundreds of times. For advanced laxity, a neck lift with a board-certified facial surgeon still gives the best long-term result. There’s no app, no cream, no exercise that competes with those two options.
Most disappointment with jowl treatments comes from picking the wrong tool for the job. A consultation with someone like Dr. Grandhige, who looks at your skin in person and tells you the truth about what each option will and won’t do, saves you more money than any discount package ever will. That’s the part of the process most people skip. It’s also the part that matters most.
Jowls form when collagen and elastin break down, facial fat shifts downward, and gravity pulls tissue below the jawline. Most people see early signs between ages forty and fifty. Sun damage drives roughly 90 percent of visible facial aging, so genetics and UV exposure together determine when yours start.
Yes, but the results are subtler and shorter-lasting than surgical correction. Fillers, ultrasound skin tightening, and radiofrequency devices work well for mild to moderate sagging. Most non-surgical options last between one and two years before they need touch-ups.
The clinical evidence for facial exercises is thin. They may slightly improve muscle tone in the platysma and lower face, but they don't tighten loose skin or replace lost fat. If your jowl is visible past the jawline, exercises won't fix it.
A neck lift performed by a board-certified facial surgeon gives the best long-term outcome for advanced sagging, with results lasting seven to ten years. Non-surgical options can buy time but they won't match a surgical result. The trade-off is recovery time and cost.
Most hyaluronic acid fillers used for cheek volume and jawline support last nine to eighteen months. Calcium hydroxylapatite and poly-L-lactic acid options can last longer because they stimulate your own collagen production. How long results last depends on metabolism, the product used, and how many syringes were placed.
When performed by a qualified provider, radiofrequency treatments have a strong safety record. The FDA issued a safety communication in October 2025 about RF microneedling risks including burns, fat loss, and nerve damage from improper use. Always verify your provider's training and experience with the specific device.
You can slow them down significantly, but not stop them completely. Daily broad-spectrum sunscreen, not smoking, sleeping on your back, and eating enough protein all reduce the rate of collagen loss. Sunscreen alone has the biggest effect over a lifetime.

The power of a profile transformation. 🔥
For this patient, we combined PDO Lifting Threads and advanced chin/jawline filler to completely redefine his lower face. By strategically lifting the mid-to-lower face and sculpting the jaw, we didn’t just enhance one feature—we elevated his entire facial appearance.
An incredible, natural-looking result that brings balance and confidence from every angle.
Note: Minor bunching or puckering is completely normal immediately following a PDO thread lift. The slight bunching visible in the after photo will fully subside on its own over the next 2-4 weeks as the threads settle.
