Botox does help excessive head sweating for most people who try it. It won’t cure the problem, but it can shut down the sweating on your scalp, forehead, and face for months at a time. Small injections calm the overactive nerves that tell your sweat glands to fire. One thing up front. This is an off-label use, and the FDA cleared Botox only for severe underarm sweating, so head and facial use rides on separate research and on the skill of your injector.
Botox for head sweating, known medically as craniofacial hyperhidrosis, uses a purified protein called onabotulinumtoxinA. A provider injects it just under the skin of the scalp, forehead, or face, where it blocks the chemical messenger that switches sweat glands on. Sweating drops within a few days, and results usually hold for four to six months before you need another round.
This guide sticks to head and facial sweating. We’re not covering underarm treatment, the one spot the FDA signed off on, except to draw a comparison. If sweat runs down your forehead in a cool room or soaks your hairline before a meeting, that’s the problem we’re talking about.
Craniofacial hyperhidrosis is heavy, uncontrolled sweating on the scalp, forehead, and face that shows up without the usual triggers like heat or exercise. The sweat glands there overreact to a normal nerve signal, so they keep firing when they shouldn’t.
It can start early in life with no clear cause, or it can trace back to something else, like the hot flashes of menopause, thyroid trouble, or certain medications. Either way, the sweating is real and it wears on people. Many patients tell us they carry towels, avoid dark hats, or reschedule around anything that makes them nervous.
Doctors treat this in a stepwise order. Antiperspirants and prescription pills come first. When those fall short, injections become the next move, and that’s the same order you’d see with underarm Botox.
Botox works by cutting the signal between your nerves and your sweat glands. Your nerves normally release a chemical called acetylcholine, which tells the glands to produce sweat. Botox blocks that release, so the glands go quiet in the treated patch of skin.
The effect stays local. It only calms sweating where the injections go, which is why a provider maps the area before starting. If you want the longer version of what Botox does once it’s in the skin, the short story is that it relaxes the signal, not the gland itself.
That local action is the whole point for the head. A pill would dry out your entire body and hand you side effects like dry mouth. Injections stay in the zone that’s actually sweating.
Botox can treat three zones on the head: the scalp, the forehead, and the wider face. The forehead and hairline are the most common targets, since that’s where craniofacial sweat tends to pool and drip.
A provider often runs a starch-iodine test first. They coat the skin, let it dry, and watch which spots turn dark as you sweat. Those dark spots become the injection map. It’s a simple way to aim the dose instead of guessing, and it’s close to how forehead Botox gets planned for wrinkles, just aimed at glands instead of muscles.
The scalp is trickier. It’s a large area with lots of glands, so it usually needs more units and a careful hand to avoid drifting into muscles you don’t want touched.
The evidence for facial and head sweating has grown strong, even though it’s still off-label. In a 2024 study of 77 people with facial hyperhidrosis, symptom scores on the standard severity scale fell from 3.6 to 1.2 after treatment and held at 1.3 a full year out. Measured sweat output dropped from 0.75 grams to 0.18 grams over a 15-minute test.
Quality-of-life scores moved just as hard. On the dermatology life index, where higher means worse, patients went from 24.8 down to about 6.2 and stayed there at 12 months. Those aren’t small shifts. They’re the difference between planning your day around your sweat and forgetting about it.
The catch, stated plainly, is that most of this data comes from smaller, recent studies, not the huge trials that backed underarm approval. The results point one direction, but the research base is younger.
A head-sweating session is quick and done in the office. Plan for the visit to run under an hour, with the actual injecting taking about 10 to 15 minutes.
Your provider marks the map, cleans the skin, and often applies numbing cream or ice, since the scalp and forehead can be tender. Then come the injections, spaced in a grid across the sweaty zone using a fine needle. You’ll feel small pinches, not deep pain. Most people head straight back to work, which is one reason Botox to reduce sweating appeals to busy patients. Skip a hard workout and hot showers for the rest of the day, and that’s about the extent of the downtime.
Prep is light. Tell your provider about any blood thinners well ahead, since stopping them early can cut down on bruising, but never stop a medication on your own.
Results start showing up in two to four days and reach full strength around two weeks. From there, most people get four to six months before the sweating creeps back.
One myth trips people up more than any other. People assume head Botox lasts as long as underarm Botox. It usually doesn’t. Underarm results can stretch past six months, while the scalp and face often run shorter and need touch-ups a bit sooner. That’s not a failure of the treatment. It’s just how these areas respond, and knowing it up front saves you the frustration of thinking something went wrong.
Botox also isn’t for everyone. It’s not recommended if you’re pregnant or breastfeeding, and it’s off the table with certain nerve or muscle conditions, which is the same reason we tell people to check whether it’s safe during pregnancy before booking anything.
Side effects from head-sweating Botox are usually mild and short, like soreness, a little swelling, or a bit of bruising where the needle went in. Because it’s a targeted treatment and not a whole-body drug, you skip the dry mouth and stomach issues that oral options bring.
The real risk on the head is placement, not the drug. If the toxin drifts into a forehead muscle, you can get temporary heaviness or an uneven look until it wears off. This is where a rushed or undertrained injector costs you. On the face, precision is the whole game.
Serious reactions are rare and tend to come from unapproved products. The FDA has warned about counterfeit and unlicensed botulinum toxin sold online, some of it marketed straight at hyperhidrosis patients. Cheap vials from the wrong source aren’t a bargain. They’re a health risk.
Pick your injector before you pick your price. On the scalp and face, technique decides whether you get clean results or a lopsided forehead, so experience with these exact areas matters more than a low quote.
