Yes. In most cases you’re awake during neck liposuction. The procedure is usually done under local anesthesia, sometimes with light sedation, so you stay conscious and comfortable while a board-certified surgeon removes fat under the chin and along the jawline. General anesthesia is an option for larger or combined cases, but for an isolated neck it’s often unnecessary.
I’m Dr. Gopal Grandhige, and at Formation in Tampa this is one of the procedures I reach for most when someone wants a sharper chin and jaw. Here’s a plain answer to what “awake” actually means, who it fits, and where it falls short.
Awake neck liposuction is a fat-removal procedure for the chin and jawline done under local anesthesia instead of general anesthesia. You stay conscious while the surgeon numbs the area and suctions out small, targeted fat deposits. It suits people with a defined pocket of submental fat and reasonable skin elasticity.
Awake neck lipo swaps general anesthesia for a high concentration of local numbing medicine, so you’re conscious the whole time. I inject a tumescent solution (a blend of saline, lidocaine, and epinephrine) into the fat beneath your chin. That fluid numbs the area, shrinks the small blood vessels to limit bleeding, and firms up the fat so it lifts out cleanly.
Traditional lipo often puts you fully under. That means a breathing tube, an anesthesiologist in the room, and a groggy wake-up. For a treatment zone as small as the neck, you can skip all of it. ASPS surgeons performed roughly 24,000 chin liposuction procedures in 2024, up about 1% over the prior year, and a growing share for a small area like the neck are done awake.
There’s a real working advantage too. When you’re awake, I can ask you to lift your chin or turn your head so I can check the contour from every angle as I go. That live feedback is hard to get when someone’s asleep and flat on the table.
The biggest win is dodging the risks that come with general anesthesia: allergic reactions, breathing complications, and the nausea and fog that follow. You walk in, and you walk out the same day, clear-headed.
The tumescent fluid also causes less tissue trauma, so bruising and swelling tend to run lighter than with older methods. That’s part of why people ask how long the results last and are surprised the fat removal is permanent as long as weight stays stable.
For the right patient, awake neck lipo is one of the safer ways to contour the jaw. Skipping general anesthesia removes an entire category of risk, and the lidocaine solution has mild antibacterial properties that lower infection odds.
The numbers back that up. Serious complications for liposuction overall sit near 1% to 1.5%, and isolated neck cases typically run under 5%, mostly minor issues like temporary numbness or small contour irregularities. RealSelf reports a 94% worth-it rating across 509 recent chin liposuction reviews, which tracks with what I see in my own patients.
One caveat I won’t sugarcoat. “Awake” is only as safe as the person holding the cannula. The single most expensive mistake I see is choosing a non-surgeon at a med spa, then paying for a revision later. Ask how many neck cases your provider has actually done.
The ideal candidate has a distinct pocket of fat under the chin, decent skin elasticity, and a stable weight. The neck is one of the areas that respond best to liposuction because the fat pad sits in an easy-to-reach spot.
Age matters less than skin quality. A 55-year-old with springy skin can be a better candidate than a 35-year-old whose skin won’t bounce back. If your concern is loose muscle bands or heavy sagging rather than fat, lipo alone won’t fix it, and you may be looking at a neck lift instead. That’s an operative case my colleague Dr. Ahmad Saad handles.
Neck lipo and fat-dissolving injections aim at the same double chin, but they get there differently. Lipo removes fat in one session with immediate, permanent results once you heal. Injectable fat treatments need multiple visits, bring weeks of swelling each round, and give a subtler change.
Surgical liposuction still leads for precise, one-and-done contouring, which is why it ranks among the top surgical procedures nationwide. If your fat pocket is small and you’d rather avoid any incision, non-surgical jawline contouring can be a reasonable starting point. For a defined result that holds, lipo usually wins.
Here’s the part the glossy blogs leave out. Remove the fat, and the skin that was draped over it has to shrink to the new shape. If your skin elasticity is poor, you can trade a full neck for a slightly loose one.
I’d rather tell you that up front than fix it after. For patients with borderline skin, I’ll often pair the lipo with radiofrequency skin tightening in the same visit to help the skin redrape. Used as a marketing gimmick, these devices don’t perform miracles. Skin quality still sets the ceiling on your result.
First I mark your neck while you’re sitting up, so the contour lines match how your face looks in real life. Once the area is fully numb, I make one or two incisions smaller than a grain of rice, usually tucked under the chin where any mark hides.
Then I remove the fat in careful passes, asking you to shift your head as I check both sides for symmetry. For patients whose skin needs help redraping, I finish with a quick energy pass using a device cleared by the FDA for skin coagulation after lipo. You can talk to me and the team the whole time, close your eyes, or use a drape for privacy. Most neck cases wrap in about an hour.
Recovery is faster than general-anesthesia lipo because your body isn’t clearing sedative drugs. Here’s the timeline I give patients in 2026:

It depends on your skin elasticity. Younger, springier skin usually redrapes well over the new contour. For borderline skin, pairing lipo with radiofrequency tightening in the same visit helps the skin shrink to fit. Poor elasticity can leave mild looseness, which is why an honest skin assessment matters before surgery.
ย For a defined double chin, liposuction gives a stronger, one-session result that's permanent with stable weight. Fat-dissolving injections need multiple visits and produce a subtler change with swelling after each round. Injections suit very small fat pockets; lipo suits people who want a clear, lasting jawline.
ย It's riskier. Complication and revision rates climb when unqualified providers at med spas handle it. A board-certified surgeon has the training to manage anatomy near the jawline safely. Ask how many neck cases the provider has done and request to see their own before-and-after photos.
