Neck liposuction can cause nerve damage. But the version most people picture, a permanent facial droop, almost never happens when a board-certified surgeon does the work. The usual outcome is temporary: numbness, tingling, or a tight feeling under the chin that fades over weeks to months as small nerves settle down.
Neck liposuction removes fat under the chin and jaw through tiny tubes. It can irritate or, in rare cases, injure nerves close to the surface, which usually shows up as short-term numbness or tingling. Lasting nerve damage is uncommon and tied mostly to who performs the procedure and the technique they use.
Neck liposuction is a fat-removal procedure for the area under the chin and along the jaw. A neck lift is bigger surgery that also tightens muscle and removes loose skin. They solve different problems, and mixing them up is how people end up disappointed.
Your surgeon makes one or two small openings, usually under the chin or behind the ears. A thin tube called a microcannula slides in and suctions out fat. Most necks need only local anesthesia with light sedation. The whole thing often takes under an hour.
The fat under your chin is what blurs the line between jaw and neck. Take it out, and the angle sharpens. That sharper jawline is the entire point of the procedure, and for the right candidate it does the job in one short visit.
Here’s where I’ll push back on something patients read online constantly. Liposuction does not tighten skin. If your skin still snaps back on its own, lipo alone gives a clean result. If it doesn’t, pulling the fat out can leave the skin looser than before. That isn’t a complication. It’s physics.
Older necks, and necks after big weight loss, often need skin tightening or a neck lift, not just suction. A good surgeon tells you that at the consult instead of selling you the cheaper option and hoping for the best.
Yes, it can, because the work happens close to nerves that control feeling and some facial movement. The risk is low, and most nerve effects clear up on their own. It’s still a documented risk worth understanding before you commit.
Two nerves matter most here. The great auricular nerve sits near the surface and handles feeling around the lower ear and upper neck. Bump it, and you get a numb patch in that area. It usually recovers.
The one surgeons respect most is the marginal mandibular branch of the facial nerve. It runs near the jawline and helps move your lower lip. Injure it and the smile can look uneven on one side. This is rare, and a careful surgeon stays well below the jaw border to stay clear of it. At Formation, our plastic surgeon Dr. Ahmad Saad works in that safe plane on every neck case, which is the whole reason this nerve almost never gets touched.
Not often. Liposuction was the most-performed surgical procedure in the United States in 2024, up 1% from the year before, according to the American Society of Plastic Surgeons, and neck liposuction ranks among the most common facial procedures. A procedure done that often, with complications staying uncommon, tells you something real. In trained hands, this is a well-mapped operation.
Now the catch, and I won’t soften it. Those low numbers come from board-certified surgeons. Numbness and worse climb fast when someone undertrained is holding the cannula near your facial nerve.
Most early symptoms, like numbness, tingling, and a burning feeling, are normal nerve irritation, not damage. The signs that actually matter are the ones that don’t improve over time.
Right after surgery, the skin under your chin may feel numb, tight, or buzzy. Some people say the area feels asleep. That’s the nerve waking up, not failing. It’s the most common thing patients call about, and it’s almost always fine.
People hear “nerve damage” and picture a permanent droop. Actually, that framing isn’t quite right. The better way to think about it: small sensory nerves get jostled during any neck procedure, and they complain for a while before they calm back down.
Watch for a few specific things. A lower lip that pulls to one side when you smile. Weakness on one side of the mouth. Pain that gets worse instead of better after the second week. Numbness that hasn’t changed at all by three or four months. Any of those earns a same-week call to your surgeon.
Most numbness and tingling fade within a few weeks to a few months. Full sensation can take up to a year, because nerves regrow slowly.
Small sensory nerves regrow at roughly a millimeter a day. That’s why a numb patch can take months to come all the way back. It’s normal biology, not a red flag, and rushing it doesn’t help.
Age plays a role, and younger patients tend to bounce back quicker. Smoking is the big one I warn people about. It chokes blood flow to healing tissue and drags the whole timeline out. Diabetes and certain medications matter too. Your general health does more for nerve recovery than any cream or supplement on the shelf.
Mild nerve pain usually responds to time, gentle care, and basic over-the-counter pain relief. Pain that hangs around may need a doctor’s help.
Keep your head elevated the first few nights. Wear your compression garment exactly as told. Once your surgeon clears it, light and gentle massage can ease the tight, buzzy feeling while nerves settle. For most people the discomfort is mild and short-lived.
If pain pushes past the normal window, don’t tough it out. Treatment ranges from prescription nerve medication to a nerve block in stubborn cases. These situations are uncommon, but they respond well to care, and the sooner you flag them the better the outcome.
No. Permanent nerve damage from neck liposuction is rare, especially with a board-certified surgeon and solid aftercare.
Medical reviews of submental liposuction back this up, reporting that serious complications are rare even though they can be significant when they do occur. When permanent damage does happen, the reasons tend to repeat: aggressive technique too close to the jawline, an undertrained provider, or a patient with a condition that hurts healing. I’d rather you hear that plainly than read a page pretending the risk is zero. It isn’t zero. It’s small, and most of it sits in your hands through the surgeon you pick.
The single biggest factor in your nerve risk is who does the surgery. Technique and provider choice outweigh any device or gadget.
I’ll say it flat out. Don’t get neck liposuction at a med spa from someone who isn’t a surgeon. The neck is not the belly. The facial nerve sits right there under the skin. A board-certified surgeon trains for years to work in that space and knows exactly where not to go. This is the one area where saving a few dollars on the provider can cost you a crooked smile.
The “Ozempic makeover” wave has pushed a lot of people toward neck contouring after GLP-1 weight loss, and plastic surgery leaders have flagged a sharp jump in demand for neck procedures. More demand means more places offering it, and not all of them should.
Yes. A compression garment lowers swelling and supports the tissue, which gives irritated nerves a calmer place to heal. Less swelling also means a cleaner final contour.
The chin strap isn’t cute, and patients hate wearing it. Wear it anyway. Follow the schedule your surgeon hands you instead of guessing your way through recovery.
Follow-up appointments let your surgeon spot nerve issues while they’re still easy to address. We check sensation and movement at each visit and compare it to the last one.
Tell us about every odd symptom, even one you think sounds silly. The buzzy patch is usually nothing. An uneven smile is not, and we want to know about it fast. Catching a problem at week two beats finding it at month six every time.
Modern tools and smaller cannulas have made neck liposuction gentler on the nerves than it was a decade ago. Microcannulas remove fat with less trauma than the bigger tubes surgeons used years ago.
Some surgeons now pair suction with skin-tightening energy to treat laxity in the same visit, which cuts the odds of a loose-skin result that sends you back for more. The tech helps. It still doesn’t replace a surgeon who knows the anatomy cold, and any honest will tell you the same.
So, can neck liposuction cause nerve damage? Yes, but the useful answer goes further than that one word. The lasting kind is rare, the temporary kind is normal and fades, and the biggest lever you control is the surgeon you choose. Pick someone who works the neck week in and week out, and you’ve handled most of the risk before you ever lie down.

Ask directly how they protect the marginal mandibular nerve.
Yes. Neck liposuction is often paired with skin-tightening energy or a chin enhancement in the same visit. Combining procedures can improve the final contour and spare you a second recovery, but it should be planned around your anatomy and discussed at your consultation.
