Botox for smile lines softens the wrinkles that form around your mouth when you smile or laugh. It works by relaxing the small muscles that pull on the skin there, so the creases look smoother for a few months. Here’s the part most people don’t hear up front. It helps the lines made by movement, not the deep folds left behind by lost volume. For those, you’ll likely need something else.
So if you’re staring at the lines running from your nose to the corners of your mouth, hoping one round of injections will wipe them out, slow down. The honest answer depends on what kind of line you’re dealing with.
I’m not going to talk about pricing here, by the way. It swings wildly by city and injector and tells you almost nothing about whether the result will look good.
Smile lines are common, and treating them is more common still. Cosmetic injectables stayed the top nonsurgical option in the country, with neuromodulators like Botox topping 9.8 million procedures in 2024, up about 4% from the year before, per the American Society of Plastic Surgeons. People want a fast fix. Botox can be one. It just isn’t the whole fix.
Botox is a purified protein that briefly relaxes the muscle it’s injected into. Around the mouth, that calms the pulling that deepens smile lines, also called laugh lines or nasolabial folds. A clinician uses a fine needle to place tiny amounts in specific spots, and the muscle eases off over a few days. The effect is temporary, usually 3 to 6 months, and it works best on lines that show up when you smile rather than creases that sit there at rest.
These lines run from the sides of your nose to the corners of your mouth, and they deepen as collagen breaks down from sun, smoking, and age. When you’re young, they only appear when you smile. Over time, they can settle in and stay.
I’ll be blunt about one thing. People often ask for Botox around the mouth expecting it to plump the area. It doesn’t plump anything. It relaxes. Keep that distinction in your head and the rest of this makes sense.
Botox blocks the signal that tells a muscle to contract. Less contraction means less creasing on the skin above it. That’s the whole mechanism, and it’s why Botox shines on what we call dynamic wrinkles, the ones driven by movement.
Now the part that gets oversold. Deep smile lines are usually static. They sit there whether your face moves or not, because the cheek has lost fat and collagen and the skin has thinned. Botox can soften the muscle that tugs at the top of the fold, but it can’t refill what time took out. One published study found only about a third of patients saw a meaningful improvement in their smile lines from a toxin alone.
A third. That’s not nothing, but it’s not the eraser the ads imply.
Actually, let me reframe that. Calling Botox a “smile line treatment” oversells it. The better way to think about it here is as one lever, not the whole machine.
The FDA cleared Botox for frown lines, forehead lines, and crow’s feet. Using it lower on the face, around the mouth, is off-label. That’s common and done safely every day by skilled injectors, but it’s a real reason to care who holds the syringe. The approved uses keep growing, too: in 2024 the FDA cleared Botox for neck bands, a sign of how quickly the science moves.
The best candidate has fine smile lines that mostly show up with expression. If your lines vanish when your face relaxes, Botox has something to work with. If they’re carved deep at rest, set your expectations accordingly.
You’re probably not a good fit if you’re pregnant or nursing, or if you have certain neuromuscular conditions. A good injector will ask. They’ll also talk through whether Botox is safe for you given your history before anyone picks up a needle. For the record, women still account for about 94% of neuromodulator injections, per ASPS, though more men get them every year.
Here’s a take that won’t win me friends in the prevention-marketing crowd. Most people in their twenties don’t need this. Daily sunscreen and not smoking will do more for your smile lines over ten years than chasing early injections. Save your money until movement is actually the problem.
The visit is short. Most people are in and out inside half an hour.
No cutting, no stitches, no anesthesia beyond the topical kind. A skilled injector tailors the dose to your muscle strength, not a one-size template, which is what keeps your smile looking like yours. You can drive yourself home.
Botox around the mouth is low-risk in trained hands, but it’s not zero-risk. The mouth is a busy neighborhood of muscles you use to talk, eat, and smile, so placement has to be exact.
Common, short-lived side effects of Botox include redness, swelling, bruising, mild tenderness, or numbness at the injection spots. These usually fade within a week.
The bigger worry in this area is a lopsided or stiff smile if the toxin lands in the wrong muscle or drifts. That’s an injector-skill problem, not a Botox problem. In rare cases, people report trouble with speech or a heavy feeling around the mouth. Call your provider if anything odd lingers past a week, and seek help right away if you have trouble breathing or swallowing.
You can go back to most of your day immediately. A few simple rules protect your result.
You won’t see much on day one, and that’s normal. Botox needs a few days to start working, with the full effect around two weeks out. Bruising is the most common hiccup, and it hides easily under a little makeup the next day. Then the effect gradually wears off, which is why repeat visits every 3 to 6 months keep things smooth.
Before-and-after photos can mislead, so read them with a critical eye. Photos sell. I get it. But before you trust a gallery, look for the catch.
Are the lighting and angle the same in both shots? Is the “after” taken at peak effect or six months later? Were the smile lines dynamic or deep and static? A clinic showing fine-line softening on a movement wrinkle is telling the truth. A clinic claiming Botox erased a deep fold is either using filler too or stretching reality. And if a gallery shows only flawless deep-fold reversals, ask what else they injected, because Botox alone doesn’t do that.
Short answer: they do different jobs, and the best result often uses both. Botox relaxes muscles. Fillers add volume back to the hollow that lets the fold form.
For deep nasolabial folds driven by lost cheek volume, dermal fillers usually do more of the heavy lifting, and they last longer too. Cleveland Clinic notes filler results can hold six months to a year, versus the shorter window for a toxin. Many FDA-approved fillers use hyaluronic acid, the same substance your skin already makes.
I’ll tell patients straight. If your folds are deep, leading with Botox alone is the expensive way to be disappointed. Often the smart play is a little filler for volume, with Botox to calm the muscle pulling at the top. Picking a toxin for a volume problem is the most common mistake I see people make with this area.
Your provider’s skill matters more than the brand of toxin or any deal on price. This is where smile-line Botox lives or dies. The mouth punishes guesswork.
Pick someone who does facial injections constantly and works under real medical oversight, not a weekend pop-up. A board-certified surgeon in the room signals the training and accountability you want when the target controls your smile. Late in 2025, the FDA warned about counterfeit and unapproved Botox products moving through unlicensed sellers, which is one more reason to skip the bargain deal and ask exactly what’s going into your face. Ask how many treatments around the mouth they’ve done, and to see honest examples.
