In most of the United States, the answer is 18 for elective cosmetic procedures you choose on your own. Under 18, you can still have many surgeries with a parent or guardian’s written consent. A few procedures set the bar higher. Silicone breast implants, for example, get pushed all the way to 22.
But fifteen years in this work taught me something the legal number leaves out. The age requirement is the easy part. The harder question is whether your body and your head are ready, and those two things almost never line up neatly with a birthday.
The minimum age for plastic surgery is 18 for most elective cosmetic procedures done without a parent’s permission. Minors can have surgery with parental consent, though surgeons often wait until the body finishes growing. Reconstructive procedures, like repairing an injury or a birth difference, have no set age at all.
That’s the short version. The rest of this guide sorts it out by procedure, because a nose job and a tummy tuck live in completely different worlds.
The legal floor for elective plastic surgery is 18 in most states. Below that age, a parent or legal guardian has to sign off, and a handful of states raise specific procedures to 21.
There’s no single federal law banning cosmetic surgery for every minor. It’s a patchwork that shifts from state to state. What stays consistent is that any reputable surgeon will require parental consent for a patient under 18, and most won’t touch purely cosmetic requests from young teens at all. Working with one of Formation’s board-certified plastic surgeons matters far more than the number on your driver’s license.
The U.S. Food and Drug Administration layers device-specific rules on top of state law, which is why breast implants carry their own age minimums (more on that below).
Parental consent makes surgery legal for a minor. It does not override a surgeon’s judgment. A good surgeon can have a signed form in hand and still say no, because the deciding factor was never the paperwork. It’s whether the procedure fits that patient at that point in their life. I’ve turned away families who came in ready to sign, and I’d do it again. Saying no to a 16-year-old today is often the kindest thing you can do for the 25-year-old they’ll become.
Age is a starting point, not a finish line. Surgeons weigh three things more heavily than your birthday: physical maturity, emotional readiness, and the reason you want the surgery.
Plastic surgery is more common than ever. Surgeons performed nearly 1.6 million cosmetic procedures in 2024 alone. But popularity doesn’t bend biology, and biology is what decides safe timing.
You’ll read all over the internet that any age is fine as long as you’re healthy. That’s lazy advice. Some areas, like the breasts, keep changing into the early 20s. Operate too early and the result you loved at 18 might not be the result staring back at you at 25.
Motivation carries just as much weight. If large breasts cause back pain, or a deviated septum makes it hard to breathe, surgery solves a real problem. But a teen who wants an operation because of a comment online, or because a partner pushed for it, is usually better served by a conversation than a scalpel. That’s not me being preachy. It’s me having watched it go wrong.
There’s no universal age for plastic surgery, because each procedure runs on the body’s clock instead of the calendar. Some are safe in childhood. Others should wait until your 20s. The smart move is to match the procedure to your development, not to your impatience.
A nose can be reshaped once the face stops growing. For girls that’s usually around 14 or 15. For boys it lands closer to 16 or 17. Rhinoplasty is one of the few cosmetic procedures many surgeons will perform on older teens, as long as the bones are finished and the teen wants it for their own reasons. Our facial plastic surgery consultation always opens with that maturity check before we talk about anything else.
Ear surgery is the rare exception that’s safe for young children. By age 5 or 6, the ears have reached close to full adult size, so otoplasty can correct the shape or prominent ears early. Plenty of parents choose to do it before a child starts school to spare them years of teasing. It’s one of the few times “young” is actually the right time.
Breast reduction can be done between 15 and 18 when large breasts cause back pain, poor posture, or skin irritation. Because this surgery removes tissue instead of adding it, full development matters less here than it does with implants. If the breasts keep growing afterward, a second procedure may be needed later on.
Breast augmentation carries the strictest age rules of any common procedure. The FDA’s rules on saline and silicone implants set the cosmetic minimum at 18 for saline and 22 for silicone, while reconstruction is allowed at any age. Below those ages, implants are off-label.
I tell patients in their late teens to wait anyway. Breasts can keep shifting into the early 20s, and a future pregnancy or breastfeeding can change the result all over again. Rushing the timing is how people end up needing a revision they could have skipped.
Liposuction isn’t a weight-loss tool, and that’s the biggest reason it’s wrong for teens. It removes stubborn fat from a stable adult body. At Formation, liposuction candidates are usually adults at or near their goal weight whose shape won’t budge with diet and exercise. Doing it on a still-growing body, or as a shortcut around healthy habits, sets a patient up for disappointment. Your body keeps changing in your teens, and so does the result.
No. There’s no upper age limit for plastic surgery. Once you’re an adult, your health matters far more than the year you were born. A fit 65-year-old is often a better surgical candidate than an unhealthy 35-year-old, and I’ve operated on plenty of people who prove it.
But there’s a flip side nobody mentions. Waiting too long can limit the result you wanted. Skin-tightening and resurfacing work best before heavy sun damage and deep skin laxity settle in. The same logic applies to body contouring: starting from a firmer baseline gives a cleaner result. In Tampa, where the sun does real damage all year, I see this play out constantly. Patients who hold off until their 70s for skin work rarely get what they pictured, simply because there’s more to undo.
The worst mistake isn’t picking the wrong age. It’s picking the wrong provider.
Most rhinoplasties and breast augmentations are performed on adults between 18 and 34, a pattern backed by recent procedural statistics. So plenty of young people are perfectly good candidates. Their age was never the issue. Rushing to the wrong injector or a med spa is.
Going with an unqualified provider for a surgical procedure is how young patients end up with revisions, scarring, and a recovery that drags on far longer than the original surgery ever should have. Ask who’s actually holding the scalpel. Ask about board certification, and ask how many of your exact procedures they’ve performed. Vetting a qualified plastic surgeon up front spares you from fixing someone else’s mistake later.
Patients 19 and under make up only about 1 to 2 percent of surgical procedures. That’s a small group, and revision risk climbs fast when expectations are unrealistic or the body is still changing.

So how old do you have to be for plastic surgery? Eighteen for most elective procedures, with parental consent opening the door sooner for some and a couple of implant types waiting until 22. But the legal age is the floor, not the green light. The real answer depends on whether your body has finished growing and whether you want this for yourself. Get that part right, work with a surgeon who’ll tell you the truth, and the number on your ID becomes the least interesting part of the whole decision.

Not for loose skin. Weight-loss drugs can shrink fat, but they don't tighten the skin left behind. The American Society of Plastic Surgeons reported that more than 800,000 aesthetic patients used these medications in 2024, and many still pursued surgery afterward.
