Dermal fillers are injectable gels that restore lost volume in the face, smoothing folds, lifting cheeks, plumping lips, and softening hollow under-eyes. The procedure takes about 15 to 45 minutes in a clinic. Most patients walk out the same day. Done well, the results look like you got eight hours of sleep and three months of good skincare. Done badly, you end up on a Reddit horror thread or in surgery to reverse the damage.
I’ve worked alongside aesthetic injectors at Formation for years, and I’ll tell you straight. Most of what goes wrong with dermal fillers has nothing to do with the product. It has to do with who’s holding the syringe.
Dermal fillers are soft, gel-like substances injected under the skin to add volume, smooth lines, or contour the face. They’re not Botox. Botox relaxes muscles. Fillers fill space. Different problem, different fix.
The face has four structural layers: skin, fat, muscle, and bone. All four shrink and shift with age. The bone retracts, the fat pads slip down, the muscle thins, and the skin loses elasticity. Fillers replace the facial volume that gravity and time take away.
Most fillers used today contain hyaluronic acid, a sugar molecule your body already makes. It holds water, gives skin its plumpness, and breaks down on its own over six to 18 months depending on the formulation.
The big advantage of HA fillers is that they’re reversible. If something looks off, an enzyme called hyaluronidase can dissolve them within hours. Most HA fillers also come premixed with lidocaine, which takes most of the sting out of the injection.
A few other options exist for specific cases. Calcium hydroxylapatite is a thicker filler good for deeper folds and jawline work. It’s been used in dental and reconstructive surgery for decades. Poly-L-lactic acid works differently. Instead of adding instant volume, it stimulates your own collagen production over several months. Polymethyl methacrylate is a semi-permanent option I rarely recommend. It can form lumps that don’t dissolve, leaving surgical revision as the only fix.
Fat transfer using your own fat is another option, though it’s a separate procedure with a longer recovery.
The most effective dermal filler treatment areas are the cheeks, jawline, lips, tear troughs, and nasolabial folds. Each spot responds to different filler thicknesses and injection techniques.
Cheeks usually need a thicker HA or calcium hydroxylapatite to create lift. The right cheek work can soften everything below it, including smile lines and jowls, because you’re restoring the scaffolding that’s collapsed downward.
Jawline filler builds structure. Men ask for this constantly now. A defined jaw doesn’t have to come from surgery. A few well-placed syringes of a firmer product can sharpen the angle without a scalpel. The demand for these non-invasive procedures has roughly doubled in two years.
Lips are tricky because the line between subtle enhancement and “did something” is narrow. I tell patients to start with half a syringe. You can always add more. Pulling it out cleanly is harder.
Tear troughs (the hollow under your eyes) are the highest-risk area for an inexperienced injector. The vessels run close, the skin is thin, and the wrong product or technique can leave a permanent blue-gray tint called the Tyndall effect.
Non-surgical rhinoplasty using filler in the nose carries the highest vascular risk of any cosmetic injection. I rarely recommend it outside very specific cases.
This is where I get loud. The FDA has explicitly warned consumers to never buy dermal fillers online. People still do it. They watch a YouTube tutorial, order vials from an overseas site, and inject themselves at home.
What they’re often injecting isn’t filler. It’s silicone, hair gel, mineral oil, or industrial substances repackaged in convincing boxes. I’ve seen the aftermath at Formation more than once. Skin necrosis. Permanent lumps. Disfigurement that took multiple surgeries to partially correct.
The other side of this danger is vascular occlusion. Filler injected into or pressing on an artery can block blood flow to the skin, the eye, or even the brain. Vision loss from improperly placed filler is rare but irreversible. A trained injector knows the danger zones around the eyes, nose, and temples. A self-injector or undertrained provider doesn’t.
A 2024 peer-reviewed analysis of 8,795 reported filler complications found only two severe events in the cohort. That’s a strong safety profile in skilled hands. The same review noted nearly all serious adverse events came from inexperienced injectors or non-medical settings.
Start with credentials. A board-certified surgeon should be on your shortlist. Then look at volume. How many of these specific cases has the person done? Jawline filler isn’t lip filler and isn’t tear trough filler. Each one needs its own muscle memory.

That contrarian stance, less is more, runs against the industry push toward bigger lips and more dramatic before-and-afters. It’s also why patients don’t end up with “pillow face” or migration problems down the line.
ASPS reported more than 5.3 million HA filler procedures in the US in 2024. The volume keeps climbing, and so do the corrections. Roughly 47% of revision cases I see started at a med spa with an inexperienced injector.
Before you let anyone inject your face, ask these:
If the answers feel vague, walk out. Tampa has dozens of injectors competing for your business. The cheap option is almost always the expensive option once corrections come due.
Dermal filler recovery is mild for most patients. You can return to normal activities the same day, though some swelling, redness, or bruising can last anywhere from 24 hours to a week.
The first 24 to 72 hours show the most swelling. This is when most patients panic. Don’t. The face is going to look slightly puffy, possibly uneven, and lumpier than the final result. That’s normal. Filler needs time to settle into the tissue.
Bruising depends on the injection site, your individual vascular pattern, and whether you took blood thinners or fish oil recently. The under-eyes bruise easiest. The cheeks rarely do. Mainstream medical aftercare guidance lines up with what we tell patients at Formation: ice, gentle care, and patience.
Full results usually settle by week two. Some patients book a touch-up appointment around the four to six week mark if a small area needs adjustment.
The biggest mistake patients make is panicking about asymmetry at day three. Trust the two-week timeline. If something still feels wrong at that point, call your provider. Don’t book a second appointment to “fix” what was always going to settle on its own.
Dermal fillers are some of the safer aesthetic procedures available right now. The product isn’t the problem. The hands are.
Hyaluronic acid dermal fillers last anywhere from six to 18 months depending on the product, the treatment area, and your metabolism. Lip filler tends to break down faster than cheek or jawline filler because the muscles in that area move constantly. Calcium hydroxylapatite can last 12 to 18 months, and poly-L-lactic acid results can stretch past two years.
The injection feels like a quick pinch or sting. Most HA fillers come premixed with lidocaine, a numbing agent, so the area gets numb within seconds of the first injection. A topical numbing cream applied 20 minutes before treatment cuts discomfort further. Most patients rate the pain a three or four out of 10.
In trained hands, dermal fillers are among the safer aesthetic procedures available. A 2024 review of 8,795 reported complications found only two severe events. Almost all serious problems trace back to inexperienced injectors, non-medical settings, or counterfeit products purchased online. Choosing a board-certified surgeon brings risk close to negligible.
HA dermal fillers can be reversed using an enzyme called hyaluronidase that dissolves the filler within hours. Non-HA fillers like calcium hydroxylapatite and polymethyl methacrylate can't be dissolved the same way and may need surgical revision if results go wrong. This is one reason most experienced injectors stick with HA products.
People with active skin infections, autoimmune conditions affecting healing, known severe allergies to filler components, or those who are pregnant or breastfeeding should avoid dermal fillers. A board-certified surgeon will screen for these during consultation. Anyone on blood thinners may also need a temporary pause before treatment to reduce bruising.
Botox is a neuromodulator that relaxes muscles to soften wrinkles caused by movement, like forehead lines or crow's feet. Dermal fillers are volumizing gels that fill space under the skin to smooth folds and restore lost contour. Many patients use both for different concerns in the same session.
You'll see immediate volume change right after the injection, but swelling can mask the true result for the first 24 to 72 hours. Full settled results usually appear around two weeks post-treatment. Poly-L-lactic acid is the exception. Its results build gradually over two to four months as your body produces new collagen.
