Here’s where I’ll push back on what you’ve probably read elsewhere. A lot of clinics still quote “up to 2 years” or “2-plus years” for PDO thread lift longevity. In our experience at Formation, that’s marketing math, not real-world math. I’ve seen the receipts. The mechanical lift starts softening earlier than patients expect, and the collagen-driven firmness is what actually carries them past the 12-month mark.
This article won’t cover surgical facelifts in depth, and it won’t compare every thread brand on the market. What it will do is give you a straight read on how long results last, what shortens them, and when a touch-up actually makes sense.

Smooth threads work mainly for texture and collagen response, while barbed and cog threads are what create the visible lift. You’ll typically see noticeable lift from day one, with peak tightening around month 6. From there, results gradually soften through months 12 to 18. By month 18 to 24, most patients are back to where they started, sometimes with slightly better collagen baseline.
This isn’t a cliff. It’s a slow fade. You won’t wake up one morning looking different. You’ll catch it slowly in photos, in the mirror at certain angles, on a video call where you notice your jawline isn’t holding the way it was.
The honest framing: patient-reported satisfaction data shows happiness is often high at one week and continues climbing through month 6, then begins to decline as the months pass. That curve is real. Wear sunscreen every day. SPF 30 broad-spectrum, applied in the morning, reapplied if you’re outside for long stretches. No exceptions.
Sleep on your back for the first two weeks. Side sleeping creates pressure that can shift threads before they settle. After two weeks, the threads are anchored and you can go back to your normal position.
Stay hydrated and eat for collagen. Vitamin C, zinc, lean protein, and dark leafy greens give your body the raw materials. This isn’t a wellness pitch, it’s basic chemistry. Collagen needs amino acids and vitamin C to form. Patient outcomes consistently correlate with hydration and protein intake during the collagen-building window.
Skip saunas, hot yoga, and intense workouts for the first week. Heat and sweat in those early days can affect how the threads anchor.
Schedule a check-in at month 6. We’re not selling you another procedure at that point. We’re looking at how the collagen is settling and whether your face is on track. If it’s not, smaller adjustments now save bigger spend later.
Most patients fall into one of two patterns. The maintenance crowd comes in around month 12 for a small number of threads to refresh specific areas, usually the jawline or marionette lines. The full refresh crowd waits until month 18 to 24 and gets a complete redo.

The bigger risk isn’t the threads. It’s the provider. Thread lifts are technique-dependent in a way that injectables aren’t. Placement depth, vector planning, and anchor points all change the outcome. Bad placement causes asymmetry, visible threads, and dimpling that lasts longer than the lift. Good placement gives you a clean result that ages well.
