There’s no age where you’re automatically disqualified from plastic surgery. The question of what age you should stop having plastic surgery comes down to one thing: your health, not the number on your driver’s license. Patients in their 70s and even 80s safely undergo cosmetic procedures every year, with complication rates comparable to younger patients in most categories. The only procedure where researchers have found a meaningful difference is abdominoplasty (tummy tuck), where older patients show slightly higher complication rates.
That’s the data-backed answer. But the real conversation is more specific than that, and most of what gets repeated online skips the details that actually matter.
At Formation in Tampa, Dr. Gopal Grandhige evaluates candidacy based on cardiac clearance, healing capacity, and realistic goals. He’s turned away 35-year-olds who weren’t ready and operated on patients well past 65 who healed beautifully. Age is a data point, not a verdict.
This article won’t cover reconstructive surgery, insurance-covered procedures, or non-surgical treatments in depth. We’re focused on elective cosmetic surgery, the age question, and what the 2024–2025 clinical data actually shows.
No. Board-certified surgeons evaluate candidacy based on physiological health, not chronological age.
The American Academy of Dermatology reviewed data from over 129,000 cosmetic surgery patients and found that patients 65 and older had roughly the same serious complication rates as younger patients. The one exception was tummy tucks. And here’s the part that surprised the researchers: the 65+ group was actually in better overall health than the younger group. They were less likely to smoke and more likely to be at a healthy weight.
That tells you something important. Surgeons are already screening older patients more carefully. The ones who make it to the operating table at 70 tend to be healthier than the average 40-year-old walking through the door.
I’ve seen this firsthand across dozens of consultations. The patients who do poorly aren’t old. They’re unprepared. They smoke, they skip pre-op clearances, they choose providers based on price instead of credentials. Age has very little to do with it.
What surgeons actually look at includes cardiac health and blood pressure, lung capacity (non-smokers heal significantly better), protein levels and healing markers from bloodwork, general activity levels and muscle mass, and mental readiness with a realistic support system for recovery.
Every decade has a different sweet spot. The body changes, and the procedures that make sense shift with it.
Teens and early 20s. Most surgeons won’t perform elective cosmetic surgery until the body reaches full adult size, which happens around 15–18 for males and 14–18 for females. The exception is rhinoplasty, which is the most common procedure in this age group. Male breast reduction (gynecomastia surgery) is also common for teens and younger men.
Mid-20s to early 30s. Rhinoplasty is still popular here, and it’s the ideal window for it. You want facial bones that have stopped growing but skin that’s still firm, which rules out doing it much past 50 for optimal results. Breast augmentation also peaks in this range. Patients 22 and older can choose between silicone and saline, while those under 22 are limited to saline.
30s to early 40s. Neuromodulators (think Botox) dominate this bracket, with roughly 9.88 million procedures performed in 2024 alone according to the ASPS annual statistics report. Liposuction and breast augmentation are close behind. Women done with having children often look into a mommy makeover, which typically combines a tummy tuck, breast lift, and liposuction.
The industry pushes mommy makeovers hard, but I think a lot of women would be better served doing procedures in stages rather than all at once. A combined surgery means longer time under anesthesia, a harder recovery, and more variables for complications. If your budget and schedule allow it, staging them 6–12 months apart often produces better outcomes with less risk.
40s to early 50s. This is where anti-aging procedures pick up. Eyelid surgery (blepharoplasty) and facelifts become the most requested procedures. According to the ASPS, patients aged 40–54 represent the largest share of all cosmetic surgical procedures, followed closely by the 55–69 bracket. ASPS data from 2024 shows women account for about 92% of surgical cosmetic procedures, though male procedures are growing.
55 and over. Facelifts are the standout here. Baby Boomers accounted for 59% of all facelifts performed in 2024 and 61% of skin resurfacing treatments. But most articles won’t tell you this: the timing of your facelift matters more than your age. If you wait until your skin has lost significant elasticity and volume, the results will look “done.” Getting a facelift earlier (mid-40s to early 50s) while your tissue still has some bounce creates a refreshed look that’s much harder to detect. Wait too long and you’re fighting gravity with less to work with.
The honest answer is that there’s no universal cutoff. But there are red flags.
You should seriously reconsider any elective procedure if you can’t get cardiac or pulmonary clearance from your internist, if you’re a smoker who won’t quit (smoking wrecks healing and doubles complication risk), if you have poorly controlled diabetes or autoimmune conditions, or if you’re on blood thinners that can’t be safely paused. An experienced surgeon will evaluate all of this before agreeing to operate. If a provider doesn’t ask for medical clearance from your primary care doctor, especially past age 55, that’s a warning sign about their standards.
Actually, the better way to frame this question isn’t “when should I stop?” It’s “am I healthy enough right now?” A healthy 72-year-old who exercises, doesn’t smoke, and has normal bloodwork is a better candidate than a sedentary 45-year-old with uncontrolled hypertension. Age is a proxy for health, and it’s a lazy one.
The rise of GLP-1 medications like semaglutide has changed the plastic surgery conversation. Rapid weight loss from these drugs is creating a new wave of patients (many of them over 50) who now have excess skin they want removed.
The ASPS 2024 report specifically noted increased demand for body contouring and skin removal procedures linked to GLP-1 weight loss. If you’ve lost 50+ pounds on a GLP-1 medication, you’re going to have loose skin that no amount of exercise will tighten. That’s not marketing. That’s biology.
The timing question here is important. Most surgeons want you weight-stable for at least 6 months before doing body contouring. If you’re still losing, the results won’t hold.
The bigger risk in plastic surgery isn’t being too old. It’s picking the wrong provider. Choosing an unqualified provider (like a med spa doing surgical procedures) can lead to corrections that add months of recovery on top of the original downtime. I’ve seen this play out repeatedly. The cheapest option is almost never the smartest one.
Northeast and West Coast markets tend to run significantly above national averages for surgeon fees. Tampa falls closer to the national median, which is one reason patients travel here. But don’t pick a surgeon based on geography or price alone. Look for board certification through the American Board of Plastic Surgery, facility accreditation (AAAHC or similar), and a willingness to require full medical clearance before booking your procedure.
The question of what age you should stop having plastic surgery doesn’t have a number attached to it. The 2024 ASPS data is clear: roughly 1.586 million cosmetic surgical procedures were performed by ASPS members last year, across every age group. The fastest-growing demographic for facelifts is over 55.
If you’re in good health, you’ve done your research, and you’re working with a board-certified surgeon in Tampa who requires proper medical clearance, your age is not the barrier. Your health is the conversation that matters.
Yes, if you're medically cleared. A study of over 129,000 patients found that adults 65 and older had complication rates similar to younger patients for most cosmetic procedures. The single exception was abdominoplasty, which carried higher risk in older age groups. Cardiac and pulmonary clearance from your internist is the first step.
It can. Older skin has less collagen and elasticity, which means results from procedures like facelifts may not hold as long as they would on a 45-year-old. Lifestyle factors (sun exposure, smoking, nutrition) play a bigger role than age alone. A facelift on a healthy 50-year-old can last 7–10 years.
Most surgeons recommend getting a facelift between the mid-40s and early 50s, while skin still has enough elasticity to create natural-looking results. According to the ASPS 2024 data, patients aged 55 and over accounted for 59% of all facelifts performed. Getting one earlier doesn't mean you need one. It means the results are subtler.
Significantly. The ASPS 2024 report noted rising demand for skin tightening and body contouring procedures driven by rapid GLP-1-related weight loss. Patients who lose 50+ pounds often develop excess skin that exercise alone won't fix, creating a new group of candidates for procedures like tummy tucks, arm lifts, and lower body lifts.
Ask about their medical clearance protocol for older patients. Ask for their complication rates specifically for your age group. Ask to see before-and-after photos of patients over 60. And ask whether they operate in an accredited surgical facility (look for AAAHC or similar accreditation). A surgeon who can't answer these questions directly isn't the right fit.
It depends on your goals. Non-surgical options like neuromodulators and dermal fillers carry lower risk and shorter recovery times, but results are temporary (typically 6–18 months). Surgical procedures like facelifts offer more dramatic, longer-lasting results (5–10+ years). For patients concerned about anesthesia risk, non-surgical treatments are a reasonable middle ground.
The power of a profile transformation. 🔥
For this patient, we combined PDO Lifting Threads and advanced chin/jawline filler to completely redefine his lower face. By strategically lifting the mid-to-lower face and sculpting the jaw, we didn’t just enhance one feature—we elevated his entire facial appearance.
An incredible, natural-looking result that brings balance and confidence from every angle.
Note: Minor bunching or puckering is completely normal immediately following a PDO thread lift. The slight bunching visible in the after photo will fully subside on its own over the next 2-4 weeks as the threads settle.
