Botox, formally known as onabotulinumtoxinA, is a refined version of botulinum toxin, a neurotoxic protein that originates from the bacterium Clostridium botulinum. It functions by temporarily calming muscles through the suppression of acetylcholine, which is the neurotransmitter that signals muscles to contract. By interrupting this process, Botox effectively minimizes unwanted muscle movement in both medical and cosmetic applications. The FDA has approved Botox for a range of uses, including the treatment of facial wrinkles, chronic migraines, muscle spasms, excessive sweating, overactive bladder, and incontinence. Even with its widespread popularity, questions about its safety for breastfeeding mothers persist because of the limited research available.
The side effects associated with injectable treatments depend on where the product is injected and the dosage used. The most frequently reported reactions include headaches, minor drooping of the eyelid, or localized swelling near the injection site. Importantly, studies demonstrate that Botox tends to stay at the site of injection and shows very little absorption into the broader system.
On rare occasions, Botox may migrate beyond the targeted area, potentially causing more significant symptoms such as breathing difficulties or trouble swallowing. These more serious reactions, sometimes described as Botox “poisoning,” are generally associated with incorrect administration or the use of counterfeit or contaminated products rather than properly dosed, standard treatments. This is one reason why choosing an accredited facility with experienced providers is so important.
Botox originates from botulinum toxin, a powerful neurotoxin created by the bacterium Clostridium botulinum. In its unprocessed natural state, this toxin is responsible for botulism, a severe illness that can cause gastrointestinal problems, muscle weakness, and in the worst cases, paralysis. The bacterium survives in the environment as dormant spores, which can activate and produce the active toxin under certain conditions.
However, the botulinum toxin used in cosmetic and medical Botox treatments is a purified, inactive form that is delivered in very small, carefully controlled amounts. It does not contain any live bacteria or spores. When administered correctly, Botox stays localized at the injection site with minimal spread throughout the body, and the chance of severe side effects in adults is extremely low. Leading institutions like Johns Hopkins Medicine emphasize the importance of understanding this distinction when evaluating treatment safety.
By contrast, infant botulism happens when spores are swallowed and then grow in a baby’s underdeveloped digestive system, producing toxin from within. A well known source of these spores is honey, which is why healthcare professionals advise against giving honey to babies younger than 12 months.
Despite sharing a common origin with botulism, Botox presents a much lower level of risk. It is not expected to enter breast milk, and the product itself contains no live bacteria or spores. Reinforcing this point, two documented cases involving breastfeeding mothers who suffered severe botulism revealed no detectable botulinum toxin in their breast milk, and their infants showed no signs of illness. These findings strongly support the conclusion that maternal Botox use carries an extremely low risk to nursing babies.
A 2024 study by Hudson, Wilson, Lieberman, Mittelman, and Parikh researchers affiliated with institutions including the University of Michigan Medical School examined breast milk samples from four women who received between 40 and 92 units of facial Botox through intramuscular injection. Two of the four participants showed no detectable Botox in their milk at all. The remaining two participants had Botox concentrations in their milk ranging from 85 to 747 pg/mL, measured using an ELISA assay. Notably, these levels did not show any consistent relationship with the maternal dose or the amount of time that had passed since the injection.
A 2025 study by Gu, Xu, Koviazina, Tan, Zheng, Kappes, Kotsifaki, Shen, and Tsigkou analyzed breast milk from three women who each received 64 units of facial Botox. The highest concentration of Botox found in milk was 167 pg/mL, again measured by ELISA assay. Similar to the earlier study, no clear pattern emerged between the maternal dose or timing and the levels detected in breast milk. Additional testing methods were also used. Mass spectrometry found no Botox, while other techniques such as Raman spectrometry and western blots detected trace amounts of Botox over a prolonged period.
The amounts of botulinum toxin involved in these injections are remarkably small. All milk concentrations measured fell below 1 ng per mL, which equals one part per billion. To offer some context, the estimated lethal intravenous dose of botulinum toxin for a 10 kg baby would be approximately 10 to 20 ng, and even at that level, only about 50% mortality would be expected. Using the maximum maternal dose from these studies, even if the entire amount were hypothetically injected directly into a baby’s bloodstream, it would likely not be fatal.
There is also a critical distinction between direct injection and oral consumption. Stomach acid breaks down the toxin, preventing it from reaching the infant’s bloodstream. Purified botulinum toxin appears to have an especially hard time entering the bloodstream when consumed orally. In cases of foodborne botulism, additional proteins are required to shield the toxin from stomach acid and enable systemic infection.
Researchers do acknowledge difficulty interpreting some of the findings. Based on reported concentration ranges, calculations suggest an infant could theoretically receive more Botox per day through milk than the mother received in her injection, which is contradictory. The most likely explanation is that the ELISA assays used to measure Botox in milk may have produced inaccurate readings, as this type of testing is notoriously difficult in breast milk. While the multiple methods used in the 2025 study do suggest that at least some Botox may persist in milk for an extended period, the exact quantities remain uncertain.

Based on the evidence currently available, there is no recommendation to pump and discard breast milk after receiving Botox injections. However, it is always wise to stay alert to any changes in your baby. While not expected, symptoms such as unusual drowsiness, droopy eyelids, or difficulty swallowing in your infant should be taken seriously and prompt immediate medical attention. These include abobotulinumtoxinA (Dysport), incobotulinumtoxinA (Xeomin), prabotulinumtoxinA (Jeuveau), daxibotulinumtoxinA (Daxxify), and rimabotulinumtoxinB (Myobloc). These products differ in potency, dosing schedules, and specific clinical uses, but they all work through the same mechanism of blocking acetylcholine release at the neuromuscular junction to produce muscle relaxation. It is important to note that none of these alternatives have been formally studied during breastfeeding.
