A tummy tuck removes the loose skin that pregnancy or weight loss left behind and closes the gap between separated abdominal muscles. Diet and training can’t fix either one. At Formation, Dr. Gopal Grandhige plans and performs every tummy tuck himself at the Hyde Park practice, about 17 miles from Oldsmar, and follows you for a full year afterward.
Abdominal skin and muscle both have a stretch limit, and pregnancy or a large weight change can push past it. Once skin is stretched beyond the point where it recoils, it stays loose. Training, creams and devices that work from the surface don’t restore it.
The muscle problem is a gap. The two vertical bands of the rectus muscle pull apart along the midline, a condition called diastasis recti. A wall with a gap in it pushes forward when you stand, cough or lift, however strong it gets. Crunches build strength and leave the bulge.
During a tummy tuck, excess skin is removed, the separated muscle is sutured closed and the remaining tissue is reshaped. Liposuction can’t do either part on its own, because it removes fat and leaves skin and muscle where they were. Many patients stand straighter and move more comfortably once the core is closed. The full surgical detail is on Formation’s abdominoplasty in Tampa page.
It’s a common operation. The 2025 national procedure statistics ranked the tummy tuck third among cosmetic surgeries, with 173,251 performed, and surgeons named restoration after pregnancy, aging and weight loss as the leading reason patients came in.
Dr. Grandhige runs three checks at every tummy tuck consultation. Together they decide whether your abdomen needs skin removed, muscle repaired or neither.
He lifts a fold of skin below your navel and lets go. Skin with tone moves back toward the body right away. Skin that settles slowly or holds a crease has lost its recoil.
With you standing relaxed, he looks for skin hanging over the lower belly. A fold that hangs while you’re upright has to come off. A roll that only shows when you sit is usually fat under skin that still holds its shape.
When you lie back and lift your head, a ridge or dome down the midline signals separated muscle. So does a soft gap above or below the navel where fingertips sink in.
Clear all three and a tuck is probably more surgery than you need. Liposuction can address the fat, and energy-based tightening such as Renuvion, a helium plasma energy device, is a reasonable finisher for skin that’s only mildly loose.
Fail the fold check or the wall check and you’re on the remove side of the line, because a device can’t cut away a fold or close a gap. Mixed results get decided in the exam room. The choice there runs between a full tuck, a mini-abdominoplasty (offered in select cases) and liposuction with tightening.
Major complications after a tummy tuck are uncommon. A 2024 Aesthetic Surgery Journal analysis of 55,596 abdominoplasty patients treated from 2015 to 2022 put the overall rate at 2.1 percent. The fleur-de-lis version carried the highest rate of the types studied. That version adds a vertical incision to the usual low one.
The same analysis found that once other risk factors were accounted for, adding cosmetic procedures to the tuck didn’t raise the risk of a major complication. An older dataset from the same insurer pointed the other way. A review of 25,478 tummy tucks from 2008 to 2013 reported a 4 percent rate and found that combining procedures pushed it higher. It also flagged male sex, age 55 and over, and a BMI of 30 or more as added risks.
Both numbers have limits. Each study drew on an insurance database that records only major complications a surgeon files as a claim. Smaller problems handled in the office, such as a fluid collection that needs draining, aren’t captured by either figure. A national average also can’t tell you your own risk.
Formation’s screening is built to answer that question. It covers your medical history, recent labs and a risk review before a date is set. Surgery takes place in a Quad A accredited facility, with the same anesthesia provider and the same team on every case. That team includes a physician assistant Dr. Grandhige employs directly. The team works with him on every case and already knows his method, and that consistency is a safety measure.
Liposuction that refines the flanks and transitions around your tuck often goes into the same operation. Larger contouring elsewhere may move to a second date, so one recovery doesn’t stack on another. That’s the Formation Staging Line, and the page on abdominoplasty with liposuction explains how the two are combined.
Florida sets limits of its own. Under the state’s office surgery rule, liposuction combined with a tummy tuck in a physician’s office can’t exceed 1,000 cc of supernatant fat. That compares with a 4,000 cc ceiling for liposuction alone in the same setting, and cosmetic surgery patients must leave within 24 hours of arriving. The rule covers surgery performed outside licensed hospitals and surgery centers.
Supernatant fat means the fat measured after the injected fluid settles out. Where your operation takes place, and how much contouring fits into it, is settled in your plan before a date is booked.
Staging covers the calendar too. If your weight is still moving, the whole plan waits until it settles.
You don’t have to make the drive to find out whether you’re a candidate. Formation’s complimentary virtual evaluation with patient coordinator Hanna reviews your goals and outlines a plan before you decide to come in.
The in-office consultation with Dr. Grandhige carries a fee and is a full surgical assessment. He examines your anatomy, asks what you’ve already tried and tells you plainly what the operation will and won’t change. The fee can be applied toward any procedure or product at the practice. Sometimes the right call is to wait, address a health issue first or do something smaller, and you’ll hear that in the room.
Oldsmar is close enough to recover in your own home, so the household plan matters as much as the medical one. Line up help for at least the first week, and longer for heavy lifting, small children included.
Short, frequent walks are part of healing, and Oldsmar has the paths for it. The Oldsmar Trail links city parks over more than 10 miles, centered on R.E. Olds Park, and most of it is concrete. Early walks belong on the paved stretches near the park, which has free parking and restrooms. The unpaved sections through Mobbly Bayou Wilderness Preserve can wait until Dr. Grandhige clears you for longer, uneven walks.
Plan your date around the bay. Hurricane season runs June 1 through November 30, and a storm can knock out power for days. That’s the hardest setting for a first week of round-the-clock compression and limited movement. If you book in late summer or fall, check your Pinellas County evacuation zone and arrange a backup place to recover. A date during the Pinellas County Schools winter break often lines up family help with the coolest stretch of the year, when compression is easiest to tolerate.
Swimming and Mobbly Beach Park wait until your incision is cleared. After that, keep the scar out of direct sun while it matures.
Dr. Gopal Grandhige is a board-certified general surgeon, certified by the American Board of Surgery, who specializes in aesthetic surgery. He earned his medical degree at the University of Michigan and matched into Yale’s six-year combined plastic surgery residency. He then changed course to complete a full general surgery residency, followed by fellowships in burn surgery, critical care and minimally invasive metabolic and foregut surgery.
That path shows up on the operating table. More than 15 years and several thousand operations, including major abdominal cases, inform how he rebuilds an abdominal wall rather than tightening its surface. He limits his surgical volume so he can stay involved before, during and after each case. He performs advanced and revision tummy tucks himself instead of referring them out. His full training record is on his profile.
Formation’s 60/40 Recovery Standard holds that the operation is 60 percent of your result and aftercare is the other 40. Careless aftercare can spoil a well-performed surgery. So recovery runs on scheduled visits and lymphatic treatment from providers Dr. Grandhige has vetted, not a discharge handout.
A nurse is with you for the first 24 hours. The first week is rest, short walks and pain control, with someone helping at home. If you’d rather skip the early drives, Formation helps arrange a stay at a nearby hotel where it has relationships. Dr. Grandhige visits patients there so they don’t have to come to the office.
In weeks two and three, light activity returns, compression stays important and lymphatic treatments are on the schedule. Most patients with desk jobs are back at work after 10 to 14 days. Swelling improves through weeks four to six while lymphatic care continues. If a fluid collection forms, it’s drained in the office. Unrestricted exercise returns at six to eight weeks, and your shape keeps refining for three to six months.
Follow-up runs a full year, and nearly all patients stay with it. One patient came in after her pregnancies with her daughters beside her, joking that they should pay for it since they caused it. After her tuck and muscle repair, she reported better posture and less back discomfort. She now dresses up for visits she once made in loose sweatpants. More outcomes are in the tummy tuck before-and-after gallery.
The right time for a tummy tuck is after your last pregnancy, once your weight has held steady.
If another baby is possible, wait, because a later pregnancy stretches the repaired muscle and remaining skin again. The same applies to weight that’s still dropping, including on a GLP-1 medication. In 2025, 82 percent of American Society of Plastic Surgeons member surgeons reported consultation requests tied to GLP-1 use. The timing rule is the same for those patients: skin that hasn’t settled keeps changing after surgery.
Diabetes and high blood pressure don’t rule you out, but they’re brought under control first. Smoking stops well ahead of your date, since it cuts the oxygen supply that healing tissue needs.
Preparation is where Formation’s care differs. Your health and diet are assessed before surgery. Nutrition, protein, supplements and peptides are then adjusted in the weeks just before and after it, so your body has what it needs to rebuild tissue. That’s Formation’s philosophy in practice. The operation is the same one every surgeon learns. The thinking around it isn’t.
Three limits are worth knowing up front. The scar is permanent and sits low across the abdomen. Swelling outlasts most people’s expectations, so your final shape takes three to six months to show. And over-resection creates tight, artificial contours that age poorly, which is why Dr. Grandhige leans toward restraint.
Formation is on South Howard Avenue in Hyde Park, about 17 miles from Oldsmar and roughly half an hour away outside rush hour.
The usual route follows Tampa Road east, which becomes Hillsborough Avenue at the county line, then joins the Veterans Expressway heading south toward the airport. From there, pick up I-275 toward downtown and exit at Howard Avenue. Leave extra time at peak hours.
You won’t be driving at first. Arrange a driver for at least the first week and for as long as you’re taking prescription pain medication. Expect several trips in the first month for checks and lymphatic treatment, and book them mid-morning or early afternoon where you can.
Does fat move after lipo?
It’s a common concern, especially when someone notices weight gain in an area that wasn’t treated.
Here’s what actually happens.
The fat cells removed during liposuction are gone permanently. But if someone gains weight afterward, the remaining fat cells can still grow, including in areas that weren’t treated.
The fat didn’t move. The body gained weight.
That’s why Dr. Gopal emphasizes being at a weight patients can realistically maintain before surgery. Not the lowest weight they can reach, but one they can sustain long after recovery.

Want a more sculpted body?
High-definition liposuction takes body contouring a step further. Instead of simply removing excess fat, it focuses on sculpting around the body’s natural muscle structure to create more visible definition and a more athletic contour.
Dr. Gopal Grandhige specializes in HD liposuction, customizing each procedure based on the patient’s anatomy, body shape, and desired level of definition.
Because the goal isn’t to give everyone the same body. It’s to create a result that works with their natural anatomy.

Don’t plan lipo 2 months before vacation.
If the goal is to enjoy a more sculpted, defined body next summer, the planning needs to start months before that.
Fall is one of our busiest seasons at Formation, and there’s a reason for it. Many patients are already thinking about how they want to look and feel next summer.
But getting there takes time.
At Formation, Dr. Gopal requires at least four weeks of preparation before surgery. That time is spent preparing patients physically and mentally for the procedure and recovery.
After surgery, swelling needs to go down, the skin needs to contract, and the tissues need time to settle into their new contour.
It can take 6–9 months for results to look and feel natural.
That’s why Dr. Gopal recommends planning liposuction 6–12 months ahead, especially when there’s a specific season or vacation in mind.
If next summer is the goal, now is the time to start the conversation.

How defined would you go?
Not everyone wants the same look from HD liposuction. Some prefer a smooth, subtle contour, while others want their abs to stand out.
Here’s how the three levels of definition differ:
01. Low definition: A leaner, natural-looking abdomen with subtle definition and light contouring around the underlying muscles.
02. Medium definition: More visible separation between the abdominal muscles, with added definition through the abs and obliques without a sharply etched six-pack.
03. High definition: More pronounced sculpting around the abs and obliques to highlight the underlying muscle structure and create a more chiseled, athletic look.
Dr. Gopal tailors his HD liposuction technique to the level of definition you’re looking for, while considering what’s achievable for your body.
Which would you choose: 1, 2 or 3? Tell us in the comments!

This isn’t traditional lipo. It’s HD lipo.
For this patient, getting smaller isn’t the goal.
He already works out. He already has the muscle. Dr. Gopal’s job is to use HD liposuction to bring out more of that existing definition and shape the areas around it so the body looks more athletic and defined.
That’s why the markings matter. Before surgery even begins, Dr. Gopal is mapping out how the abs, chest, arms and surrounding areas should work together in the final result.

This is the part of the job that never gets old.
There’s something special about knowing how different a patient’s life might feel on the other side.
Not just because their body may look different, but because of everything that can come with it.
Feeling more comfortable in their own skin.
Saying yes to plans without thinking twice.
Looking forward to summer instead of dreading it.
Plastic surgery can change a body. But sometimes, what matters most is how differently someone gets to experience their life afterward.
And that’s a pretty incredible part of what we get to do at Formation Tampa.

Three small incisions instead of one long one.
For some patients with diastasis recti, repairing the separated abdominal muscles doesn’t necessarily require a traditional tummy tuck.
A tummy tuck makes sense when there’s loose skin that needs to be removed at the same time. But some patients are lean, don’t have much loose skin, and still have that persistent pooch from muscle separation.
For the right patient, Dr. Gopal can repair the diastasis using a minimally invasive robotic approach through three small incisions, about 8 mm each, hidden along the bikini line.
The key is choosing the right procedure for the right anatomy. If there’s significant loose skin or excess fat, repairing the muscles alone may actually make that looseness more noticeable.
That’s why the consultation matters. It’s not just about seeing a belly pooch. It’s about understanding what’s causing it and what actually needs to be addressed.

The last one got 11/10.
Some of the biggest factors affecting liposuction results have nothing to do with what happens in the operating room.
Dr. Gopal rates 13 factors that can influence the final result, from working out before surgery and maintaining a stable weight to compression garments, skin elasticity, and lymphatic massage.
A few of his ratings might surprise you.
For example, he rates skin elasticity a 10/10 because even after the fat is removed, the skin still needs to contract and settle over the new contour.
And lymphatic massage? He gives it an 11/10 because of the role it plays in his recovery protocol, helping manage swelling and supporting how the skin settles over the newly sculpted areas.
The takeaway: great liposuction results depend on more than the surgery itself.
Patient preparation, anatomy, recovery, and long-term weight maintenance all play a role.

No. Liposuction removes fat, while a tummy tuck removes loose skin and repairs separated abdominal muscle. If your skin still recoils and your muscle wall is intact, high-definition liposuction may be all you need.
Most desk-job patients return to work after 10 to 14 days. Swelling improves over four to six weeks, unrestricted exercise returns at six to eight weeks, and your shape keeps refining for three to six months.
Often, yes, where it refines the flanks and transitions. Florida caps liposuction combined with a tummy tuck at 1,000 cc of supernatant fat when the operation happens in a physician's office, so larger contouring may be staged into a second operation.
A 2024 analysis of 55,596 patients found a 2.1 percent major-complication rate. Earlier data tied higher risk to obesity, age 55 and over, and male sex, which is why Formation reviews your history and recent labs before setting a date.
Yes. A later pregnancy stretches the repaired muscle and skin again. The plan also waits if your weight is still dropping, including on a GLP-1 medication.
Cooler months make round-the-clock compression easier, and a school break can line up help at home. If your date falls in hurricane season, which runs through November 30, arrange a backup place to recover in case a storm cuts power.
