A PDO thread lift uses barbed absorbable sutures placed under the skin to pull a softening jawline back toward a cleaner line. The lift is there the day it’s placed. Over the months that follow, your body deposits collagen along the thread tracks, which firms the skin a little further before the threads themselves dissolve.
That’s the whole promise, and for the right jaw it’s a real one. Formation sits on South Howard Avenue in South Tampa, about 45 minutes down the Suncoast Parkway from New Port Richey. Threads are one of four tools we use on the neck and jaw, and this page is mostly about telling you which of the four your face actually needs.
Polydioxanone is a synthetic absorbable suture material that surgeons have used for wound closure since the 1980s. A PDO thread lift takes that same material, adds barbs along its length, and threads it through the tissue under your skin so the barbs catch and hold soft tissue in a slightly higher position.
Two things happen from there. The mechanical lift is immediate, because the barbs are physically holding tissue where the surgeon placed it. The biological response is slower. Your body reads the thread as something to wall off, so it builds collagen along each track. By roughly month four to nine, depending on the product, the polydioxanone has broken down and what remains is that collagen scaffold.
This matters for expectations. The lift you see in the mirror on day one is not the lift you keep. What you keep is a portion of it, held by tissue you grew. Anyone telling you threads are a facelift substitute is selling you the day-one photo.
Most necks and jawlines are aging in more than one layer at once. That’s the reason two people with the same complaint need different procedures. Dr. Grandhige reads every neck across four layers before recommending anything, and each layer has its own answer.
Fat under the chin and along the jaw blurs the border between face and neck. No thread removes fat. If a submental fat pad is what’s blurring your jawline, threads will pull on a heavy tissue envelope and the result will look strained. Fat comes out with liposuction, done in the office under a light oral sedative and local anesthesia through small incisions under the chin and behind the ears.
The platysma is the flat sheet of muscle across the front of your neck. When its edges separate with age, they show up as two vertical cords, the platysmal bands. Threads cannot reach this layer. Where bands are present, the answer is MyEllevate, a permanent suspension suture, combined with platysmaplasty. The relevant difference is durability: MyEllevate does not dissolve and can be removed if it ever needs to be. A PDO thread is gone within the year by design.
Skin that has lost recoil needs heat, not tension. Threads move tissue; they do not shrink it. This is where FaceTite or Renuvion, a helium plasma device, does work threads can’t do. If your skin doesn’t spring back when you pinch and release it under the chin, tightening comes before or instead of lifting.
Some jawlines look soft because the bone underneath them sits back, not because anything is sagging. A weak chin projection reads as a heavy neck no matter how much you lift. That’s a structural problem solved with Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed along the chin and jaw angle. Our injectables page covers that side of the read.
Threads sit between layer two and layer three. They’re the right tool when soft tissue has descended slightly, the skin still has recoil, there’s no meaningful fat pad, and you want the change without an operation. That’s a narrower window than most pages admit.
The elasticity window runs roughly from skin age 25 to 55. Inside it, tissue still responds to being repositioned and still builds collagen at a useful rate. Outside it, the barbs have less to hold and the collagen response is weaker.
You’re a reasonable candidate if your jawline has softened over the last few years, you can still see a border between jaw and neck, the skin under your chin returns when you release it, and you’re looking for refinement rather than reversal.
You’re not, and we’ll say so, if you have a genuine standing fold of skin at rest, visible platysmal bands, a substantial submental fat pad, or an expectation that threads will do what a neck lift does. Patients past what tightening and contouring can reach get referred outward. Formation stays in contact with the surgeons who do skin-excision work well, and we’d rather send you to the right one than place threads that disappoint you in eight months.
Three questions worth asking any provider before you book, here or anywhere: which thread product they use and what its cleared indication says, how many threads they’d place for your degree of laxity and why that number, and exactly what they do if a thread ends up palpable. A provider who can’t answer the third question hasn’t seen enough of them. You can compare this against our broader neck and jawline contouring options before deciding.
Clinic pages tend to say “mild swelling and bruising are possible.” The pooled data is more specific than that. A 2026 meta-analysis in Frontiers in Surgery combined 26 studies covering 2,827 thread lift patients and reported incidence rates for the common complications: swelling in 34% of cases, bruising in 26%, pain in 11%, visible or palpable threads in 10%, skin dimpling or asymmetry in 7%, and infection in about 2%.
Read those two ways. Swelling, bruising and pain are early events that resolve within a few weeks and are closer to expected tissue response than true complications. Visible threads, dimpling and thread exposure show up or persist past four weeks, and those are the ones that matter clinically because they can need correction.
The authors are direct about the limits of their own numbers. Heterogeneity across the included studies was high, and they did not separate results by thread material, so PDO sits pooled with polycaprolactone, poly-L-lactic acid and permanent polypropylene. Technique and product both move these figures. Take the percentages as a floor for the conversation, not a forecast for your face.
One more piece of accuracy. PDO threads are 510(k)-cleared as Class II absorbable polydioxanone sutures indicated for soft tissue approximation. Clearance is a finding of substantial equivalence to an existing device, not an approval, and most cosmetic lifting falls outside that specific indication. Any page calling threads “FDA-approved for lifting” is overstating the record.
Threads work through a collagen response. Collagen deposition is a nutritional and inflammatory process, which means the state your body is in when the thread goes in partly determines what you’re left with after it dissolves. Most practices treat that as irrelevant. We don’t.
Before treatment, health and diet get assessed. Protein intake, inflammatory load, sleep and medication are reviewed. In the window immediately before and after, nutrition, supplements and peptides are adjusted so the body builds the most structure it can while the material is still there doing its job. That window is short and it doesn’t come back.
The same thinking runs through recovery. Dr. Grandhige treats the procedure as 60% of the result and the post-procedure care as the other 40%. Poor aftercare can undo good technique, so we own the follow-up rather than handing you a printout.
The instrument is identical at every practice that carries it. The same barbed suture, the same cannula. What differs is the thinking around it, and that’s where results separate. You can read more about how that shapes care in our practice philosophy.
Placement happens in the office. The area is numbed with local anesthetic, entry points are marked along the planned vectors, and the threads are passed and set. You’re awake and you drive yourself home.
Expect swelling, tightness and some bruising for the first few days. Most patients are back at work within two or three days. Strenuous training and any facial massage stay off the table for one week while the barbs settle into the tissue; return to full training after that. Bruising typically clears inside one to three weeks. Swelling settles over two to four.
From there the change is slow. The mechanical lift softens slightly as tissue relaxes around the barbs, and the collagen contribution builds underneath it through roughly month three. What you see at three months is closer to your real result than anything in the first fortnight.
If a thread becomes palpable or a dimple forms, you call us and you’re seen. Most dimpling resolves on its own inside three to six months, and the rest responds to massage or a minor adjustment in the office. We follow patients for a full year, so this is a scheduled conversation, not a favour. Our before and after gallery shows how neck and jawline results mature over that timeline.
Formation is at 1315 South Howard Avenue in South Tampa. From New Port Richey the run is State Road 54 east to the Suncoast Parkway, south onto the Veterans Expressway, then into South Tampa. Roughly 34 to 38 miles and about 45 to 50 minutes outside rush hour.
Two local details worth knowing. The Suncoast Parkway and Veterans Expressway went all-electronic in 2020, so there are no cash booths; you need a SunPass transponder or you’ll be billed by plate afterward. And because thread placement uses local anesthetic rather than general anesthesia, you drive that corridor home yourself the same afternoon.
That distance also changes the logistics compared with our out-of-state patients. Pasco County patients are a day trip, so the hotel and recovery-lodging arrangements we make for patients flying in from Atlanta or Miami don’t apply to you. What does apply is the follow-up cadence, and it’s worth planning for a handful of return visits over the first year rather than one and done.
Two ways in. The complimentary virtual evaluation is a video call with Hanna, our patient coordinator, who reviews your goals and outlines a plan before you commit to driving down. The in-office consultation with Dr. Grandhige carries a fee and is a full surgical assessment: anatomical evaluation, candidacy across all four layers, and an honest read on options and limits. That fee can be applied toward any procedure or product at the practice.

The threads themselves dissolve somewhere between four and nine months depending on the product. What persists past that is the collagen laid down along the tracks, which holds a portion of the original lift. Plan on this being a procedure you'd repeat rather than one you do once.
This is the most common concern patients raise, and they usually raise it with our staff before they raise it with Dr. Grandhige. Done conservatively, the change reads as looking rested rather than as having had work. Overcorrection is what people notice, which is why thread count matters more than most patients expect.
Discomfort during placement is minor once the area is numbed; you'll feel pressure and pulling rather than pain. Pooled data puts post-procedure pain at around 11% of patients, generally manageable and short-lived.
One week. Strenuous training and facial massage both stay off for that period while the barbs anchor. After a week there's no restriction.
Call the office. It occurs in roughly one in ten cases across the published literature. Depending on where it sits, the answer is observation, massage, or a small in-office adjustment. It's a known outcome with a known response, not an emergency.
No. Those are the edges of the platysma muscle, and threads sit above that layer. Bands need MyEllevate combined with platysmaplasty. If cords are your main complaint and someone offers you threads for them, ask why.
Worth raising. Tissue fibrosis along thread tracks is documented in the surgical literature, and scar planes are less predictable than untouched ones. If a surgical neck lift is realistically on your horizon in the next several years, say so at consultation so it factors into the plan.
No. A meaningful share of people who come in asking for threads have a fat or muscle problem instead, and threads won't touch either. If everything about a consultation sounds easy and guaranteed, that's a warning sign about the practice, not a reason to book.
A PDO thread lift is a real option for a narrow set of jawlines: mild descent, intact skin recoil, no meaningful fat pad, no platysmal bands, and a patient who wants improvement without an operation and understands it isn’t permanent.
Outside that window, threads underdeliver, and the fix is knowing which of the four layers is actually driving what you see in photographs. That read takes twenty minutes and it’s the difference between a procedure that works and one you regret at month eight.
If you’re coming from New Port Richey, start with the complimentary virtual evaluation before you make the drive.
Not every patient who wants surgery is ready for it.
One patient came to Dr. Gopal wanting a tummy tuck. Over the next 18 months, she returned multiple times, having lost more weight each time. And each time, the answer was still no.
Not because she couldn’t have the surgery, but because she wasn’t where she wanted to be yet.
Instead of rushing into surgery, the focus was on helping her reach a healthier, more sustainable weight through nutritional counseling, lifestyle support, and medical weight management.
By the time she had lost 80 pounds, she was ready, both physically and mentally.
For Dr. Gopal, being a good surgeon isn’t just about knowing how to perform a procedure. It’s also about knowing when not to perform one.
Sometimes the best result starts long before the operating room.

Six hours. Hundreds of decisions.
High-definition liposuction is as much about precision as it is about technique.
Dr. Gopal can spend 6–7 hours on a single HD liposuction procedure, carefully evaluating the body’s anatomy, refining muscle definition, and making sure each contour transitions naturally into the next.
And that level of attention doesn’t change as the hours go by.
Because the final result is shaped by hundreds of small decisions made throughout the procedure.

Which one surprised you the most? 👀
Liposuction is one of those procedures people have heard a lot about, but not everything they’ve heard is accurate.
From what liposuction can actually achieve to how long results take to develop, there are plenty of misconceptions that can lead to unrealistic expectations.
Dr. Gopal Grandhige, board-certified plastic surgeon at Formation in Tampa, breaks down some of the most common beliefs about liposuction and explains what’s fact and what’s fiction.

This is not your typical liposuction. 👀
Most people think liposuction is simply about removing unwanted fat. But high-definition liposuction takes a different approach.
Before surgery, Dr. Gopal carefully maps out the patient’s natural anatomy, marking the chest, abdomen, and waist to guide where fat will be removed and where it should be preserved.
Why does that matter?
Because removing fat evenly isn’t necessarily going to create definition. HD liposuction uses strategic fat removal to highlight the underlying muscle structure and create a more sculpted, athletic appearance.
And those lines? They’re part of the planning that makes this level of precision possible.
The photos show an actual patient immediately after surgery. Swelling is still present, and the final results will develop as healing progresses.

Does fat move after lipo?
It’s a common concern, especially when someone notices weight gain in an area that wasn’t treated.
Here’s what actually happens.
The fat cells removed during liposuction are gone permanently. But if someone gains weight afterward, the remaining fat cells can still grow, including in areas that weren’t treated.
The fat didn’t move. The body gained weight.
That’s why Dr. Gopal emphasizes being at a weight patients can realistically maintain before surgery. Not the lowest weight they can reach, but one they can sustain long after recovery.

Don’t plan lipo 2 months before vacation.
If the goal is to enjoy a more sculpted, defined body next summer, the planning needs to start months before that.
Fall is one of our busiest seasons at Formation, and there’s a reason for it. Many patients are already thinking about how they want to look and feel next summer.
But getting there takes time.
At Formation, Dr. Gopal requires at least four weeks of preparation before surgery. That time is spent preparing patients physically and mentally for the procedure and recovery.
After surgery, swelling needs to go down, the skin needs to contract, and the tissues need time to settle into their new contour.
It can take 6–9 months for results to look and feel natural.
That’s why Dr. Gopal recommends planning liposuction 6–12 months ahead, especially when there’s a specific season or vacation in mind.
If next summer is the goal, now is the time to start the conversation.

How defined would you go?
Not everyone wants the same look from HD liposuction. Some prefer a smooth, subtle contour, while others want their abs to stand out.
Here’s how the three levels of definition differ:
01. Low definition: A leaner, natural-looking abdomen with subtle definition and light contouring around the underlying muscles.
02. Medium definition: More visible separation between the abdominal muscles, with added definition through the abs and obliques without a sharply etched six-pack.
03. High definition: More pronounced sculpting around the abs and obliques to highlight the underlying muscle structure and create a more chiseled, athletic look.
Dr. Gopal tailors his HD liposuction technique to the level of definition you’re looking for, while considering what’s achievable for your body.
Which would you choose: 1, 2 or 3? Tell us in the comments!

This isn’t traditional lipo. It’s HD lipo.
For this patient, getting smaller isn’t the goal.
He already works out. He already has the muscle. Dr. Gopal’s job is to use HD liposuction to bring out more of that existing definition and shape the areas around it so the body looks more athletic and defined.
That’s why the markings matter. Before surgery even begins, Dr. Gopal is mapping out how the abs, chest, arms and surrounding areas should work together in the final result.

This is the part of the job that never gets old.
There’s something special about knowing how different a patient’s life might feel on the other side.
Not just because their body may look different, but because of everything that can come with it.
Feeling more comfortable in their own skin.
Saying yes to plans without thinking twice.
Looking forward to summer instead of dreading it.
Plastic surgery can change a body. But sometimes, what matters most is how differently someone gets to experience their life afterward.
And that’s a pretty incredible part of what we get to do at Formation Tampa.
