he variable in a thread lift is the hand, not the product. Dr. Gopal Grandhige is a board-certified general surgeon who specializes in aesthetic surgery. He trained at the University of Michigan Medical School, matched into a combined plastic surgery residency at Yale, then completed a full general surgery residency along with fellowships in burn surgery, critical care, and minimally invasive metabolic and foregut surgery.
Burn and critical care surgery is where you learn how tissue behaves once it is injured. A thread lift is a controlled tissue injury. He has been operating over fifteen years across several thousand cases, all of it in Tampa, and holds his volume low enough to evaluate you, plan, place and follow up himself.
One point on the product, stated once. PDO threads are 510(k)-cleared, not FDA-approved. A 510(k) clearance means the device was shown to be substantially equivalent to something already on the market, and for most PDO products the cleared indication is soft tissue approximation rather than facial lifting. Ask these anywhere you go, including here. A provider who cannot answer all four is guessing.
How many threads, and why that number? A count with a reason behind it means someone measured your face.
Who is physically placing them, and what is the supervision arrangement? In Florida, placing threads is a medical act. A physician can delegate it to licensed clinicians within their scope, and the requirements are set by the state rather than by the clinic. Unlicensed personnel cannot do it at all. The rules sit with the Florida Board of Medicine, and arrangements vary practice to practice, so ask.
Which thread, and does its cleared indication cover facial lifting? Most providers have never read the clearance for the product in their own drawer.
Do you also operate on patients who had threads first? Only a surgeon can answer this honestly, and the answer tells you whether anyone is thinking about your face in five years or five weeks.
Threads suit mild to moderate laxity in skin that still has recoil, inside the twenty-five to fifty-five elasticity window, with most thread patients landing from the late thirties into the early fifties.
You are likely a candidate if your jawline has softened but the skin still springs back when you pinch it, you have early jowling rather than established jowls, and you want a sharper profile without weeks away from work.
Wait or do something else if there is real fullness under the chin, because that is fat. If recoil is largely gone, because threads will pull and then give out. If you want a facelift result, because no thread count produces one. Or if you are hoping to postpone a decision, in which case waiting may be the right call and you will be told so.
That is where the one-in-three figure comes from. About a third of people who book a thread consultation here are told something else fits better, or that nothing needs doing yet. The durable part of a thread result is collagen your own body builds, so what your body has to build with is a real variable.
Before treatment, Dr. Grandhige assesses your health and diet. Protein intake, inflammation, sleep and training load all get looked at, because they all feed tissue healing. In the window immediately before and after placement, nutrition, supplements and peptides get adjusted so your body builds the most structure it can while the threads are still there to build along. Your plan gets dialed in before the appointment and revisited at each follow-up.
The instrument is identical at every practice in Tampa and Sarasota. The thinking around it is not.
Plan on a few visible days and one week back to full training.
The first few days bring swelling, some bruising, and a tight pulling sensation along the thread lines. Sleep on your back, slightly elevated. By days four and five most people look normal enough for work. At one week you are back to work, back to social plans and back to training. Across weeks two to four the swelling clears and you see your true starting point. Skip saunas, steam rooms and hot yoga in that window, because heat and heavy sweating push swelling up and slow the settling. On the Gulf Coast in September that matters more than it sounds. At three months the collagen contribution starts showing, and six to nine months is the final read.
Threads can make a later facelift harder, and almost nobody selling them will say so. It matters most to the exact people threads appeal to, patients in their forties who will probably consider surgery eventually.
The mechanism is the same one that makes threads work. Your body lays down collagen along each track, and that fibrosis can persist after the polydioxanone is gone. Surgeons operating later on patients with a thread history report denser, less predictable tissue planes and longer dissection. That is not a barrier. It is a reason to think about sequence. If a facelift is realistically five or ten years out, it belongs in the conversation before the first thread goes in.
The complication picture is real but mostly temporary. A meta-analysis of facial thread lifting in Aesthetic Plastic Surgery pooling 26 studies found swelling in about 35 percent of cases, skin dimpling in 10 percent, paresthesia in 6 percent, visible or palpable threads in 4 percent, and infection and thread extrusion in 2 percent each.
Two findings inside it matter more than the headline numbers. Absorbable threads, which is what PDO is, carried a lower risk of paresthesia than non-absorbable threads, 3.1 percent against 11.7 percent, and a lower risk of extrusion, 1.6 percent against 7.6 percent. And patients over 50 ran a higher risk of dimpling, 16 percent against 5.6 percent, and of infection, 5.9 percent against 0.7 percent. Read it with the obvious limitation: pooled data blends different thread types, techniques and operators across 26 studies, so treat the figures as a range rather than a forecast.
What Formation sees most are the two at the top, temporary swelling and occasionally a small dimpled area that resolves with hands-on lymphatic work in the first few weeks.
Formation is at 1315 South Howard Avenue in South Tampa’s SoHo district. From downtown Sarasota the drive runs roughly 55 miles and just over an hour, north on I-75 to I-275 across the Skyway, then in through St. Petersburg to Kennedy and Howard. From Siesta Key or Palmer Ranch add fifteen minutes. The route crosses the Sunshine Skyway, which closes to all traffic when sustained winds hit 40 miles per hour or fog drops visibility below 100 feet. High-profile vehicle restrictions come first, then the full span, and the detour runs I-75 around the top of the bay through Hillsborough County, roughly thirty miles longer. Leave early in storm season rather than cutting it fine.
One current wrinkle on the way out of town: FDOT is rebuilding the I-75 interchange at Fruitville Road, Exit 210, into a diverging diamond with about two miles of widening from Palmer Boulevard north, and completion is estimated for fall 2026. If Fruitville is your usual on-ramp, Bee Ridge or University are cleaner until it is finished.
Because a thread lift runs on local anesthesia and takes about an hour, none of this needs an overnight. You drive up, you drive home. That changes if the four-layer read points somewhere else. For patients traveling in for surgery, Formation holds relationships with nearby hotels at reduced rates, and Dr. Grandhige visits them in their rooms during the early days so they are not driving to the office.
