What A PDO Thread Lift Does

A PDO thread lift repositions loose tissue in the cheeks, jowls, jawline and upper neck using dissolvable sutures placed under the skin through a needle. No incisions, no general anesthesia, no operating room. You see a change the same day, and most people are back at a desk the next morning.

PDO stands for polydioxanone, the same absorbable suture material surgeons have closed wounds with for decades. Barbed versions grip tissue as they are drawn through it, which is what produces the lift. Smooth versions do not lift and are used for texture.

That is the honest short version. The longer version, and the reason most Wesley Chapel patients end up changing their plan after a consultation, is that a thread lift is a good answer to one specific problem and a poor answer to three others that look similar in the mirror.

Who Threads Suit, And Who They Do Not

The candidacy window is narrower than the marketing suggests. Skin needs enough elasticity to hold a repositioned edge, which in practice means skin roughly in the 25 to 55 range. Below that, there is usually nothing to lift. Above it, tissue tends to settle back toward where it started, and a thread has no mechanism to prevent that.

You are likely a fit if the looseness is early, the change you want is modest, your skin still springs back when you release it, and your motivation is a cleaner jaw border rather than a new face.

You are likely not a fit if there is a fold of skin that hangs at rest, if you have vertical bands standing out when you tense your neck, or if the fullness under your chin is fat rather than lax skin. In those cases threads will underdeliver, and Dr. Grandhige will tell you so at the consultation rather than after.

Patients whose degree of skin excess is past what repositioning or tightening can reach get referred outward. Formation maintains relationships with surgeons who do excisional face and neck work well, and points patients to the right one. Saying no is part of the practice model, not an exception to it. What Recovery Looks Like

Expect soreness, tightness and some bruising for the first several days. Small dimples or puckering along the thread path are common early and settle as tissue relaxes. Sleep with your head elevated.

We hold you off heavy training for about a week, and off wide mouth opening, dental work, facials and facial massage for longer. Chewing tough food early works against the threads while tissue is still setting.

Recovery here is managed rather than left alone. Dr. Grandhige’s position is that the procedure is 60 percent of the result and post-procedure care is the other 40 percent, and that poor aftercare can undo good technique. Follow-up is scheduled, not optional, and patients stay in contact for a full year.

One thing we will not do is give you a durability number. The largest systematic review of thread-lift sutures, published in Plastic and Reconstructive Surgery in 2018, found that nearly all the available literature showed limited durability of the lifting effect, and that the two studies reporting otherwise were funded by the thread manufacturers. The threads themselves absorb. The collagen response is real but modest. Anyone quoting you a confident figure is quoting marketing.

The Four-Layer Neck Read: Where Threads Fit And Where They Do Not

Most jawlines and necks are aging in more than one layer at once. Sorting which layer is driving the look you dislike is the entire consultation. We run it as a four-layer read, and each layer has its own answer.

Layer One, Fat

Fullness under the chin and along the jaw that blurs the border between face and neck. Threads do not remove fat. Pulling tissue upward over a fat pad that is still there produces a tighter version of the same silhouette. When fat is the driver, the answer is neck liposuction, performed in the office under local anesthesia with an oral sedative.

Layer Two, The Platysma

The platysma is the flat sheet of muscle across the front of the neck. When its edges separate with age, they show as two vertical platysmal bands. A thread will not hold a separated muscle together. Where bands are the finding, the durable answer is a suspension suture such as MyEllevate, a permanent suspension suture, combined with platysmaplasty. Formation stays in contact with surgeons who perform that work.

Layer Three, Skin

Skin that has lost its snap-back but has no hanging fold. This is where energy-based tightening earns its place. FaceTite, a radiofrequency device, and Renuvion, a helium plasma device, reach beneath the surface and tighten from the underside. Lasers and microneedling work top down and do not get there. Threads reposition skin, they do not tighten it.

Layer Four, Frame

A weak chin or an obtuse jaw angle. No amount of lifting fixes a frame problem, because there is nothing underneath for the tissue to drape over. Structural filler is the tool: Radiesse, a calcium hydroxylapatite, or Juvederm, a hyaluronic acid, placed on bone at the chin and angle. Threads sit between layers two and three. They are a repositioning tool for mild to moderate laxity in someone whose skin still has recoil. They are not a fat tool, a muscle tool, a tightening tool, or a frame tool. Reading the layers correctly is the difference between a result you keep and a procedure you repeat.

How The Procedure Runs

Thread placement happens in the office at Formation on South Howard Avenue, not in a surgical facility. Entry points are marked on the skin, the areas are numbed with local anesthetic, and threads are introduced through a fine cannula or needle along vectors chosen for your anatomy. Tissue is drawn up along the barbs and the ends are trimmed flush.

Most appointments run under an hour. You are awake and talking throughout. You will feel pulling and pressure, not sharp pain.

Vector choice is where technique separates. In male patients, threads pulled too vertically soften the jaw and read as feminized, which defeats the reason most men come in. Dr. Grandhige plans vectors around the underlying skeletal structure and existing jaw angle, the same restraint he applies to neck and jawline contouring. How We Prepare You Before And After

Every patient at Formation gets their health and diet assessed before treatment, not just the area being treated. Nutrition, supplements and peptides are adjusted in the window immediately before and immediately after so the body builds as much structure as it is capable of building while the tissue response is active.

This matters more with threads than with almost anything else, because the lasting portion of a thread result depends entirely on what your own tissue lays down around the suture. Inflammation control, protein intake and sleep are inputs to that process, not wellness garnish.

The thread itself is the same product in every office in Pasco and Hillsborough County. Anyone can order it. What differs is the reading that happens before it goes in and the management that happens after.

Threads Compared To The Other Routes

Threads give you the lift the day it is placed and then trade that away over time. Energy-based tightening gives you nothing on day one and builds over three to six months, but it changes the skin rather than moving it. Jawline and chin filler changes the frame the tissue sits on and does not touch laxity at all. Liposuction removes what is actually there.

A permanent suspension suture is the closer comparison, and the distinction is worth understanding. MyEllevate uses a nonabsorbable suture that can be removed if needed. Barbed PDO threads dissolve on their own schedule, which means the lift is temporary by design.

None of these is the best one. The right one is whichever matches the layer driving your concern, which is why the read comes before the recommendation.

On the regulatory point, since almost every page in this market blurs it: PDO sutures are cleared through the FDA’s 510(k) pathway for soft tissue approximation, not FDA-approved. A small number of specific devices carry a narrow clearance related to mid-face suspension. Cleared and approved are different words with different evidence behind them.

Common Questions About PDO Thread Lifts

Loose tissue in the cheeks, jowls, jawline and upper neck gets repositioned along the vector the thread is placed. It does not remove fat, repair muscle separation, or tighten skin from within. Those are three separate problems with three separate answers.

Probably not. Threads suit mild to moderate laxity in skin that still has recoil, generally in the 25 to 55 range. If skin hangs in a fold at rest, repositioning it will not hold, and Dr. Grandhige will say so at the evaluation.
