Most of the stitches from a tummy tuck never come out.
If you have stitches on the skin surface, expect them out somewhere between day 7 and day 21, with the belly button often cleared first. Everything under the skin either dissolves on its own or stays in permanently. So the real answer to how long stitches stay in after a tummy tuck depends on which stitches you’re asking about, and there are four separate answers.
Nobody explains that at the pre-op visit. You get a folder, a garment, and a follow-up date.
This article covers each layer, what moves the date earlier or later, and the one thing that matters more than the date itself. We’re skipping scar products and drain output. Both deserve their own space.
A tummy tuck closes in layers, and each layer gets a different stitch with a different ending. Stitches on the skin surface come out within about one to three weeks. Stitches under the skin dissolve over weeks to months. Stitches holding the abdominal muscles back together are often permanent and stay for life.
That layered closure is the whole point. Each layer carries part of the load so the skin isn’t holding your abdomen together by itself. Spread the tension across four levels and the scar heals flatter.
One stitch type doing all of that work would fail. Skin alone can’t hold a repaired abdominal wall, and a heavy muscle stitch can’t give you a fine scar line.
It also means “when do my stitches come out” is four questions wearing one coat. Patients who understand that stop panicking when they feel a small bump at month three. If you want the wider view, the healing timeline runs longer than most people plan for.

Only the fourth level is visible to you. The other three are doing the structural work, which is why what you can see says very little about what’s actually holding you together.
The muscle repair is the layer you feel most. New tension across your core makes bending, coughing and standing upright feel foreign for a while.
That’s also why nobody pulls stitches early as a favor.
Seven to twenty-one days, with most practices landing at ten to fourteen. The Aesthetic Society’s aftercare guidance puts external suture removal inside the first week or two, and notes that absorbable stitches don’t need removing at all. The same page is careful to say recovery time varies a great deal between individuals, which is the honest caveat on any number you read.
Abdominal skin gets a longer window than facial skin for one reason. Tension. A face has almost none. A fresh abdominoplasty closure pulls every time you stand, so taking stitches out at day five can let the edges drift apart.
Leave them in too long and you trade one problem for another. Suture tracks. Small dotted scars beside the main line that don’t fade well.
The belly button often comes out first, around day seven to fourteen. It’s a smaller circle under less pull than the long horizontal line, and it’s the most visible part of your result, so it tends to get the earliest release.
Absorbable stitches usually need nothing from you.
Once in a while one works its way to the surface and pokes through, which patients call a spitting stitch. It gets trimmed in the office in under a minute and means nothing about your result.
By definition, absorbable stitches lose most of their strength over a period ranging from a few weeks to several months, per NIH reference material on suture types. That range is why a bump can surface at week two or at month four and both are normal.
One more thing worth knowing. Trials comparing absorbable against non-absorbable skin closure found no major difference in scarring, complications or patient satisfaction. The published limitation is that those trials looked at general skin closure rather than abdominoplasty, where the tension across the incision is far higher.
Sometimes, and staples turn up more often than patients expect. Surgeons frequently staple the incision closed as a temporary step, check the alignment, then take the staples out and place the real layered sutures underneath. Those staples never leave the operating room.
When staples do stay on as the surface closure, they come out on a schedule close to external stitches. Usually inside the first two weeks, and well before most people return to work.
Removal feels odd rather than painful. A quick pinch each, and a full line takes a couple of minutes.
They’re load-bearing for life. Rectus plication uses permanent or long-acting absorbable sutures, according to the NIH clinical reference on abdominoplasty, because that repair has to hold against every cough and sit-up you’ll ever do.
Permanent material carries one trade-off that rarely gets discussed before surgery.
Suture extrusion sits on the published list of local complications. A permanent stitch can travel toward the surface years later and cause a small irritated spot that needs an office visit to settle.
The same reference gives an overall complication rate of 10 to 20 percent, rising to 30 to 50 percent in patients after massive weight loss. Read those figures with the stated limitation in mind, because they pool very different patient groups rather than describing one cohort.
So is permanent the wrong choice? No. It means you should ask what went into your abdominal wall and write it down, because in ten years nobody will remember.
The edges can separate. Not by much, usually, but enough to widen your scar for good.
This is where I’ll push back on how patients treat the removal appointment. People circle it like a finish line. It isn’t one. Your closure keeps healing for months after the last stitch is gone, and the scar has only a fraction of its final strength on the day the skin looks sealed.
Dr. Grandhige is blunt about this. Surgery is roughly 60 percent of the result and post-op care is the other 40, and poor post-op care can undo a well-performed operation. Stitch removal doesn’t change that math.

Two patients can have identical surgery and different removal dates. Your surgeon reads the incision, not the calendar.
Several things push the date later.
Nicotine narrows the vessels feeding your incision, and less blood at the edges means slower closure and a longer wait. The same NIH reference recommends stopping at least four weeks before and after surgery. Poorly controlled diabetes works against wound healing in a similar way.
Weight that swings hard in either direction during the first months creates its own problem, because the closure is being asked to hold a shape that keeps changing underneath it.
Both are reasons we’d rather sort things out before you’re on the table, which is most of what you’re doing when you prepare before surgery.

Not usually, because they are separate decisions. Drains typically come out 3 to 14 days after surgery based on how much fluid is draining, while external stitches come out on the schedule your incision supports. The windows overlap, so sometimes both happen at one visit.
Most patients can shower within a few days of surgery, but the instruction depends on your dressings and whether you have drains. Baths, pools and hot tubs wait much longer, because soaking a fresh incision raises infection risk. Follow the written instruction you were given rather than a general timeline.
