You can reduce jaw size without surgery, but only when the cause is the right one. If your lower face looks wide because the chewing muscle at the back corner of the jaw is oversized, Botox (botulinum toxin) placed into that muscle will slim it. The muscle thins from reduced use over several weeks, and the jaw narrows. When the width comes from fat, loose skin, or the shape of the bone, injections won’t touch it. Sometimes they make things look worse.

A wide lower face comes down to five things: muscle, fat, skin, bone, and clenching habits. Each one has its own fix, and telling them apart is most of the work.
Get that read wrong and you spend months on a treatment that was never going to help.
The masseter is the usual reason a jaw looks wide on someone lean and fit. It’s the muscle you feel bulge when you bite down hard. Some people are born with a large one. Others build one over years of gum, heavy chewing, or nighttime clenching. This is the single cause botulinum toxin treats well. Shrink the muscle, narrow the jaw.
Sometimes the fullness isn’t muscle at all. It’s fat sitting under the chin and along the jaw line. Botox does nothing to fat. A small liposuction under the chin removes it directly and sharpens the border between jaw and neck in one visit. Younger patients often carry soft fullness here at a stable weight, and it rarely responds to more cardio. Older patients develop it as the face loses support.
Skin that has lost its snap hangs and blurs the jaw line. That reads as width in photos. It’s really laxity, and injecting a muscle won’t lift it. Loose skin responds to a skin tightening treatment like FaceTite, a radiofrequency device that firms tissue from underneath rather than from the surface. Skin quality is why two people with identical masseters get different results from the same injection.
Sometimes, yes. A broad mandible is structural, and no injection changes bone. This is the one situation where surgery is the honest answer. I would rather say that out loud than sell someone repeat injections that were never going to reach the problem.
Grinding and clenching, usually at night, overwork the masseter until it grows. Here the injections do double duty. They soften the jaw and ease the muscle strain sitting behind it. Patients often report less morning soreness and fewer tension headaches after Botox for jaw tension. That relief is real, even when the photos were the reason they booked.
Botox reduces jaw size by weakening the masseter so the muscle slowly loses bulk. Less muscle means a narrower lower face. The visit is short, and nothing gets cut.

The masseter can no longer contract at full force, so it thins from disuse, the same way any muscle thins when you stop training it. A 2025 review of masseter injections found the toxin reliably reduces masseter volume and improves both the shape of the lower face and symptoms like jaw pain, though the authors point out the field still hasn’t agreed on a standard dose. That’s why the number of units depends on your muscle and your injector’s read of it, not a fixed menu.
The appointment runs about fifteen minutes. No anesthesia beyond the pinch of the needle. You drive yourself home and go back to work the same afternoon.
That’s the appeal. Fast, no scars, reversible if you hate it. It’s also the catch. Reversible means temporary.
You’re a good candidate if a large masseter is driving the width and your skin still has decent tone. You’re not if the fullness is mostly fat, loose skin, or bone.
The best results go to people already lean and training hard who carry a square, heavy jaw that won’t budge. A lot of them clench or grind without knowing it, and find out when a partner or a dentist tells them.
Try this at home. Press the back corner of your jaw, just in front of the earlobe, and bite down. Feel a firm bulge push against your fingers? That’s the masseter, and that’s what masseter Botox thins. Feel something soft that shifts under the pressure? That’s fat, and fat needs a different tool entirely.
Skin tone matters as much as muscle size. Springy skin tightens down over the smaller muscle and the jaw looks cleaner. Skin that has already loosened does not, and that’s where this treatment starts working against you.
Botox is the wrong choice more often than the marketing admits. Shrinking a healthy muscle in the wrong face makes it look older, not slimmer.
The masseter gives the lower face some of its structure. Take bulk out of it in someone with early skin laxity or heavy cheeks, and the tissue sitting over it has less to rest on. The jaw narrows and the face looks tired at the same time. Some people trade a wide jaw for early jowls.
So the read comes before the needle. When a weak chin or a soft jaw angle is the real issue, projection defines the line better than shrinking anything ever could. Radiesse, a calcium hydroxylapatite, and Juvederm, a hyaluronic acid, both work as jawline filler to build a frame the face never had. When the problem is skin or fat, the fixes above apply. And sometimes the right call is nothing at all, or a referral to a surgeon who does jaw bone work.
Saying no is part of the job. The best result is the one nobody can point at, and you don’t get there by treating every wide jaw the same way.
Botox slims a muscle-driven jaw with no downtime and wears off. Surgery reshapes bone permanently and carries anesthesia, recovery, and real risk. The cause decides which one is even on the table.
Jaw reduction surgery shaves the mandible angle and permanently changes the skeleton underneath. It’s the only option when bone is the problem, and it means general anesthesia, weeks of recovery, and the risks that come with any operation.
For a muscle-driven jaw, most people start with injections because the commitment is small. Like the result, and you keep going. Skip it, and it fades on its own. If bone is the real driver, no amount of Botox substitutes for an operation, and repeat treatments just delay that conversation.
Expect a quick set of injections, near-zero downtime, and a change that arrives over weeks. Nothing looks different on the day.
Jaw slimming Botox builds slowly because the muscle has to lose bulk from reduced use, and tissue takes its time.
Chewing strength can dip early and returns as the muscle recovers. Hard steak and bagels are the usual complaints, and the usual answer is patience. The look itself should stay quiet.
Not yet for the jaw. Masseter injections are used off-label, and in 2026 that status finally started to move.
Botox Cosmetic is approved for frown lines, forehead lines, crow’s feet, and, since October 2024, the vertical neck cords called platysma bands. Masseter prominence isn’t on that list. Doctors can still use an approved drug outside its labeled use, which is what off-label means. It happens across medicine every day.
That may change soon. On August 4, 2026, the FDA accepted Allergan’s application for review, backed by two Phase 3 studies that met their primary endpoints for masseter prominence compared with placebo. Accepted for review is not approved. The agency hasn’t ruled, no timeline is public, and until a decision lands the jaw remains an off-label use.
Any provider who tells you it’s already approved for the jaw is either behind on the news or hoping you are.
