That’s the honest short version. The longer version comes with a caution I give patients at Formation before anything else. The category is wider than most people assume. Fat-dissolving injections under the chin belong here. So do muscle-relaxing injections along the jaw and volume replacement in the midface. If you’re weighing both sides, the line between non-surgical and surgical facial contouring options is simple: non-invasive treatments change how tissue behaves, surgery changes where tissue sits.
One is a nudge. The other is a move.
Demand for non-surgical tightening has climbed hard, and the growth is uneven depending on which dataset you read. The ISAPS Global Survey for 2024 counted 1,239,306 non-surgical skin tightening procedures worldwide, up 38.9% over the prior year. The United States alone accounted for 356,072 of those, roughly 29% of the global figure and more than any other country.
Then look at the domestic numbers and the story changes. ASPS procedural statistics recorded 439,032 nonsurgical skin tightening treatments in 2024 against 438,211 in 2023. Flat. Zero growth.
Two credible sources, two different pictures. My read is that the global jump reflects newer markets buying devices for the first time, while the American market matured years ago and now just cycles patients through whatever platform their provider purchased most recently. If you’re in Tampa, treat the 38.9% headline with some skepticism. It isn’t describing your neighborhood.
What does describe your neighborhood: Florida sits in the ASPS South Atlantic region, which logged 7,798 submental liposuction cases in 2024, roughly 32% of every chin lipo performed in the country. The same region accounted for only 20% of the nation’s non-surgical skin tightening. Read those two figures together and a pattern falls out. Patients here care about the jaw and neck more than almost anywhere else, and they cross into surgery to fix it more readily than the national average.
The other driver is medication. ASPS members prescribed weight loss drugs to 837,485 patients in 2024, and 41% of those patients said they were considering nonsurgical procedures. Rapid weight loss empties the face before it tightens it. A lot of what walks into consults now is volume loss wearing a laxity costume.
One more number deserves your attention. Noninvasive fat reduction, the freeze-and-melt category, fell 40% in a single year, from 745,967 treatments down to 447,581. Patients stopped believing it. Skin tightening hasn’t had that reckoning yet, and part of why I write pieces like this is that I think a smaller version of it is coming.
Three mechanisms, and they are not interchangeable. Picking the wrong one is the single most common reason people tell me non-invasive facial contouring “didn’t do anything.”
Radiofrequency and focused ultrasound both push heat into layers beneath the skin surface. The tissue registers that heat as a small, controlled injury and repairs itself with fresh collagen and elastin. Ultrasound reaches deeper. Radiofrequency is generally more comfortable.
Expect a series, not a single visit. Most protocols run three to six sessions spaced a few weeks apart, and the visible change shows up around month three. Skin quality improves before contour does. Anyone promising you a jawline in one appointment is selling, not planning. This is the family that covers most non-surgical skin tightening work on the lower face and neck.
Injectables are the workhorse of the category, and the volume numbers dwarf everything else. ASPS counted 9,883,711 neuromodulator treatments in 2024, up 4%, plus 5,331,426 hyaluronic acid filler procedures. Nothing else in aesthetics comes close.
Filler restores structure where fat pads have thinned. Neuromodulators soften muscle pull along the jaw and neck. Fat-dissolving injections reduce the pad under the chin. Each does one job well and none of them tighten loose skin, which is where patients get confused. Choosing between injectables and fillers comes down to whether you’ve lost volume or lost muscle balance.
Absorbable threads are placed under the skin with a needle and anchored so the tissue sits higher. There’s an immediate lift, then the threads dissolve over several months while collagen forms along the tracks. A thread lift is the closest thing in this category to an actual mechanical change, which is exactly why it carries more bruising and more technique dependence than a device pass.
Threads are also the most operator-sensitive option here. Same product, two injectors, wildly different results.
The best candidates are in their thirties to early fifties with mild to moderate laxity and skin that still snaps back when you pinch it. Pinch the skin under your jaw and let go. If it returns instantly, energy and threads have something to work with. If it hangs, they don’t.
Now the part the industry doesn’t advertise.
Non-invasive facial contouring is sold constantly as a facelift alternative. It is not one. In one published series of 50 patients treated with monopolar radiofrequency, 52.9% reported that their skin laxity improved. Call it close to a coin flip on a face that already has decent elasticity. That’s a fair trade for someone who wants refinement. It’s nothing at all on a face that needs skin removed, and device manufacturers understand that difference even when their marketing blurs it.
Poor candidates share a few features:
I turn people away from these treatments regularly. Not because they’re bad treatments. Because a wrong-fit patient will spend a year on sessions, see almost nothing, and arrive at surgery angrier and poorer than if we’d been honest in week one. When skin folds instead of springing back, surgery for loose neck skin is the conversation to have, not another package of sessions.
Plan on subtle improvement that holds for roughly one to two years, then needs maintenance. One 65-patient series following bipolar radiofrequency of the lower face and neck found 90.8% of patients still satisfied at the one-year mark. So results don’t simply evaporate.
Actually, that framing isn’t quite right. Results fade, yes, but aging keeps moving underneath them at the same time. You’re not maintaining a fixed result. You’re running slightly ahead of a moving line. Patients who understand that stay happy for years. Patients who expected a permanent fix get frustrated around month fourteen and blame the treatment.
The most common outdated advice I still hear repeated in Tampa consults is that one session lasts forever. It never did.
Timing trips people up too. If you have a wedding in six weeks, this is the wrong tool. Collagen doesn’t care about your calendar.
The research supports high satisfaction and modest measurable change, and those two findings sit together more awkwardly than the brochures suggest. A 2025 systematic review in Aesthetic Surgery Journal Open Forum pooled 15 studies and 1,230 patients across monopolar, bipolar, fractional, and multipolar radiofrequency. Satisfaction ran from 82% to 100% across 13 studies. No serious or permanent complications turned up anywhere in the pooled group. Mean pain scored 1.94 out of 10.
Now the parts nobody quotes.
Skin firmness improved in 52.9% to 100% of patients, but that range comes from only two studies totaling 90 people. The lifting effect, the thing patients actually want, rests on a single pilot study of ten participants. Not one of the 15 studies measured downtime. The authors flag likely publication bias, note that several included studies were manufacturer-sponsored, and rate their own confidence as moderate at best.
I’d still recommend radiofrequency to the right patient tomorrow. But “82% to 100% satisfied” and “we have solid proof this lifts your jawline” are two different claims, and only one of them is supported. The honest summary is that patients reliably feel better about their skin, while the measurable structural change is smaller and less certain than the satisfaction scores imply.
That gap isn’t a scandal. It describes most of aesthetic medicine. It’s also why a provider who quotes you satisfaction rates without mentioning effect size is telling you half of something.
These treatments are low-risk, not no-risk, and the gap between those two words matters. On October 15, 2025, the FDA issued a safety communication on radiofrequency microneedling after reports of burns, scarring, fat loss, disfigurement, and nerve damage. Some of those cases required surgical repair.
That doesn’t make radiofrequency microneedling dangerous. Millions of passes happen without incident. It does mean the “lunchtime treatment” framing is marketing, and that operator training matters more than the logo on the handpiece. The FDA’s own language is blunt about this. These are medical procedures, they belong in the hands of licensed and trained health care professionals, and at-home use of the devices is not authorized.
The risk almost nobody asks about is fat atrophy. Deeper energy can damage the fat pads that give a face its structure, and that loss doesn’t grow back. It reads as gauntness. Correcting it means filler, fat grafting, or surgery, and it usually costs more time than the original treatment ever did. Patients already thin in the midface carry the most risk, which is exactly the group showing up after rapid weight loss asking for tightening.
Ask your provider one question before any deep energy treatment: what happens to my fat pads. If the answer is vague, walk.
Surgery wins when the problem is tissue that needs to be removed or repositioned, not tissue that needs to be stimulated. That’s the whole test.
Loose neck skin that folds when you look down. A submental fat pad that stays put no matter how lean you get. A jawline blurred by anatomy rather than by skin quality. Energy devices will nibble at those. Neck and chin liposuction with skin tightening resolves them in one procedure with about a week of visible recovery.
ASPS members performed 24,000 submental liposuction procedures nationally in 2024, and nearly a third of them happened in the South Atlantic region. That is not a fringe operation. It is the standard answer for a problem that non-surgical treatment keeps getting asked to solve.
This is where Formation diverges from most places offering these services. Men also made up 12% of neck lift patients, a higher share than they hold in almost any other facial procedure.
Men’s faces punish over-treatment differently. Too much filler in the midface feminizes the cheek. Over-tightening the lower face erases the squared angle that reads as masculine. Neck and jawline contouring for women runs the opposite direction, planned around softer transitions and midface support rather than a hard mandibular line.
The goal in male neck and jawline contouring is almost always definition along the mandible and a clean submental line, not lift. Most men in their forties who come to me want to look rested on video calls, not younger. Those are different requests, and the second one leads to worse decisions.
Credentials and facility standards matter more than which device sits in the room. Every clinic in Tampa has access to roughly the same equipment. What differs is judgment.
Ask these five things:
Question four is the one that separates a clinic from a sales floor. A provider willing to talk you out of something is worth more than one with a longer treatment menu.
One more filter, and it costs you nothing to apply. Ask how many sessions the plan includes before anyone talks about money. If the answer arrives as a package number instead of a treatment rationale, you’re being sold inventory.
